Predictive value of early neuroimaging, pulsed Doppler and neurophysiology in full term infants with hypoxic-ischaemic encephalopathy.

To evaluate their prognostic value, five different non-invasive techniques were used on 34 full term infants with hypoxic-ischaemic encephalopathy (HIE) within six hours of delivery. Cranial ultrasonography, the resistance index (RI) of the middle cerebral artery obtained with Doppler ultrasonography, somatosensory evoked potentials (SEPs), visual evoked potentials (VEPs) and the cerebral function monitor (CFM) were used. According to the criteria of Sarnat, 11 infants developed mild, seven moderate, and 16 severe encephalopathy. The CFM had the highest positive (PPV 84.2%) and negative predic

The role of subcortical brain tissue iron as an indicator of dopamine neurophysiology in adolescent cannabis use.

Approximately 10-20% of U.S. adolescents report past-year cannabis use (CU). Although regular CU beginning in adolescence is expected to blunt dopamine-related neurophysiology, this hypothesis has not been tested in adolescents due to methodological limitations. However, neurophysiology contributing to dopamine can be noninvasively indexed via subcortical tissue iron measured with magnetic resonance imaging (MRI). We examined adolescent CU quantity, frequency, and problems in relation to tissue iron in regions with high dopamine activity, hypothesizing that greater CU would be linked to less t…

Sensory Dysfunction in ALS and Other Motor Neuron Diseases: Clinical Relevance, Histopathology, Neurophysiology, and Insights from Neuroimaging.

Background: The clinical profiles of MNDs are dominated by inexorable motor decline, but subclinical proprioceptive, nociceptive and somatosensory deficits may also exacerbate mobility, dexterity, and bulbar function. While extra-motor pathology and frontotemporal involvement are widely recognised in motor neuron diseases (MNDs), reports of sensory involvement are conflicting. The potential contribution of sensory deficits to clinical disability is not firmly established and the spectrum of sensory manifestations is poorly characterised. Methods: A systematic review was conducted to examine th

Synergistic association of Aβ and tau pathology with cortical neurophysiology and cognitive decline in asymptomatic older adults.

Animal and computational models of Alzheimer's disease (AD) indicate that early amyloid-β (Aβ) deposits drive neurons into a hyperactive regime, and that subsequent tau depositions manifest an opposite, suppressive effect as behavioral deficits emerge. Here we report analogous changes in macroscopic oscillatory neurophysiology in the human brain. We used positron emission tomography and task-free magnetoencephalography to test the effects of Aβ and tau deposition on cortical neurophysiology in 104 cognitively unimpaired older adults with a family history of sporadic AD. In these asymptomatic i…

Transcranial magnetic stimulation neurophysiology in patients with non-Alzheimer's neurodegenerative diseases: A systematic review and meta-analysis.

Non-Alzheimer's dementia (NAD) accounts for 30% of all neurodegenerative conditions and is characterized by cognitive decline beyond mere memory dysfunction. Diagnosing NAD remains challenging due to the lack of established biomarkers. Transcranial magnetic stimulation (TMS) is a non-invasive neurophysiological tool that enables the investigation of cortical excitability in the human brain. Paired-pulse TMS paradigms include short- and long-interval intracortical inhibition (SICI/LICI), intracortical facilitation (ICF), and short-latency afferent inhibition (SAI), which can assess neurophysiol

Design and implementation of a low cost, modular, adaptable and open-source XYZ positioning system for neurophysiology.

In recent years, open-source 3D printing technologies have become increasingly applied to biological research. We have created a fully open-source, versatile and low cost XYZ positioning system using 3D printer components. As this system is controlled by a Python3 based operating system running on a Raspberry Pi 3 Model B, its behaviour can be adapted to meet multiple needs in neurophysiology. We have developed two main applications of this system. First, we have created an automated microscopy script that links seamlessly with image stitching plugins in ImageJ (Fiji) allowing the user to crea…

Cerebellar neurophysiology in Gilles de la Tourette syndrome and its role as a target for therapeutic intervention.

Therapy resistance of approximately one-third of patients with Gilles de la Tourette syndrome (GTS) requires consideration of alternative therapeutic interventions. The article demonstrates the role of the cerebellum in neuropsychiatric disorders and GTS in particular, specifically its role in functions relating to motor and cognitive symptoms. Certain circuits in the cerebellum have been shown to undergo learning-induced changes during conditioning, with cells in the cortex of the cerebellum appearing to decrease their activity whilst those in deep nuclei seem to do the inverse. Evidence exis

Upgaze eyelid position allows differentiation between congenital and aponeurotic blepharoptosis according to the neurophysiology of eyelid retraction.

To differentiate between congenital and aponeurotic blepharoptosis, we investigated whether upgaze with stretching of the mechanoreceptor of Mueller muscle increases involuntary reflex contraction of the levator slow-twitch muscle fibers. In 50 cases each of unilateral congenital blepharoptosis and of asymmetric aponeurotic blepharoptosis, the mean increases by upgaze in the upper eyelid margin to the line between the medial and lateral canthi as upper eyelid retraction distance (UERD) of the ptotic eyelid 0.4 mm and 2.9 mm, respectively. Elicitation and habituation of the electrodermal activity responses: Implications for neurophysiological applications.

Electrodermal activity (EDA) serves as a reliable indicator of sympathetic activation for the measurement of emotional arousal. Despite extensive use of EDA in psychophysiological and neurophysiological research, little is known about how repeated stimulus presentation influences EDA responses, particularly when more than one parameter is measured simultaneously. This study examined the influence of repeated stimulus presentation on three EDA parameters (SCRs, SPRs, and SSRs). A total of sixty healthy subjects participated in the study. They were exposed to four external stimuli: sound, image,

Anxiety and neurophysiological correlates of stuttering severity in adults who stutter.

BACKGROUND: Stuttering has traditionally been conceptualized as a motor speech disorder; however, accumulating evidence suggests that auditory processing differences and emotional factors may also contribute to its clinical presentation. Despite increasing interest in these domains, the interplay between auditory cortical processing, stuttering severity, and anxiety in adults remains incompletely understood. OBJECTIVE: This study aimed to examine auditory evoked cortical responses (P1, N1, P2, and mismatch negativity [MMN]) in adults who stutter compared with fluent-speaking controls, and to

Neurophysiological Resting-State Markers of Catatonia in Schizophrenia and Mood Disorders.

BACKGROUND AND HYPOTHESIS: Identifying reliable diagnostic biomarkers in catatonia remains a key challenge to improve early intervention and reduce morbidity and mortality. Since its pathophysiology may involve cortical dysconnectivity, electroencephalography (EEG) could provide accessible disease-associated measures, such as power spectral density (PSD), peak alpha frequency (PAF), and C and D microstates. However, EEG is yet to be used for this purpose. STUDY DESIGN: This study is a case-control retrospective transdiagnostic hospital-based cohort. We analyzed resting-state EEG data from pat

Neurophysiological signatures of Stanford Neuromodulation Therapy in treatment resistant depression.

Treatment-resistant depression (TRD) affects approximately 30% of patients with major depressive disorder. Stanford Neuromodulation Therapy (SNT), a high-dose intermittent theta-burst transcranial magnetic stimulation protocol, produces rapid antidepressant effects, but its neurophysiological mechanisms remain unclear. Here, we used longitudinal TMS-EEG to characterize the progressive neurophysiological changes induced by SNT, assess their site-specificity, and explore whether baseline neural markers are associated with clinical response. We conducted a double-blind, randomized, sham-controlle

Anterior cruciate ligament injury: beyond the ligament-a neurophysiological perspective.

The anterior cruciate ligament (ACL) is not merely a passive stabilizer of the knee, yet its injuries are still commonly treated as a mechanical problem despite growing evidence implicating the central nervous system (CNS). This mini-review highlights the ACL as a critical sensory organ, housing mechanoreceptors that provide afferent feedback regarding the knee position. We further examine evidence across peripheral, cortical, and supraspinal levels to characterize CNS involvement in ACL injury.

Neurophysiological Characteristics of Nitrous Oxide-Induced Polyneuropathy: A Case Series.

BACKGROUND: Nitrous oxide (N2O) misuse is a growing health concern, with N2O-induced neurological disorders increasingly reported across Europe. Among these, N2O-induced polyneuropathy (PNP) can lead to permanent deficits, yet its neurophysiological characteristics remain inconsistently described. This retrospective case series aimed to characterize the electrophysiological pattern of N2O-induced polyneuropathy. METHODS: We reviewed the records of 12 patients with acute neurological symptoms and a history of N2O abuse, all of whom underwent electrophysiological testing at a German tertiary ca

Neurophysiological mechanisms of exertional dyspnea in advanced pregnancy: A case study.

The neurophysiological mechanisms of exertional dyspnea in advanced pregnancy remain incompletely understood. This short case report describes the neurophysiological and sensory responses during standardized cardiopulmonary exercise testing (CPET) in one healthy adult female at three timepoints: a) 3 months pre-pregnancy, b) 35 weeks pregnant (third trimester [T3]), and, c) 1 year post-partum. At rest and during exercise, detailed measurements of neurophysiological, gas-exchange and sensory parameters were completed. Compared to both pre-pregnancy and post-partum, ventilatory requirements, ele

Neurophysiological evidence of rapid holistic tone-based lexical access.

Word accents are restricted by a word's morphological structure but also distinguish lexical meanings in minimal pairs. We investigated how early tonal information carried by morphophonemic word accents influences rapid lexical access in native speakers. We also asked whether tone-bearing words are decomposed into morphemes and word accents, or retrieved holistically via full-form neural representations. Here, suffix morpheme and word accent tone were varied orthogonally to create valid and invalid combinations of tone and morphological structure/segmental information. These stimuli were prese
