Relationships between upper extremity neuromuscular function and patient-reported outcomes among individuals with a history of glenohumeral labral repair.

BACKGROUND: Patient-reported outcomes (PROs) are essential components of contemporary healthcare practices, which allow clinicians to effectively monitor clinical outcomes. Objectively measured neuromuscular impairments have been reported among individuals with glenohumeral labral repair, yet their relationship with PROs is unclear. Understanding those relationships would help clinicians explain clinical meaning of such impairments from a patient-oriented perspective. The objective of this study was to determine the relationships between objective metrics of upper extremity neuromuscular funct

Muscle Fibrosis in Amyotrophic Lateral Sclerosis: Molecular Mechanisms, Diagnostic Advances, and Therapeutic Strategies.

Amyotrophic lateral sclerosis (ALS) is a fatal neurodegenerative disorder primarily characterized by the degeneration of motor neurons. However, the pathological process of ALS extends beyond the central nervous system, with dynamic changes in skeletal muscle playing a crucial role in the progression of the disease. Recent research has shown that muscle fibrosis, marked by the abnormal accumulation of extracellular matrix (ECM), leads to reduced muscle elasticity, compromised contractile function, and impaired regeneration of neuromuscular junctions (NMJs). This condition represents not only t

Influence of menstrual cycle on autonomic nervous system, muscular strength and mood states.

This study examined modulations of the cardiac autonomic nervous system, neuromuscular performance and psychological states during the menstrual cycle (MC) in naturally menstruating women. Fifteen healthy young females were tested during menstruation, estimated late follicular, and estimated mid-luteal phases. Each session included mood and motivation assessments, resting heart rate variability (HRV) analysis and a maximal voluntary isometric contraction test of the knee extensors. Our data revealed a significant decrease of the parasympathetic activity reflected by lower vagally derived HRV i

Biological Mechanisms Involved in Muscle Dysfunction in COPD: An Integrative Damage-Regeneration-Remodeling Framework.

Skeletal muscle dysfunction is a major systemic manifestation of COPD that shapes symptoms, exercise tolerance and mortality. Current evidence can be integrated within a Damage-Regeneration-Remodeling framework linking mechanics and biology to clinical phenotypes. Pulmonary hyperinflation and chest wall geometry chronically load the diaphragm and other respiratory muscles in COPD, whereas inactivity and exacerbation-related disuse underload locomotor muscles. Across muscle compartments, oxidative/nitrosative stress, activation of proteolytic pathways, mitochondrial and endoplasmic reticulum st

Patient Reported Outcome Measures in Adult Spinal Muscular Atrophy: A Scoping Review and Graphical Visualization of the Evidence.

BACKGROUND: Spinal Muscular Atrophy (SMA) is a hereditary neuromuscular disease with an estimated prevalence of 1/10 000 births. SMA is increasingly recognized as a multi-system disease with a need to study additional under-recognized health domains such as quality of life, fatigue, bulbar function, respiratory function, and independence. OBJECTIVE: Identify and assess reported evidence from the literature investigating Patient Reported Outcome Measures (PROMs) in adults with SMA. Develop a novel method drawing from network theory to graphically depict the literature, PROMs, and supporting ps

Recurrent laryngeal motor nerve conduction studies in a rat model: Establishing an objective measure for investigating laryngeal innervation.

INTRODUCTION/AIMS: Disease or injury can cause neuromuscular changes to the larynx that can affect voice, breathing, and swallowing. Motor nerve conduction studies have had limited use in the study of laryngeal neurophysiology, despite their importance in other anatomic sites. The aim of this study was to explore the feasibility of performing recurrent laryngeal motor nerve conduction studies (rlMNCS) in a rat model. METHODS: rlMNCS were performed in 15 rats under anesthesia. A bipolar stimulating electrode was placed on the recurrent laryngeal nerve (RLN) 5 mm below the cricoid cartilage. Vi

Comparison of postoperative pulmonary complications between sugammadex and neostigmine in lung cancer patients undergoing video-assisted thoracoscopic lobectomy: a prospective double-blinded randomized trial.

BACKGROUND: Reversal of neuromuscular blockade (NMB) at the end of surgery is important for reducing postoperative residual NMB; this is associated with an increased risk of postoperative pulmonary complications (PPCs). Moreover, PPCs are associated with poor prognosis after video-assisted thoracoscopic surgery (VATS) for lobectomy. We compared the effects of two reversal agents, sugammadex and neostigmine, on the incidence of PPCs and duration of hospital stay in patients undergoing VATS lobectomy. METHODS: After VATS lobectomy was completed under neuromuscular monitoring, the sugammadex gro…

Enteral Nutrition Safety With Advanced Treatments: Extracorporeal Membrane Oxygenation, Prone Positioning, and Infusion of Neuromuscular Blockers.

This review aims at assessing the safety and efficacy of enteral nutrition in critically ill patients receiving extracorporeal membrane oxygenation, prone positioning, and infusion of neuromuscular blockers. Existing data from randomized controlled trials demonstrate the survival benefit of early enteral nutrition in critically ill patients. Observational data have demonstrated that enteral nutrition in patients receiving extracorporeal membrane oxygenation, prone positioning, and infusion of neuromuscular blockers is generally safe. However, these patients are at increased risk for gastrointe

A Method for Cranial Nerve XI Silencing During Surgery of the Foramen Magnum Region: Technical Case Report.

BACKGROUND AND IMPORTANCE: Skull base surgery involves the microdissection and intraoperative monitoring of cranial nerves, including cranial nerve XI (CN XI). Manipulation of CN XI can evoke brisk trapezius contraction, which in turn may disturb the surgical procedure and risk patient safety. Here we describe a method for temporarily silencing CN XI via direct intraoperative application of 1% lidocaine. CLINICAL PRESENTATION: A 41-yr-old woman presented with symptoms of elevated intracranial pressure and obstructive hydrocephalus secondary to a hemangioblastoma of the right cerebellar tonsil

The feasibility of sugammadex for general anesthesia and facial nerve monitoring in patients undergoing parotid surgery.

The use of neuromuscular blocking agent (NMBA) during anesthesia may interfere with facial nerve monitoring (FNM) during parotid surgery. Sugammadex has been reported to be an effective and safe reversal of rocuronium-induced neuromuscular block (NMB) during surgery. This study investigated the feasibility and clinical effectiveness of sugammadex for NMB reversal during FNM in Parotid surgery. Fifty patients undergoing parotid surgery were randomized allocated into conventional anesthesia group (Group C, n = 25) and sugammadex group (Group S, n = 25). Group C did not receive any NMBA. Group S

Effects of strength training in fibromyalgia on balance, neuromuscular performance, and symptomatic analysis: a 12-week study protocol.

UNLABELLED: Fibromyalgia affects skeletal muscles presenting with muscle stiffness, pain, and fatigue. The exercise practice is wellstabilized and suggested for the reduction of symptoms. However, there are some gaps in the literature related to balance and neuromuscular performance in a strength training protocol. The aim of this study is to structure a protocol to verify the effects of strength training applied over a short period, on balance, neuromuscular performance, and fibromyalgia symptoms. Additionally, we intend to analyze the effects of a short period of cessation of training. Parti

Video-Based Biomechanical Analysis Captures Disease-Specific Movement Signatures of Different Neuromuscular Diseases.

BACKGROUND: Assessing human movement is essential for diagnosing and monitoring movement-related conditions like neuromuscular disorders. Timed function tests (TFTs) are among the most widespread types of assessments due to their speed and simplicity, but they cannot capture disease-specific movement patterns. Conversely, biomechanical analysis can produce sensitive disease-specific biomarkers, but it is traditionally confined to laboratory settings. Recent advances in smartphone video-based biomechanical analysis enable the quantification of three-dimensional movement with the ease and speed

Influence of using individualised ergonomic handles on upper limb kinematics and neuromuscular activity, perceived comfort and performance during scull rowing.

Articular stress, discomfort exposure and the use handles that are not adapted to the task-specific grip during repetitive movements may increase the upper limb injuries risk during boat rowing. Therefore, the purpose of this study was to introduce and evaluate the impact of two individualised ergonomic handles on comfort, upper limb's biomechanics, and performance in scull rowing, compared to standard handles. These two individualised handle designs were based on the irregular hexagon, and one was further individualised with the rower's comfort feedback. Perceived comfort, upper limbs kinemat

Biasing effect of the electromyogram on BIS: a controlled study during high-dose fentanyl induction.

OBJECTIVE: A biasing effect of the electromyogram (EMG) on the Bispectral Index (BIS) may explain discrepancies in previous studies assessing BIS in the presence of neuromuscular activity. Our aims were: to evaluate variations of BIS in the presence of high EMG activity associated with muscular rigidity after administration of high-dose fentanyl; to compare muscular rigidity, as measured by the EMG variable of the BIS monitor, in patients who were administered two different dosages of fentanyl at induction of cardiac anaesthesia. METHODS: 26 patients undergoing CABG surgery, after premedicati

From Local Interventions to Systemic Modulation in Expressive and Neuromuscular Medicine: A Conceptual Reframing of Clinical Action.

RATIONALE: Clinical interventions in expressive and neuromuscular medicine have become increasingly precise at the local level, targeting specific muscles, doses, or anatomical sites. At the same time, the outcomes used to judge clinical success are frequently global, encompassing expression, pain, function, or behaviour. This mismatch between localised inputs and system-level outcomes reflects a common clinical reality that remains insufficiently addressed by conceptual models grounded primarily in local corrective logic. AIMS AND OBJECTIVES: The aim of this article is to clarify the concept
