[Disease-specific outpatient clinics: are they useful?].

In recent years disease-specific outpatient clinics have gained a foothold in the Netherlands. In some instances, this is warranted. An example would be an outpatient clinic for early-onset Alzheimer's disease to ensure that patients get optimal diagnostic procedures and receive state-of-the-art treatment. However, the initiation of outpatient clinics for common diseases predominantly treated in primary care, such as a one-stop-shop for the diagnosis of asthma, is a more questionable development. It may lead to patients assuming that such a referral is always necessary for the diagnosis, patie

Outpatient Emergencies: Acute Heart Failure.

Heart failure is an epidemic in the United States and a major health problem worldwide. The syndrome of acute heart failure is marked by a recent onset of symptoms usually in terms of days to a few weeks of worsening fatigue, shortness of breath, orthopnea, swelling, and sudden onset of weight gain. Physicians caring for patients with heart failure must know the risk factors for this disease, pathophysiology, symptomatology, important examination findings, key diagnostic tests, and management approach so as to improve symptoms and reduce mortality.

Outpatient management of transient ischaemic attack.

Stroke is a significant cause of death and disability in Singapore; in 2014, it was the fourth most common cause of death. Transient ischaemic attack (TIA) is defined as a transient episode of neurological dysfunction caused by focal brain, spinal cord or retinal ischaemia without evidence of acute infarction. The diagnosis of TIA/acute stroke needs to be considered in all patients who present with sudden focal neurological dysfunction. Prompt referral for assessment, neuroimaging and intervention provides the best chance for neurological recovery and/or minimising further neurological damage.

Heart Failure Update: Outpatient Management.

Outpatient management of heart failure (HF) is aimed at treating symptoms and preventing hospitalizations and readmissions. Management is initiated in a stepwise approach. Blockade of the renin-angiotensin system is a cornerstone of therapy and should be started, along with beta blockers, as soon as the diagnosis of HF is made. Other drugs, including diuretics, aldosterone antagonists, hydralazine, and nitrates, may be added based on symptoms and American College of Cardiology/American Heart Association stage. Despite a great interest in and theoretical benefit of naturoceutical products in th

Outpatient management of Gitelman's syndrome in pregnancy.

Gitelman's syndrome is a congenital renal tubular defect which affects the apical membrane of the distal convoluted tubule of the renal system. The syndrome is characterised by hypokalaemia, hypomagnesaemia, metabolic alkalosis and hypocalcuria. There are only a few cases describing the impact of Gitelman's syndrome on pregnancy and the foetus. Although most pregnancies have favourable outcomes, fetal demise has been reported in the third trimester. We report the successful outcome of pregnancy in a patient with Gitelman's syndrome who continued on amiloride in pregnancy to optimise potassium

Overtreatment and undertreatment of hyperlipidemia in the outpatient setting.

BACKGROUND: National guidelines recommend lipid-lowering therapy for patients with coronary heart disease (CHD), its equivalent (eg, diabetes mellitus, peripheral arterial disease, cerebrovascular disease), and those at high risk of CHD. Quality-of-care studies demonstrate that patients at high risk of CHD are undertreated. Overtreatment of patients at low risk of CHD remains relatively unexplored. Our study aimed to determine patient characteristics associated with under- and overtreatment of hyperlipidemia. METHODS: We conducted a retrospective chart review of patients aged 35 to 80 years a

[Outpatient surgical treatment of primary hyperparathyroidism].

The standard of care for primary hyperparathyroidism is minimally invasive surgical removal of hyperfunctional parathyroid tissue. Abroad, this minimally invasive approach is often performed in the ambulatory setting. Patients with primary hyperparathyroidism are eligible for outpatient surgery if the risk of conducting the operation is low and various imaging techniques have confirmed the location of the parathyroid adenoma. Of 20 patients with primary hyperparathyroidism who had been treated at our hospital's day surgery department, 5 visited the emergency department the next day because of

[The surface anesthesia in outpatient pediatric department].

The combination of even parts lidocaine and prilocaine in crème substance was used for surface anesthesia in 73 children, aged 1,5-16 years. Such surgical interventions as molluscum contagiosum eradication, laser wart resection and prepuce synechia dissection were performed. The described anesthetic was highly effective in all areas, but the nasolabial triangle. Resection of warts larger then 0,5 sm required either additional infiltrative anesthesia or step-wise resection. Local allergic reaction was registered in one case, no systemic reactions were noticed. Generally, the used method of surf

[Outpatient diagnosis of coronary artery disease].

Coronary artery disease either presents with acute chest pain or with exercise induced chest symptoms or shortness of breath. The differentiation between stable and unstable Angina pectoris is prognostically important, unstable angina is managed as an acute coronary syndrome including hospital admission, patients with stable symptoms can be further evaluated in an outpatient setting. A broad differential diagnosis of other cardiac and non-cardiac causes must be considered. Important initial diagnostic steps are cardiovascular risk stratification and prove of ischemia (or scar, necrosis) either

Outpatient microthoracoscopic sympathectomy for palmar hyperhidrosis.

BACKGROUND: Sympathectomy for palmar hyperhidrosis has been performed for more than 80 years with excellent long-term results. However, several issues are still unresolved: best surgical approach, which level and the number of levels of the sympathetic chain should be divided and the safety of an outpatient procedure. METHODS: We reviewed the records of 205 patients who underwent sympathectomy for hyperhidrosis at our institution from April 2002 through March 2005. Fifty consecutive patients (24%) underwent an outpatient microthoracoscopic sympathectomy of a single level (T2) for palmar hyper

Outpatient management of cirrhosis: a narrative review.

Cirrhosis is the 12th leading cause of death in the United States. Individuals with cirrhosis are at risk for many potential complications. Complications can be managed or detected early with proper outpatient management. The most lethal of these complications is bleeding esophageal varices. All patients with cirrhosis should be screened for the presence of varices and treated when indicated. The most common complication seen in these patients is ascites. Ascites can be treated with dietary modifications and a diuretic regimen. Other potential complications include spontaneous bacterial perito

[The state of pediatric urological outpatient surgery].

Economic requirements and the wishes of the patients have led to a clear increase in outpatient surgery. Besides the observance of legal conditions, a well-devised structure of organization is the prerequisite for a frictionless realization. The operating surgeon decides about the suitability of the patient, is responsible for a course without complications and takes on the postoperative care. The most frequent operations are circumcision, orchidopexy and the repair of inguinal hernias. Outpatient surgery stands out for a low rate of complications and a high satisfaction of the patients.
