Pathological fracture of the femur in a patient with Paget's disease of bone: a case report.

Paget's disease of bone is a benign disease characterized by exaggerated remodeling of the bone matrix after osteoclast-mediated bone destruction. Its etiology is still unknown, despite the fact that it was discovered and described in 1877, but genetic factors and environmental triggers were shown to play their part in the pathogenesis of the disease. The main clinical presentations of the disease are related to bone pain and deformities. Radiological diagnosis is the main detection tool, though many monostotic Paget's disease cases may remain undiagnosed. We present the case of an 81-year-old

Increased 177Lu-DOTATATE uptake in Paget disease.

A 45-year-old female patient was referred for peptide receptor radionuclide therapy of liver metastases of neuroendocrine origin; the patient had undergone 2 years earlier surgical resection of a grade 2 neuroendocrine neoplasm of ampulla of Vater infiltrating the duodenal submucosa and muscular layers and the neighboring pancreatic tissue. Post-therapy whole-body scan performed to evaluate in vivo radiopharmaceutical distribution confirmed high accumulation in liver lesions and revealed increased uptake in the hip bone. Bone scan and corresponding CT images demonstrated changes of Paget disea

Symptomatic intracranial hypertension and prolonged hypocalcemia following treatment of Paget's disease of the skull with zoledronic acid.

Skull involvement in Paget's disease of bone can lead to neurological symptoms, prompting treatment. Intravenous zoledronic acid (ZA) has emerged as an effective and safe treatment option for patients with Paget's, leading to sustained remission and improved quality of life. A previously untreated 61-year-old female presented with 2-year history of facial asymmetry with progressive hearing impairment. Serum calcium levels were normal with upper normal levels of PTH and low 25OHD levels. Serum alkaline phosphatase was markedly increased and bone scan showed extensive pagetic involvement of the

New knowledge on critical osteoclast formation and activation pathways from study of rare genetic diseases of osteoclasts: focus on the RANK/RANKL axis.

Functional, biochemical and genetic studies have over the past decade identified many causative genes in the osteoclast diseases osteopetrosis and Paget's disease of bone. Here, we outline all osteoclast diseases and their genetic associations and then focus specifically on those diseases caused by mutations in the critical osteoclast molecule Receptor Activator of Nuclear factor Kappa B (RANK). Both loss and gain-of-function mutations have been found in humans leading to osteopetrosis and high bone turnover phenotypes, respectively. Osteopetrosis-associated RANK mutations are widely distribut

Osteonecrosis of the jaw and bisphosphonates--putting the risk in perspective.

Osteonecrosis of the jaw is a poorly understood condition that has recently been associated with the use of potent bisphosphonate treatment. The prevalence appears to be approximately 1-10% in patients with malignancy treated with very high doses of intravenous bisphosphonates. In Paget's disease and osteoporosis, where the doses of bisphosphonate used are an order of magnitude lower than the oncology dose, the prevalence appears to be much lower--probably less than 1 in 60,000. The aim of this article is to put the risks of osteonecrosis of the jaw into context with the benefits of bisphospho

Mathematical model predicts a critical role for osteoclast autocrine regulation in the control of bone remodeling.

Bone remodeling occurs asynchronously at multiple sites in the adult skeleton and involves resorption by osteoclasts, followed by formation of new bone by osteoblasts. Disruptions in bone remodeling contribute to the pathogenesis of disorders such as osteoporosis, osteoarthritis, and Paget's disease. Interactions among cells of osteoblast and osteoclast lineages are critical in the regulation of bone remodeling. We constructed a mathematical model of autocrine and paracrine interactions among osteoblasts and osteoclasts that allowed us to calculate cell population dynamics and changes in bone

Bisphosphonates in bone diseases other than osteoporosis.

The range of bone diseases in which bisphosphonates are used has extended far beyond osteoporosis during the last few years. Bisphosphonate therapy is now so well validated as to be the reference standard in Paget's disease and in the prevention of bone complications related to malignant osteolysis. Promising preliminary findings warrant the use of bisphosphonates in conditions that are either rare (fibrous dysplasia) or severe (pediatric osteogenesis imperfecta). The third category of indications encompasses many conditions in which the limited available data do not warrant widespread use: ex

Comparative clinical pharmacology and therapeutic use of bisphosphonates in metabolic bone diseases.

The bisphosphonates are a class of synthetic compounds used in the treatment of various metabolic bone diseases, including hypercalcaemia of malignancy, Paget's disease, postmenopausal osteoporosis and corticosteroid-induced bone loss. Although there have been numerous studies comparing first, second and third generation bisphosphonates with placebo, there has been a paucity of comparative research investigating the effectiveness of these substances with other pharmacological agents. Still, the available evidence indicates that the bisphosphonates are well tolerated and effective therapeutic a

Application of quantitative computed tomography to Paget's disease of bone.

Serial evaluation of pagetic lesions is handicapped by the inability of conventional methods to reproduce precisely the positioning of the area of interest. A computed tomography method that provides visual and quantitative comparisons between data sets acquired at different times has been developed. The utility of this method is illustrated with an example of serial examinations that demonstrate the effect of treatment on small lytic lesions in the lumbar spine. A further demonstration is made of the ability of this method to identify and quantify lytic changes within the cochlear capsule.

[Paget's disease with temporal bone involvement, hypoacusis and vertigo. Apropos a case].

The Paget disease is a chronic osteodystrophy of unknown etiology, very frequent in 50 years of age and above. It's an anatomo-radiologic syndrome that sometimes, is accompanied by clinical symptoms. May be monostotic or polyostotic but never generalized. Occasionally it affects the temporal bone producing deafness, tinnitus and vertigo. We report a case with important temporal bone demineralization, deafness and vertigo.

[Skeletal distribution and biological markers of Paget's disease].

The authors have re-examined the skeletal distribution of Paget's disease of bone by means of radionuclide bone scanning performed in 170 untreated pagetic subjects (93 men, 77 women). 863 high uptake foci were detected. The finding that 8.3 p. 100 of the sites revealed by scanning were not visible on X-ray films shows that the disease is underdetected by standard radiography. Peripheral foci in the feet and hands were discovered in more than a quarter of the patients. There was a significant positive correlation between skeletal extension index on the one hand and plasma alkaline phosphatase

Anatomico-radiological study of an ununited intracapsular fracture of the femoral neck dating back 47 years. Data relevant to osteoarthritis, bone infarct, and Paget's disease of the bone.

This is the anatomico-radiological study of a 47-year-old ununited intracapsular fracture of the hip. The femoral head was osteoporotic but its center contained a zone of osteomedullary necrosis in the form of a bone infarct. Its contour was reshaped and remnants of articular cartilage were covered by osteophyte-like bone formations; furthermore, an acetabular osteophyte was observed. Histological signs of Paget's disease of the bone were observed in the medial part of the femoral stump. The findings in this case provide the basis for a discussion of the pathogenesis of osteophytes in osteoart

Neoplasms arising in Paget's disease of bone: a study of 82 cases.

Eighty-two cases of neoplasms arising in Paget's disease of bone from the Mid-America Bone Tumor Registry, accessioned between 1958 and 1983, were reviewed. There were 77 osteosarcomas, 3 fibrosarcomas, 1 chondrosarcoma, and 1 giant cell tumor. The male/female ratio was 2:1. The age distribution was 18%, 29%, 36%, and 17% for the groups less than 51 years, 51-60 years, 61-70 years, and greater than 70 years, respectively. The femur was involved by tumor in 22%; the humerus in 21%; the pelvis in 21%; the calvarium in 12%; the tibia in 10%; and other bones in 15% of the cases. There were 48% sur

Analysis of collagen from normal and abnormal human bone.

Using cyanogen bromide digestion of bone collagen, we have studied the type of collagen in normal bone, and in samples from patients with Paget's disease of bone, osteomalacia, osteoporosis and renal osteodystrophy. Despite extensive extraction with ethylene diamine tetraacetic acid and guanidine hydrochloride, not all of the glycoproteins could be removed from bone collagen. Upon electrophoresis on sodium dodecyl sulphate polyacrylamide gels, the cyanogen bromide peptides of insoluble bone collagen showed the pattern of type I collagen only in all samples, both normal and abnormal. Amino acid
