Use of an iliac crest growth plate for the development of a neo-articulation for congenital transverse deficiencies at the wrist.

PURPOSE: Terminal congenital transverse deficiencies of the wrist and hand are rare malformations. They are usually treated with prosthetic devices. We describe here a surgical technique to create a neo-articulation that could provide mobility distal to the forearm and help children with terminal transverse deficiencies use orthotic devices. METHODS: This procedure involved an autologous, nonvascularized graft from the immature iliac crest. The graft was placed with its cartilaginous side facing the radial and ulnar articular surfaces. We studied the clinical, radiological, and functional out…

[Medical and technical compensation of disabilities].

Thanks to technological advances handicaps can be treated in a better way by physicians. This real Metrology of Handicap constitutes a preliminary stage to medical decision. Functional repercussions of a lesion can thus be accurately assessed before the well adapted compensatory means are conceived, prescribed and applied. A better understanding of physiopathological mechanisms involving lesion, deficit and handicap and of compensatory procedures used by disabled people depends from this assessment. Some examples will be reported such as energy expenditure, 3-dimensional biomechanical analysis

Real-Time Continuous Gait Phase and Speed Estimation from a Single Sensor.

Human gait involves a repetitive cycle of movements, and the phase of gait represents the location in this cycle. Gait phase is measured across many areas of study (e.g., for analyzing gait and controlling powered lower-limb prosthetic and orthotic devices). Current gait phase detection methods measure discrete gait events (e.g., heel strike, flat foot, toe off, etc.) by placing multiple sensors on the subject's lower-limbs. Using multiple sensors can create difficulty in experimental setup and real-time data processing. In addition, detecting only discrete events during the gait cycle limits …

[Amputation and exarticulation of the lesser toes].

OBJECTIVE: Amputations and exarticulations of the toes may be necessary due to several reasons. The goal is to remove necrosis or infection … x or exarticulated in the metatarsophalangeal joint. POSTOPERATIVE MANAGEMENT: Orthopedic shoes or orthotic devices are rarely necessary when a single toe is amputated/exarticulated. However, concomitant deformities of the foot have to be thoroughly addressed. If more than one toe is amputated, silicone spacers may be necessary to prevent the remaining toes from …

Staying in Touch: Toward the Restoration of Sensory Feedback in Hand Prostheses Using Peripheral Neural Stimulation.

Hand amputation is a traumatic event that dramatically and permanently changes the life of any person who undergoes one. After surgery, the amputee requires a prosthetic device to perform activities of daily living-in particular, tasks requiring grasping and manipulation functions. According to the Washington, D.C.-based Amputee Coalition, there are 1.9 million amputees who use limb prosthetic services and products, and it is estimated that, among them, 500,000 are upper-limb amputees, with approximately 185,000 new amputations every year (www.amputee-coalition.org). The Center for Orthotic an

The frontal plane relationship of the forefoot to the rearfoot in an asymptomatic population.

Physical therapists use biochemical orthotics to treat symptoms attributed to structural abnormalities of the feet. The purpose of this study was to determine the prevalence of forefoot varus, valgus, and neutral positions in a healthy population, and to compare forefoot positions between genders, legs, and age groups. Nonprobability sampling was used to obtain 120 healthy subjects. A repeated-measures, one-group design was used. A forefoot measuring device and a goniometer were used to measure the forefoot-rearfoot frontal plane relationship. Of the 234 measured feet, 86.67% had a varus, 8.75

Management of Patients With Legg-Calvé-Perthes Disease at a Single Center in Jeddah, Saudi Arabia.

BACKGROUND: Legg-Calvé-Perthes disease (LCPD) is an idiopathic pediatric hip disorder associated with avascular necrosis of the femoral head. Although there is no standardized and optimal treatment protocol for patients with LCPD, there are three primary treatment strategies: symptomatic treatment, nonsurgical containment using orthotic devices, and surgical containment. OBJECTIVE: This study aimed to describe the demographic characteristics, management and outcome of pediatric patients with LCPD at our center between 2005 and 2015. MATERIAL AND METHODS: In this retrospective study, 23 patie…

Three-dimensional photographic analysis of outcome after helmet treatment of a nonsynostotic cranial deformity.

Cranial asymmetries due to nonsynostotic deformation of the skull have been reported with increasing frequency during the last decade. Conservative approaches using helmets and physiotherapy have been shown to be effective in their treatment. Traditionally, documentation has been carried out using anthropometric caliper measurements. The present study evaluates the use of a new three-dimensional photographic system in the improved validation of changes in head deformities. This prospective analysis introduces a new technique for digital anthropometric measurement. The study series comprised 18

Flatfoot deformity pathogenesis. A trilogy.

The significant conclusion from this trilogy is that most developmental flatfeet are present at birth. It is likely that intrauterine forces and fetal foot position initially create the abnormal bony relationships between anatomically normal bones. This bony relationship is present before the child takes his first step. Second, as the result of this abnormal relationship between normal bones, the active muscular force balance between opposing muscle groups is distorted. This allows one muscle to have a mechanical advantage over its antagonist. This actively maintains and progressively worsens

Positional cranial deformation in children: A plea for the efficacy of the cranial helmet in children.

BACKGROUND: Cranial deformations have historically aroused the interest of people worldwide. One of the more debated points about positional plagiocephaly is the choice of the treatment. In this article, the senior author (CM) shares his experience on almost 30 years of use of the helmet molding therapy in children with deformation plagiocephaly. METHODS AND POPULATION: We retrospectively and systematically reviewed the cases of 2188 patients (75% males and 25% females) presenting positional head deformity and treated between 1991 and 2013 with a cranial helmet. To assess the effectiveness of

Effects of manual therapy on treatment duration and motor development in infants with severe nonsynostotic plagiocephaly: a randomised controlled pilot study.

PURPOSE: Despite growing evidence regarding nonsynostotic plagiocephaly and their repercussions on motor development, there is little evidence to support the use of manual therapy as an adjuvant option. The aim of this study was to evaluate the effects of a therapeutic approach based on manual therapy as an adjuvant option on treatment duration and motor development in infants with severe nonsynostotic plagiocephaly. METHODS: This is a randomised controlled pilot study. The study was conducted at a university hospital. Forty-six infants with severe nonsynostotic plagiocephaly (types 4-5 of th

Ability and perceived difficulty in daily activities in people with poliomyelitis sequelae.

Polio involvement and its relation to ability in terms of independence and perceived difficulty in activities in daily living (ADL) were studied in outpatients (median age 57 years) with poliomyelitis sequelae (n = 133). The use and need of assistive devices and housing conditions were recorded. The subjects had a high degree of independence in personal ADL, whereas more than 50% were dependent on cleaning, shopping and transportation. A considerable number of subjects independent in ADL reported difficulties in daily activities. The majority of interventions in the form of housing adaptation

Can we configure COOK, a cognitive Orthosis for meal preparation, with efficiency and effectiveness?

PURPOSE: Assistive Technologies for Cognition (ATCs), such as the Cognitive Orthosis for coOKing (COOK), offer support to individuals with traumatic brain injury by enhancing safety and independence. While the usability of COOK's client interface has been tested, the expert interface-used by occupational therapists to customize the interface to clients' needs-requires further study. We therefore aimed to: (1) Prioritize the modifications to be made to COOK's configuration interface; and (2) Describe the effectiveness and efficiency of the interface within a laboratory context. [Multidisciplinary approach to treatment of positional cranial asymmetry in infants].

The prevalence of positional cranial asymmetry has significantly increased since the introduction of the "Back to Sleep" campaign. Some deformities require therapeutic measures, such as growth-guiding head prostheses. The diagnosis is based on the clinical features, a thorough clinical examination and measurement of the infant's head. Prevention includes early education of parents about alternative positioning methods. Early interventions such as positioning therapy and physiotherapy can be effective in mild cases. In severe cases, the use of growth-guiding orthoses is necessary. The treatment
