Atypical facial pain was first described by Temple Fay in 1927 as a vascular syndrome of dull, throbbing pain situated deep in the eye and malar region often referred toward the ear, lateral neck, and shoulders. When this syndrome is encountered in medical practice it is often perplexing to the physician and frustrating to the patient. A systematic approach to this problem will aid in the diagnosis and possible treatment which may be facilitated by a thorough understanding of the anatomic pathways involved in various types of acute and chronic facial pain. Diagnostic techniques and therapeutic…

Pain along the vertebral column is such a frequent complaint that it is easy to overlook unusual etiologies or serious complications. In addition to the common traumatic and degenerative causes of back pain, infectious, rheumatic and neoplastic conditions must also be considered. Nonmusculoskeletal disorders may sometimes be mistaken for lumbosacral strain. A systematic and sympathetic approach to the diagnosis may uncover uncommon pathologies.

Myogenic prosopalgia is one of the forms virtually unfamiliar to practitioners. In a majority of cases it belongs to the category of pain-related myofascial dysfunctions present in about 10% of patients with facial pains. This work deals with etiology, pathogenesis, clinical features, diagnosis and treatment of the disorder.

A 52 year old patient was admitted for retrosternal pain not responding to nitroglycerin. Two years before he had suffered myocardial infarction. He had known cholecystolithiasis. Reinfarction was excluded, but the patient developed right upper quadrant abdominal pain with rebound tenderness, fever and leukocytosis. Abdominal sonography supported the diagnosis of acute cholecystitis. Acute illness resolved rapidly without complications under treatment with antibiotics. The patient underwent cholecystectomy during the free interval four weeks after discharge from the hospital. Intraoperative di…

Seventy-three patients with mid-dorsal and/or unilateral chest pain seen consecutively in the rheumatology clinic by a single clinician over a three-year period were studied, after exclusion of visceral disease. The majority were young women. The pain was dull and continuous, was aggravated by coughing and sneezing and relieved by rest. There was frequently tenderness over the thoracic spine (T4-5) and an adjacent rib, and pain at extremes of thoracic spinal movement in one or two directions was invariable. Cutaneous hyperaesthesia in a radicular distribution was found in 16.4%, but there were…

The incidence and clinical characteristics of sharp, jabbing pain about the head were studied in 100 migraineurs and 100 control subjects. Among the controls, 3% had experienced paroxysmal sharp cranial pain, whereas 42% of the migraineurs had made this observation (p less than 0.001) and half of them experienced it more often than monthly. The pain was usually (45%) unifocal at the temple or orbit, was described as icepick-like by 52% of the patients, and was often (69%) experienced concurrently with headache. Icepick-like pain appears to be a manifestation of migraine and should be distingui…

Case histories of 225 patients with intractable pain attending the Pain Clinic of a large metropolitan teaching hospital were retrospectively analysed. The 29 cases of intractable facial pain were investigated to determine if there were any clinical or historical factors which might indicate intractability and thus speed diagnosis of such cases in future. Significant factors were age, duration of pain, number of surgical interventions and psychiatric factors. Patients with intractable facial pain are more likely to be over 40 years of age, to have pain of long duration, to have had one or more…

Acute abdominal pain is a daily problem in a busy pediatric practice and in hospital emergency rooms. An initial, carefully performed history and physical examination should differentiate the child who has a surgical disease from one who requires medical therapy or only reassurance. Whenever there is doubt about the diagnosis, the child must be admitted to the hospital for observation and appropriate laboratory examinations. This will allow for the early diagnosis of appendicitis and other diseases requiring an operation and will prevent needless operations on children with medical illnesses.

Back pain is one of the chief complaints of the elderly. It may be either a chronic deep skeletal muscular pain or an acute circumscribed pain arising from nerve-root irritation. The main causes of back pain in older people are: 1) degenerative changes (spondylosis, osteoarthritis, ankylosing hyperostosis); 2) malignancy (multiple myeloma, metastases from carcinoma or lymphoma); and 3) metabolic disorders (osteoporosis, osteomalacia, chondrocalcinosis, Paget's disease). Mechanisms and variations are discussed in detail.
