[Choice of anesthetic method and prevention of ophthalmic complication in sphenoid sinus endoscopic surgery].

OBJECTIVE: To evaluate the anesthetic method and the prevention of the ophthalmic complication in endoscopic surgery for sphenoid sinus disease. METHOD: All 65 patients underwent local anesthesia with improved surface anesthesia. Arrest blood completely in the surgical operation. The position of sphenoid sinus, the assimilation and destruction of sinus wall, the location and the appearance of lesion were determined carefully. To avoid damaging the optic nerve, the behavior of eyes was observed intently. RESULT: The follow up period ranged from 6 months to 2 years. Five patients were lost to

INI1-Intact Sinonasal Carcinoma with Rhabdoid Features.

Sinonasal malignancies are known for their associated poor prognosis and diversity of histologic features. While poor prognosis is largely due to advanced disease at presentation, histologic features also play a significant role. Therefore, accurate pathologic diagnosis is of utmost importance. Here, we describe a 63-year-old male with chronic left-sided nasal obstruction and left-sided epistaxis who was found to have a large mass occupying most of the nasal cavity extending through the nasopharynx to just below the nasopharyngeal surface of the soft palate. During surgical excision, the mass

Endoscopic medial maxillectomy with preservation of inferior turbinate and nasolacrimal duct.

BACKGROUND: Endoscopic medial maxillectomy (EMM) is a safe and effective procedure for treatment of inverted papilloma (IP) originating from the maxillary sinus. However, EMM usually removes the inferior turbinate and nasolacrimal duct. The inferior turbinate has a critical function in conditioning of the nasal airflow, and resection of the nasolacrimal ducts has a risk of epiphora. We developed a newly derived surgical technique, endoscopic modified medial maxillectomy (EMMM), which enables preservation of the inferior turbinate and nasolacrimal duct. METHODS: A retrospective case series of

Respiratory epithelial adenomatoid hamartoma of bilateral olfactory clefts associated with nasal polyposis: three cases report and literature review.

We report three cases of patients with respiratory epithelial adenomatoid hamartoma (REAH) of bilateral olfactory clefts associated with nasal polyposis. REAH is a kind of rare lesion which commonly occurs in nasal cavity, paranasal sinuses, and nasopharynx. Our cases were all localized in bilateral olfactory clefts which is very rare since the most common site reported in the past in nasal cavity is the posterior nasal septum and unilateral. Furthermore, our presented cases all associated with nasal polyposis which can provide greater evidence that inflammation can induce REAH development. Al

Nasolacrimal Duct Management During Endoscopic Sinus and Skull Base Surgery.

OBJECTIVE: To evaluate rates of epiphora after transection and marsupialization of the nasolacrimal duct (NLD) during endoscopic sinus and skull base surgery. INTRODUCTION: The nasolacrimal canal forms part of the medial wall of the maxillary sinus. Transecting the NLD is sometimes necessary for tumor resection or surgical access to maxillary sinus and infratemporal fossa pathology. There is no consensus for the endoscopic management of the NLD when only the duct is transected without involving the nasolacrimal sac. METHODS: Medical records of 29 patients from 2 academic institutions who und

Rare Diseases of the Nose, the Paranasal Sinuses, and the Anterior Skull Base.

Due to their low incidence and thus resulting limited diagnostic criteria as well as therapeutic options, rare diseases of the nose, the paranasal sinuses, and the anterior skull base are a significant challenge. The value as of which a disease has to be considered as rare amounts to a maximum of 5 patients per 10 000 people. Within these diseases, however, there are extreme differences. Some rare or orphan diseases like for example the inverted papilloma belong to regularly diagnosed and treated diseases of larger departments of oto-rhino-laryngology whereas other rare diseases and malformati

Endoscopic treatment of the isolated sphenoid sinus lesions.

Presenting symptoms of the isolated sphenoid sinus lesion are often vague and non-specific. Diagnostic nasal endoscopy procedures and imaging techniques are of great value for an early and precise diagnosis. Moreover, endoscopic sinus surgery is a safe and effective technique that allows a direct route to the sphenoid sinus. Because of its close vicinity to important and vulnerable structures of the skull base, delay in diagnosis and treatment can be potentially lethal. Endoscopically controlled procedures for the sphenoid sinus provide the surgeon with an obvious alternative to the traditiona

Immunoperoxidase demonstration of human papilloma virus (HPV) in dysplastic lesions of the uterine cervix.

Routinely processed sections of 47 unselected cervical dysplasias and in situ carcinomas (both condylomatous, CO, and noncondylomatous, NCO) were subjected to staining with indirect immunoperoxidase-PAP technique using anti-human papilloma virus (anti-HPV) immune serum to demonstrate HPV antigens in these lesions. Anti-HPV was raised in guinea pigs immunized with highly purified virions from a pool of human skin wart tissues. Positive reactivity for HPV (brown intranuclear precipitates) was disclosed in 72% of the CO dysplasias and in only one case (5.6%) of the NCO group. The frequency of HPV

Condylomatous lesions associated with precancerous changes and carcinomas of the uterine cervix.

The present communication is a comparative survey of the histological specimens derived from the dysplastic or neoplastic lesion of the uterine cervix of 400 women comprising two age-matched series (200 women in each) collected from two Finnish hospitals. Special emphasis was placed on the detection of the newly described condylomatous lesions (flat, inverted, papillomatous) and the possible variations in their biological behavior in the two series. The biological behavior of the condylomatous lesions (the distribution into the three types, their high frequency in young women, and their relati

Frontal sinus obliteration with autologous calvarial bone graft: indications and results.

Despite increasing advances in endonasal frontal sinus surgery, frontal sinus obliteration (FSO) is sometimes necessary after failure of other surgical techniques. This procedure has been reported with autologous tissue or synthetic material, but few studies have reported results with autologous calvarial bone graft. The aim of this study was to report our experience with osteoplastic FSO calvarial bone graft. A retrospective review was performed on 11 patients operated upon for FSO with autologous calvarial bone graft from 2005 to 2011. Obliteration was indicated for chronic symptomatic front

Endoscopic Denker's Approach for the Treatment of Extensive Sinonasal Tumors: Our Experience.

OBJECTIVES: Surgical management of large sinonasal tumors, spreading to the pterygopalatine, and infratemporal fossae have always been a challenge for the ENT surgeon and the multidisciplinary team. METHODS: Endoscopic Denker's approach allows complete exposure of the anterior, inferior, and lateral recesses of the maxillary sinus, without necessitating a gingivobuccal or a transseptal incision. Moreover, endoscopic Denker's approach facilitates direct, straight entry to the pterygopalatine and infratemporal fossae. RESULTS: Twenty-two patients with extensive sinonasal pathology underwent re

Complete resolution of a large squamous cell carcinoma of the lacrimal duct in a young African American male after non-surgical management.

PURPOSE: The following case describes a 28-year old African American male who presented with squamous cell carcinoma of the lacrimal duct. Despite its size and location, his tumor showed complete resolution with non-surgical treatment. OBSERVATIONS: The patient presented with a 4-month history of dacryocystitis-like symptoms in the right eye, with no prior medical history and a history of tobacco smoking. Pathology indicated that SCC arose as nests within a benign sinonasal inverted papilloma. He received cisplatin chemotherapy and radiotherapy with tumor resolution in 4 months. Several month

14-3-3 gene expression exerts isoform-dependent functions in sinonasal pathophysiology.

The expression profiles of 14-3-3β and θ isoforms, known to exert both oncogenic and antiapoptotic effects, were assessed in different entities of nasal pathophysiology. Flow cytometry and immunohistochemistry were used on paraffin-embedded sections of 51 inverted papillomas (IP), 26 nasal polyps (NP), 9 polyps with IP (NPIP) and 10 specimens of normal epithelium (NE). 14-3-3β expression was significantly upregulated in IP as compared with both NP (p=0.015) and NE (p=0.002). 14-3-3β was also increased in NPIP as compared with NE (p=0.008). 14-3-3β cytoplasmic staining was more pronounced in ba
