Overweight is a risk factor for both erosive and non-erosive reflux disease.

INTRODUCTION: Studies have reported that the association between overweight and erosive oesophagitis is very strong, whilst seeming less consistent with non-erosive reflux disease. AIM: We have hypothesized that this difference may be due to the heterogeneity of endoscopy-negative population. METHODS: We studied 81 patients with erosive oesophagitis, 48 controls and 295 endoscopy-negative patients classified by impedance-pH-testing as: (1) pH-POS (abnormal acid exposure); (2) hypersensitive oesophagus (normal acid exposure/SAP+); (3) functional heartburn (normal acid exposure/SAP-). Body mas…

Myocardial perfusion image in asymptomatic postmenopausal women with physical inactivity and overweight.

OBJECTIVE: This study aimed to investigate whether physical inactivity and overweight deteriorate myocardial perfusion and cardiac function in asymptomatic postmenopausal women. METHODS: A prospective cross-sectional study design was used. Postmenopausal women recruited in this study were categorized into groups based on activity level and BMI: physically active, normal-weight group (PANw, n = 15, 60.9 ± 6.0 years), physically inactive, normal-weight group (PIANw, n = 13, 57.2 ± 5.0 years) and physically inactive, overweight group (PIAO, n = 26, 58.5 ± 5.8 years). Each subject took cardiopulm…

Impact of overweight on survival and primary graft failure after heart transplantation.

BACKGROUND: Overweight influence on prognosis after heart transplantation remains controversial. The objective of this study was to determine the relationship between overweight and survival in this setting. METHODS: We evaluated 271 cardiac transplant recipients engrafted from 1991 to 2009 whose age was 50.3±12.1 (mean; SD) years. Their body mass index (BMI) was 25.7±3.9 kg/m2. Regarding gender, 85.2% were males. Patients were classified by pretransplantation BMI into group A (>25 kg/m2) and group B (<25 kg/m2). We analyzed the differences in mortality and related factors. RESULTS: One hund…

The activation of the inflammatory cytokines in overweight patients with mild obstructive sleep apnoea.

It is widely accepted that obstructive sleep apnoea (OSA) is linked with cardiovascular diseases. The relationship is complex and remains still poorly understood. The presence of chronic systemic inflammation has been connected with pathogenesis of both OSA and cardiovascular diseases. While atherogenesis is believed to be a process of many years, little is known about the potential impact of the largest OSA subgroup, mild OSA, on the development of cardiovascular diseases. The aim of the present study was to assess whether untreated mild OSA is associated with an activation of inflammatory cy

Prediction of maximal oxygen uptake by bioelectrical impedance analysis in overweight adolescents.

AIM: Maximal oxygen uptake (VO(2max)), the gold standard for measurement of cardiorespiratory fitness, is frequently difficult to assess in overweight individuals due to physical limitations. Reactance and resistance measures obtained from bioelectrical impedance analysis (BIA) have been suggested as easily obtainable predictors of cardiorespiratory fitness, but the accuracy with which ht(2)/Z can predict VO(2max) has not previously been examined in overweight adolescents. METHODS: The impedance index was used as a predictor of VO(2max) in 87 overweight girls and 47 overweight boys ages 12 to…

Diabetes and overweight associate with non-APOE4 genotype in an Alzheimer's disease population.

Type 2 diabetes is a risk factor for late-onset Alzheimer's disease (AD), and studies suggest that pathogenic effects of diabetes and insuli … her potentially confounding variables. We also examined for interactions involving weight status as overweight and obesity are independent risk factors for insulin resistance, diabetes and AD. Prevalence of diabetes was 13% among AD cases without an APOE4 allele and 5-6% among AD cases with one or two APOE4 alleles. Odds ratio for diabetes was 0.26 [95% CI: …

Asthmatic symptoms, physical activity, and overweight in young children: a cohort study.

OBJECTIVE: Prevalence of asthma and overweight has increased simultaneously during the past decades. Several studies have reported an association between these two health problems, but it is unclear whether this relation is causal. We hypothesize that children with asthmatic symptoms are less physically active, which may contribute to the development of overweight. PATIENTS AND METHODS: The study included children from the KOALA Birth Cohort Study who were invited at 4 to 5 years of age to wear an Actigraph accelerometer for 5 days (n = 305; 152 boys). Information on wheezing was gathered by …

Is there a clear link between overweight/obesity and sleep disordered breathing in children?

The proportion of children who are overweight or obese has risen steadily in recent decades and approaches a quarter of all children in Western countries. This global epidemic of excess weight and adiposity in humans is associated with increased morbidity and mortality, especially related to diabetes and poor cardiovascular health. It would appear that obesity is also generally accepted to be an important risk factor in the development of sleep disordered breathing (SDB), in children as well as adults. The article, "The prevalence, anatomical correlates and treatment of sleep-disordered breath…

Polysomnographic evidence of sleep apnoea disorders in lean and overweight haemodialysis patients.

BACKGROUND: Sleep apnoea disorders affect a large proportion of patients with renal failure. However, it is unknown whether body composition and visceral adiposity predispose haemodialysis patients to sleep apnoea disorders. METHODS: Forty-one dialysis patients were divided in two groups according to the BMI score as; the Lean group (N=21, 1F/20M, BMI = 21.3 +/- 2.1) and the Overweight group (N=20, 9F/11M, BMI = 28.3 +/- 2.8). Sleep disturbances assessed by a full polysomnography (PSG), visceral adipose tissue (VAT), calculated by computed tomography, the quality of life, assessed by the SF-3…

Characteristics of monozygotic male and female twins discordant for overweight: a descriptive study.

OBJECTIVES: BMI is highly heritable. Yet, trends in obesity highlight environmental influences on body weight. Monozygotic (MZ) twins discordant for overweight offer a unique opportunity to examine these factors. METHODS: MZ male (n=8 pairs) and female twins (n=10 pairs) discordant for overweight (defined as the BMI of one twin being at least 24.5, and the BMI of his/her co-twin being below 24.5 and at least three points lower) were identified from the Mid-Atlantic Twin Registry. Variables were assessed via self-report questionnaires. RESULTS: One overweight and two non-overweight females me…

Effects of sibutramine and orlistat on mood in obese and overweight subjects: a randomised study.

BACKGROUND AND AIM: Intentional weight loss results in improvement in mood. Very few data exist regarding the effects of sibutramine on the mood of obese and overweight patients in general clinical samples. Moreover, no study has evaluated the effects of orlistat treatment on mood. The purpose of our study was to assess the effects of sibutramine and orlistat on mood in obese and overweight subjects. METHODS AND RESULTS: Sixty obese and overweight women were divided into three groups. The first group (n=20) received a low-calorie diet and sibutramine 10mg; the second group (n=20) received a l…

Neonatal nucleated red blood cells in infants of overweight and obese mothers.

OBJECTIVE: The perinatal outcome of the infant of obese mother is adversely affected and in theory, may involve fetal hypoxia. We hypothesized that an index of fetal hypoxia, the neonatal nucleated red blood cell (NRBC) count, is elevated in infants of overweight and obese mothers. STUDY DESIGN: Absolute NRBC counts taken during the first 12 hours of life in 41 infants of overweight and obese mothers were compared to 28 controls. RESULTS: Maternal body mass index and infant birthweight were significantly higher in the overweight and obese group (P < 0.01). Hematocrit, corrected white blood c…

Differences between the perceived and actual age of overweight onset in children and adolescents.

OBJECTIVE: Little is known about whether children or their parents can accurately recall the age at which they became overweight. DESIGN, SUBJECTS AND MAIN OUTCOME MEASURES: We interviewed 64 overweight children (7-18 years old) about their weight history and compared reported age of overweight onset with actual onset, as determined by the age at which the child's measured BMI first exceeded the 95th percentile. RESULTS: Only 28% of children reported overweight onset within 1 year of actual overweight onset. Reported overweight onset age (7.6 +/- 2.5 y) and actual onset age (5.3 +/- 2.5 y; P …

Influence of excess adiposity on exercise fitness and performance in overweight children and adolescents.

OBJECTIVE: Relatively little is known about how excess body mass affects adolescents' capacity to perform sustained exercise. We hypothesized that most of the difficulty that severely overweight adolescents have with sustained exercise occurs because the metabolic costs of moving excess mass result in use of a high proportion of their total oxygen reserve. METHODS: We compared results from a maximal cycle ergometry fitness test in 129 severely overweight adolescents who had BMIs of 41.5 +/- 9.7 kg/m2 and ages of 14.5 +/- 1.8 years (range: 12.1-17.8 years) and 34 nonoverweight adolescents who …

Arcuate neurons of overweight rats differ in their responses to amylin from controls.

Amylin and ghrelin are hormones produced, respectively, in pancreas and stomach. They have a central action on food intake and body weight. Possible changes in their effect on hypothalamic neuronal activity were investigated in overweight rats previously subjected to early postnatal overnutrition compared to controls. Single unit activity of brain slices was recorded in the medial arcuate that contains neuropeptide Y producing neurons. The orexigenic ghrelin activated neurons of both groups of rats. The anorexigenic amylin, however, significantly inhibited neurons of controls but not of overwe…

Mammographic density in relation to daidzein-metabolizing phenotypes in overweight, postmenopausal women.

Circulating hormones are associated with mammographic density, an intermediate marker of breast cancer risk. Differences in circulating hormones, including estrone and testosterone, have been observed in premenopausal women based on their capacity to metabolize daidzein, an isoflavone found predominantly in soybeans. Equol and O-desmethylangolensin (O-DMA) are products of intestinal bacterial metabolism of daidzein. There is interindividual variability in the capacity to produce daidzein metabolites; individuals can be equol producers or non-producers and O-DMA producers or non-producers. We t

Effects of sibutramine on resting metabolic rate and weight loss in overweight women.

Sibutramine, a monoamine re-uptake inhibitor, has recently been approved by the Food and Drug Administration as a weight loss agent. Sibutramine lowers bodyweight in rodents by reducing energy intake and increasing energy expenditure. Sibutramine facilitates weight loss in human subjects, but it is not clear whether it acts on energy intake, energy expenditure, or both. The present study was a randomized clinical trial designed to assess the effects of sibutramine (at 10 or 30 mg/day) on body weight and resting metabolic rate (RMR). Forty-four overweight women were randomized to 1) placebo (n=…
