Infranuclear ocular motor disorders.

This chapter covers the very large number of possible disorders that can affect the three ocular motor nerves, the neuromuscular junction, or the extraocular muscles. Conditions affecting the nerves are discussed under two major headings: those in which the site of damage can be anatomically localized (e.g., fascicular lesions and lesions occurring in the subarachnoid space, the cavernous sinus, the superior orbital fissure, or the orbit) and those in which the site of the lesion is either nonspecific or variable (e.g., vascular lesions, tumors, "ophthalmoplegic migraine," and congenital disor

Slipped, severed, torn and lost extraocular muscles.

Slipped, severed, torn and lost extraocular muscles (EOM) are infrequently encountered in clinical practice but constitute significant complications of strabismus and other eye surgery and of orbital injuries. Knowledge of the clinical aspects of these various disease entities and their anatomical underpinnings are of utmost importance in providing effective recognition and treatment. These conditions share some common presenting signs, symptoms and clinical findings that are discussed in this review. The literature will be reviewed and management strategies will be presented.

Arthrogryposis, perthes disease, and upward gaze palsy: a novel autosomal recessive syndromic form of arthrogryposis.

In this article, the unusual combination of arthrogryposis, upward gaze palsy, and Perthes disease in two sisters and their cousin who are all part of an extended consanguineous Saudi family is reported. All patients had difficult to control bronchial asthma and subtle facial dysmorphism. Two of the three had pyloric stenosis, two were intellectually normal, and one had academic problems but had a history of birth hypoxia. Pedigree is consistent with an autosomal recessive mode of inheritance. Lack of previous reports suggests this represents a novel syndromic form of arthrogryposis.

[Case of multiple sclerosis with WEBINO syndrome].

We report a 50-year-old man presenting with wall-eyed bilateral internuclear opthalmoplegia (WEBINO) syndrome. He had suffered from progressive double vision and tetraparesis, and been diagnosed as secondary progressive multiple sclerosis (MS). On admission, he presented with bilateral facial nerve palsy, pseudobulber palsy, and spastic tetraparesis, predominantly on the right side. Bilateral adduction deficits were noted on horizontal gaze, together with nystagmus of abducting eyes. On primary eye position, the right eye was fixed in the midposition, while the left eye was exotropic. The righ

[Integrated Center for Research and Treatment of Vertigo, Balance and Ocular Motor Disorders].

The German BMBF (German Ministry of Education and Research) has decided to establish an Integrated Center for Research and Treatment of Vertigo, Balance and Ocular Motor Disorders (IFB(LMU)) in Munich in 2010. After funding over a 10-year period, the long-term continuation of the IFB(LMU) by the medical faculty and the hospital is envisioned. Vertigo is one of the most common complaints in medicine. Despite its high prevalence patients with vertigo generally receive either inappropriate or inadequate treatment. This deplorable situation is internationally well known and its causes are multiple

Familial congenital oculomotor apraxia: clinical and electro-oculographic features.

The electro-oculographic (EOG) features of both horizontal and vertical eye movements in congenital oculomotor apraxia (COMA) were not previously reported. A girl referred to the ophthalmologic department for abnormal eye movements was diagnosed as COMA. The same abnormal ocular movements were observed in her younger sister and her father who was unaware of his difficulties to initiate voluntary saccades. When performed, EOG recordings of all horizontal and vertical saccadic eye movements were severely altered whatever the age of the patient. Pursuit was normal for these patients. It confirms

Progressive supranuclear palsy with wall-eyed bilateral internuclear ophthalmoplegia syndrome.

BACKGROUND: Walleyed bilateral internuclear ophthalmoplegia (WEBINO) syndrome has mainly been reported in patients with cerebrovascular diseases and multiple sclerosis, but has never been described in patients with neurodegenerative diseases. OBJECTIVE: To describe a patient with progressive supranuclear palsy (PSP) who presented with WEBINO syndrome. DESIGN: Case report and review of literature. SETTING: A university hospital. PATIENT: A 72-year-old man began to display akinesia, freezing of gait, postural instability, mild rigidity of the neck, and vertical supranuclear palsy, including

Horizontal gaze palsy with progressive scoliosis: CT and MR findings.

Horizontal gaze palsy with progressive scoliosis (HGPPS) is a rare congenital disorder characterized by absence of conjugate horizontal eye movements and progressive scoliosis developing in childhood and adolescence. We present a child with clinical and neuroimaging findings typical of HGPPS. CT and MRI of the brain demonstrated pons hypoplasia, absence of the facial colliculi, butterfly configuration of the medulla and a deep midline pontine cleft. We briefly discuss the imaging aspects of this rare entity in light of the current literature.

[A case of normal-tension glaucoma with impaired eye movements in a young patient].

BACKGROUND: We report a case of normal-tension glaucoma(NTG) with impaired eye movements in a young patient. CASE: A 22-year-old man with enlarged optic disc cuppings and visual field defects was suspected to have NTG, and was found, upon examination, to have impaired eye movements. Intraocular pressure recordings, including diurnal variation, were under 21 mmHg. Gonioscopic findings were normal. Magnetic resonance image(MRI) showed no particular findings. The patient was diagnosed with NTG. Using the 123I-iodoamphetamine single photon emission computed tomography (123I-IMP SPECT), decreased

[A boy with acute cerebellar ataxia without opsoclonus caused by neuroblastoma].

A 2-year-old boy presented with acute cerebellar ataxia without opsoclonus. The ataxia was assumed to be post-viral. After a period of years a neuroblastoma was detected. Treatment with a curative intent was successful and consisted of metaiodobenzylguanidine I 131, chemotherapy, tumour resection, chemotherapy again and follow-up treatment with isotretinoin after irradiation. In the literature, 5 other children have been described with acute cerebellar ataxia without opsoclonus in whom neuroblastoma was detected eventually. The mean age of these children at initial presentation was 26 months.

Reversible bilateral internuclear ophthalmoplegia associated with FK506.

A 50 year old man developed tonic-clonic seizures while receiving cyclosporin A after orthotopic cardiac transplant. The seizures resolved after cessation of cyclosporin A. Thirteen months later, he developed diplopia from bilateral internuclear ophthalmoplegia while receiving intravenous FK506. A temporal association was found between his symptoms and the serum FK506 concentrations. Withdrawal of the intravenous FK506 led to prompt resolution of the bilateral internuclear ophthalmoplegia.

The effects of varying examiner position on alternating cover test results.

BACKGROUND: Although the alternate cover test is a widely accepted test of ocular alignment, clinical ambiguity exists with regard to the specific location of the examiner. This study was an attempt to replicate the findings of a previous study that compared two examiner positions and their effect on heterophoria measurement at 40 cm. METHODS: Fifty-seven adult subjects (mean age, 24.0 +/- 2.88 years) with normal binocular vision were measured for heterophoria posture using the alternate cover test while the examiner position was varied between midline and offset 30 degrees to the right of pa

[Parinaud's syndrome as a sign of acute obstructive hydrocephalus: recovery after acute ventriculostomy].

Paresis or palsy of upward vertical gaze, pupillary light-near dissociation and nystagmus retractorius with convergence, were present in a boy aged 16 years and a woman aged 30 years with an obstructive hydrocephalus due to an aqueductal stenosis as a consequence of a bacterial meningitis and in a woman aged 26 years and a man aged 47 years with an outlet obstruction of the fourth ventricle after a posterior fossa operation for a tumour in the fourth ventricle. All of the patients were suspected of having a drain dysfunction. They all underwent a third-ventriculocisternostomy after which their

Complete bilateral horizontal gaze paralysis disclosing multiple sclerosis.

Two women presented with bilateral internuclear ophthalmoplegia evolving in a few days to complete bilateral horizontal gaze paralysis. Convergence and vertical eye movements were normal. Cerebral MRI showed a few small white matter lesions in the lateral ventricle regions, and, at the brainstem level, a single, small, bilateral lesion affecting the posterior part of the medial pontine tegmentum and responsible for the clinical syndrome. The condition gradually improved in both patients, following a similar progression as at the onset: improvement first involved the adduction movements in both
