Preoperative hematological assessment of pediatric patients with cerebral palsy.

OBJECTIVES: Nutritional deficiencies and use of antiepileptic drugs can lead to alterations in the hematological status of children with cerebral palsy (CP), which may increase the risk of intraoperative or postoperative hematological complications. In this retrospective study, we evaluated the preoperative routine blood tests of CP patients with different levels of walking ability, who were scheduled to undergo orthopedic procedures. METHODS: Hemoglobin level, hematocrit, red blood cell count, mean corpuscular volume, mean corpuscular hemoglobin, mean corpuscular hemoglobin concentration, re…

Clinical Case Report of Expansive Laminoplasty for Cervical Myelopathy Due to Both Disc Herniation and Developmental Cervical Spinal Canal Stenosis in Older Adolescents.

Reports on adolescent patients with cervical myelopathy who underwent anterior cervical discectomy and fusion are scarce. However, to our knowledge, no cases of expansive laminoplasty for cervical myelopathy associated with progressive neurological deficit after a series of conservative treatment, caused by both disc herniation and developmental cervical spinal canal stenosis, have been reported. From January 2006 to July 2012, we retrospectively studied 3 patients in late adolescence presenting with cervical myelopathy who underwent expansive unilateral open-door laminoplasty at our hospital.

Tendon transfers for the drop foot.

The paralytic drop foot represents a challenging problem for even the most experienced orthopedic surgeon. Careful patient selection, thorough preoperative examination and planning, and application of tendon transfer biomechanical and physiologic principles outlined in this article can lead to successful results, either through a posterior tibialis tendon transfer, Bridle transfer, or variations on these procedures. Achilles lengthening or gastrocnemius recession may also be needed at the time of tendon transfer.

Robots and Tools for Remodeling Bone.

The field of robotic surgery has progressed from small teams of researchers repurposing industrial robots, to a competitive and highly innovative subsection of the medical device industry. Surgical robots allow surgeons to perform tasks with greater ease, accuracy, or safety, and fall under one of four levels of autonomy; active, semi-active, passive, and remote manipulator. The increased accuracy afforded by surgical robots has allowed for cementless hip arthroplasty, improved postoperative alignment following knee arthroplasty, and reduced duration of intraoperative fluoroscopy among other b

Robotic-Assisted Spinal Surgery: Current Generation Instrumentation and New Applications.

In contrast to other surgical fields, robotics is relatively new to spinal surgery. The initial focus for spinal robotics has been on accurate pedicle screw placement, which early studies have shown to be successful. Beyond pedicle screw placement, however, newer generation spinal robots have the capability of navigation, which can impact other aspects of a spinal procedure. In this study, pedicle screw placement with the recently introduced Excelsius GPS robot (Globus) was similarly found to be accurate in a cohort of patients undergoing both open and minimally invasive fusion. Potential appl

Implant Removal Matrix for the Foot and Ankle Orthopaedic Surgeon.

BACKGROUND: Orthopaedic surgery frequently requires the use of metal plate and screw constructs for osteosynthesis. These constructs may be subsequently removed for a variety of indications. In the lower extremity in particular, implants (ie, hardware) can lead to prominence and pain secondary to a thin soft-tissue envelope with little subcutaneous tissue. Often, removal is performed without knowledge of the exact type of screw head configuration and/or size, which can make removal technically challenging. The purpose of this study is to consolidate screw head sizes and configurations from com

Controversies and challenges in elective orthopedic surgery in patients with hemophilia and inhibitors.

Until recently, orthopedic surgery was strongly contraindicated in patients with hemophilia and inhibitors. However, recent advances in our knowledge of bypassing agents (particularly recombinant activated factor VII [rFVIIa]) that provide effective surgical hemostasis have allowed us to successfully perform major orthopedic procedures in these patients. Adequate hemostasis during surgery and postoperative rehabilitation is crucial, as development of a wound hematoma may jeopardize long-term outcomes. It also should be noted that success depends not only on appropriate drug therapy but also on…

Hospital-acquired COVID-19 and its effect on length of stay and mortality in orthopedic admissions: A matched cohort study.

INTRODUCTION: Hospital-acquired COVID-19 poses a significant threat to orthopedic patients, a population already at risk due to immobility, comorbidities, and extended hospital stays. The combined burden of musculoskeletal injury and SARS-CoV-2 infection may prolong recovery, increase complications, and influence survival. This study aimed to evaluate the impact of nosocomial COVID-19 on hospitalization outcomes in orthopedic patients. METHODS: A retrospective cohort study was conducted at a tertiary orthopedic center, analyzing patients hospitalized between 2020 and 2022. COVID-19-positive p

INTRODUCTION: Cranio-Maxillofacial Dysplasias (CMD), including Craniofacial Microsomias, syndromes (such as Treacher Collins or Williams) and isolated Condylo-Mandibulo-Dysplasia, is a controversial subject with treatments as diverse as diagnostic classifications. The authors present here a retrospective study of 85 patients, with congenital condyle dysplasia arising from these 3 main types of CMD, treated with different techniques that aimed to normalize the facial skeleton and occlusion. METHODS: The authors studied retrospectively 85 patients, aged from 3 to 53 years old, affected by diffe

Multimodal Analgesia Provides Superior Postoperative Pain Control Following Orthopedic Surgery in Small-Breed Dogs.

Effective pain control after orthopedic surgery is essential in veterinary practice, particularly in small-breed dogs with low physiological reserves. This study aimed to compare the analgesic efficacy and tolerability of five postoperative pain protocols across nine surgical procedures. A retrospective analysis was conducted in 205 small-breed dogs (≤7 kg) undergoing orthopedic surgeries. Dogs were assigned to one of five analgesic protocols: (A) carprofen, (B) tramadol-lidocaine-ketamine continuous-rate infusion, (C) butorphanol continuous-rate infusion, (D) hydromorphone continuous-rate inf

Advances in the local and targeted delivery of anti-infective agents for management of osteomyelitis.

Osteomyelitis, a common and debilitating invasive infection of bone, is a frequent complication following orthopedic surgery and causes pathologic destruction of skeletal tissues. Bone destruction during osteomyelitis results in necrotic tissue, which is poorly penetrated by antibiotics and can serve as a nidus for relapsing infection. Osteomyelitis therefore frequently necessitates surgical debridement procedures, which provide a unique opportunity for targeted delivery of antimicrobial and adjunctive therapies. Areas covered: Following surgical debridement, tissue voids require implanted mat
