Lid retraction following blow-out fracture of the orbit.

A case of lid retraction following blow-out fracture of the orbit with enophthalmos is reported. This paradoxical complication has been described previously in only two cases, both by the same authors. It has been hypothesized that the lid retraction is secondary to Mueller's muscle overaction; however, excision of Mueller's muscle had no effect in this case. A new hypothesis is presented that this condition may be due to traction on the connective tissue sheath of the levator palpebrae muscle through its interconnections with the connective tissue systems of the other extraocular muscles.

Destructive cysts of the maxillary sinus affecting the orbit.

Symptomatic maxillary sinus cysts are diagnosed less frequently than similar cysts of the frontal and ethmoidal sinuses and are rarely reported in the ophthalmic literature. Patients with cysts of the maxillary sinus may present to the ophthalmologist with proptosis, enophthalmos, diplopia, ptosis, epiphora, and, rarely, decreased visual acuity. Four patients with maxillary sinus mucoceles are presented; one of these patients had a concurrent retention cyst in the orbit. Clinical history, radiologic findings, and histopathologic mechanisms are discussed. Mucocele is a recognized complication o…

Surgical management of volumetric changes in the bony orbit.

This article demonstrates that the periorbital fat, instead of being continuous and freely flowing, is encased within 3 compartments: 1 anterior to the extraocular muscles, 1 external to the extraocular muscles, and 1 deep to the extraocular muscles. In addition, the study establishes that fat loss anterior to the axis of the globe does not affect the anteroposterior location of the eye itself. Only fractures located posterior to the axis produce enophthalmos, and only operative procedures that create bony enlargement and fatty displacement behind this axis correct exophthalmos. Furthermore, o…

Histochemical analysis of breast carcinoma metastatic to the orbit.

A 62-year-old woman was seen for evaluation of an orbital tumor. Recognition of a relatively rare syndrome of pain, ptosis, and a progressively immobilized globe with enophthalmos suggested the diagnosis of metastatic carcinoma to the left orbit. In this case, this was confirmed by open breast and orbital biopsies, revealing infiltrating lobular breast carcinoma. This is the first reported case where estrogen and progesterone receptors were identified in a metastatic orbital tumor using fluorescent histochemical techniques. This technique was of value in confirming the diagnosis and providing …

A clinicopathologic study of hematic cysts of the orbit.

Hematic cyst of the orbit has been described in association with various diseases, but its pathogenetic mechanism remains unclear. We treated two patients (two men, 38 and 35 years old) who had growing cysts suspected of being malignant tumors. Surgical exploration of the orbits disclosed birefringent crystals (hematodin) either in the cyst wall or in the cyst's contents. The crystals imitated foreign bodies in their appearance and in the reactions they induced.

Radiation and chemotherapy of parameningeal rhabdomyosarcoma involving the orbit.

Eighteen patients with parameningeal rhabdomyosarcoma (RMS) involving the orbit were treated at Memorial Sloan-Kettering Cancer Center (MSKCC) between July 1971 and October 1983. Fifteen patients were children with a mean age of 6 years and three patients were adults with a mean age of 21 years. In four patients, the primary tumor originated in the orbit, while the remaining 14 had other parameningeal primary sites. The tumors were in a very progressive local stage, with destruction of facial bones in most patients. Six patients were treated with the T2 chemotherapy protocol and 12 received th…

Posttraumatic cerebrospinal fluid cyst of the orbit. Case report.

A 27-year-old man sustained a fracture of the orbital roof and a basofrontal dural tear in a motor-vehicle accident. One week later, he developed an orbitocranial dural fistula manifested by an orbital cyst, pulsatile proptosis, and serous drainage from the eye. Specific diagnosis was established only after computerized tomography metrizamide cisternography demonstrated direct communication of the orbital cyst with the subarachnoid space. The pertinent literature is reviewed.

[Radiologic diagnosis of the orbit and anterior cranial fossa].

Basing on frequent diseases associated with typical changes of the orbita and the anterior cranial fossa - osteomas, fibrous dysplasia, meningiomas of the sphenoid bone, neurofibromatosis, mucoceles and pyoceles, dermoid tumours, malignant tumours and transmitted inflammatory processes - the x-ray diagnostic procedure is explained. Plain x-ray films and computed tomography are the most important methods, whereas examinations employing film tomography and angiography are now restricted to a few selected cases only.

Another look at blow-out fractures of the orbit.

Thirty cases of orbital floor blow-out fractures proven by tomography were retrospectively reviewed to determine the accuracy of the plain films. Using the maximum diameter from the tomograms, the area and volume of the fractures were calculated and correlated with the presence of diplopia and enophthalmos at the time of presentation and at subsequent follow-up. Nine of the 30 cases underwent surgical repair. Orbital floor fractures were recognized in 29 of 30 cases using only the 28 degrees Caldwell and Waters views. Routine tomography is unnecessary and should be reserved as a preoperative e…

Eosinophilic granuloma of the soft tissue in the orbit.

A 61-year-old Japanese man was treated for eosinophilic granuloma of the soft tissue in the orbit. The clinical characteristics were typical for eosinophilic granuloma of the soft tissue: 8% eosinophils, very soft tumor masses in both orbits, multifocal lesions including the salivary glands, good response to steroid hormone, and hypergammaglobulinemia. The ocular signs were exophthalmos and lid swelling. Light microscopy showed many lymphoid follicles with many eosinophils, also in other parts of the body. The previously reported cases of orbital lesion are reviewed. Patients with orbital eosi…

Reversible visual loss in subperiosteal hematoma of the orbit.

A 19-year-old white male developed blindness in the left eye following a motor vehicle accident. Orbital echography and computerized axial tomography established the diagnosis of subperiosteal hematoma of the orbit. Emergency medical therapy and lateral canthotomy resulted in slight visual improvement but rebleeding occurred 12 hours after injury with visual loss. An emergency surgical decompression of the orbit was performed. Vision returned to 6/7.5 following decompression. This is the first report illustrating that orbital decompression can reverse complete visual loss from a subperiosteal …

Computerized axial tomography in inflammatory pseudotumor of the orbit.

Although thyrotoxicosis and orbital complications of acute ethmoid or frontal sinusitis are among the most common causes of unilateral exophthalmos, inflammatory pseudotumor is frequently accompanied by progressive acute unilateral proptosis. Because the associated chemosis, scleral erythema, and ophthalmoplegia constitute a spectrum of clinical findings present in numerous inflammatory orbital disorders and systemic diseases, the diagnosis of inflammatory pseudotumor is one of exclusion, often requiring orbital biopsy. Four patients without evidence of sinusitis, endocrinopathy, collagen vasc…
