Safety of probiotics in patients receiving nutritional support: a systematic review of case reports, randomized controlled trials, and nonrandomized trials.

BACKGROUND: Probiotics are increasingly used in patients receiving nutritional support; however, some case reports and trials have questioned their safety in such patients. OBJECTIVE: The objective was to investigate the safety of probiotics in patients receiving nutritional support through a systematic review of case reports, randomized controlled trials (RCTs), and nonrandomized trials. DESIGN: The systematic review followed Cochrane and PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) recommendations. Six electronic databases were searched, a hand search of conf…

High frequencies of elevated alkaline phosphatase activity and rickets exist in extremely low birth weight infants despite current nutritional support.

BACKGROUND: Osteopenia and rickets are common among extremely low birth weight infants (ELBW, <1000 g birth weight) despite current practices of vitamin and mineral supplementation. Few data are available evaluating the usual course of markers of mineral status in this population. Our objectives in this study were to determine the relationship between birth weight (BW) and peak serum alkaline phosphatase activity (P-APA) in ELBW infants and evaluate our experience with the diagnosis of rickets in these infants. METHODS: We evaluated all ELBW infants admitted to Texas Children's Hospital NICU

Nutritional support in children with end-stage liver disease: a randomized crossover trial of a branched-chain amino acid supplement.

Malnutrition is common in children with end-stage liver disease (ESLD) awaiting orthotopic liver transplantation (OLT), and nutritional support is assuming an important role in preoperative management. To evaluate preoperative nutritional therapy, 19 children (median age 1.25 y) with ESLD awaiting OLT were prospectively studied. Two high-energy, isoenergetic and isonitrogenous nutritional formulations delivered nasogastrically were compared: a branched-chain amino acid (BCAA)-enriched semielemental formulation and a matched standard semielemental formation. Twelve of 19 patients completed a ra…

Feeding under support in critical care illness: metabolic and nutritional management during extracorporeal membrane oxygenation and continuous renal replacement therapy.

PURPOSE OF REVIEW: The use of life-support therapies like extracorporeal membrane oxygenation (ECMO), mechanical circulatory systems (MCS), and continuous renal replacement therapy (CRRT) creates a complex and dynamic metabolic environment that profoundly challenges nutritional management for these critically ill patients. Current existing guideline recommendations are largely based on expert opinions and observational data. No specific guidelines exist for this population. RECENT FINDINGS: This mini-review synthesizes the current, limited evidence on the pathophysiological and metabolic alte

Association between the C-reactive protein-albumin-lymphocyte (CALLY) index and mortality in elderly patients with dysphagia requiring nutritional support.

BACKGROUND: While the novel C-reactive protein-albumin-lymphocyte (CALLY) index-integrating inflammatory, nutritional, and immune markers-has proven valuable for prognosis in various diseases, its utility in predicting outcomes for dysphagia patients is still unclear. METHODS: This retrospective cohort analysis utilized data sourced from the Dryad Digital Repository. The CALLY index (albumin × lymphocyte count/C-reactive protein × 104) was calculated. After logarithmic transformation (Ln-CALLY), the patients were divided into low Ln-CALLY group (≤ 2.21) and high Ln-CALLY group (>2.21). All-ca

Enteral nutritional support for an elderly critically ill patient with gastroesophageal reflux disease and situs inversus totalis: a case report.

Herein, we present a case of serious protein-energy malnutrition in an elderly critically ill patient with situs inversus totalis. It was difficult to implement enteral nutrition in this patient for more than 2 months of hospitalisation in another hospital, and we applied electromagnetic navigation guidance to implement enteral nutrition after successful placement of nasojejunal tubes. We reviewed the management of enteral nutrition support.

Impact of Protein and Nutritional Support on the Muscular Status of Critically Ill Patients: A Pilot, Perspective, and Exploratory Study.

BACKGROUND: Malnutrition and muscle weakness are highly prevalent in critically admitted patients. To overcome sarcopenia and muscle weakness, physical activity and neuromuscular electric stimulation have been introduced with limited efficacy. Thus, several anabolic remedies have been introduced. An adequate increase in protein support according to indirect calorimetry and body composition and methyl hydroxybutyrate (HMB) is emerging. Therefore, we wanted to investigate the impact of HMB-enriched whey formula on the nutritional status, muscle weakness, and clinical course of critically ill pat

Efficacy of a transforming growth factor beta 2 containing nutritional support formula in a murine model of inflammatory bowel disease.

OBJECTIVE: Dietary, environmental and genetic events may influence host susceptibility to inflammatory bowel diseases (IBD). Transforming growth factor beta 2 (TGF-beta 2), a multifunctional polypeptide (cytokine) present in human and bovine milk, plays a critical role in the development of tolerance, the prevention of autoimmunity, and in anti-inflammatory responses. TGF-beta 2 is a potent inhibitor of intestinal epithelial cell (IEC) growth and stimulates IEC differentiation. The objective of this study was to determine whether a diet containing TGF-beta 2 modulates intestinal injury and imm

Nutrition support of children with chronic liver disease: A rapid review and consensus statements from Australasian society of parenteral and enteral nutrition (AuSPEN) Paediatric liver group.

Chronic liver disease (CLD) in children is associated with high risk of malnutrition, micronutrient deficiencies, growth failure, and poor post-transplant outcomes. Although international 2019 guidelines exist, variability in practice across Australia and New Zealand (ANZ) highlights the need for locally relevant, updated nutritional guidance. This study aimed to conduct a rapid review of recent evidence (2018-2025) on the nutritional management of paediatric CLD and develop updated consensus statements to inform a best practice nutrition pathway for ANZ. A rapid review was conducted in accord

[Postoperative nutritional support in liver surgery. Effects of specialized parenteral nutrition enriched with branched-chain amino acids following liver resections for colorectal carcinoma metastases].

INTRODUCTION: Perioperative nutritional support in liver surgery remains specific regarding the role of the liver in the metabolism regulation. The loss of functional liver tissue following resection procedures may result in significant metabolic disorders, particularly in patients with preoperative liver impairment in chronic conditions. Perioperative nutritional support is indicated in patients suffering from malnutrition, chronic liver disorders and also following liver resections with limiting residual parenchyma. AIM OF THE STUDY: The retrospective study assessed the effect of complete p…

Mechanisms of growth retardation, drug therapy, and nutritional support in pediatric inflammatory bowel disease: a workshop sponsored by the north american and European societies for pediatric gastroenterology and nutrition.

: Growth retardation is common in children with inflammatory bowel disease (IBD). The most sensitive measure of impaired growth, growth velocity, is abnormal in 65% of children with Crohn's disease. With treatment, growth velocity may return to normal, but catch-up growth is often incomplete and ultimate height lower than predicted. In some patients delayed puberty may compensate for poor growth earlier in life, and there is good evidence that even after menarche, significant growth can occur. Surgery may have a favorable impact on growth in the short term, but final height often remains reduc
