The clavi-pectoral route provides a limited approach to the axillary vessels as they emerge from the costal-subclavian outlet. The incision is oblique, following the course of the axillary vein. Neonatal lupus erythematosus (NLE), in most cases, presents with cardiac and dermatological manifestation due to transferred IgG auto antibodies (anti Ro/SSA and anti La/SSB) from the mother. Some unusual associations with myelopathy, vasculopathy, transient myasthenia gravis, congenital nephrotic syndrome, chondrodysplasia punctata etc. are also reported. Poland syndrome is a constellation of anomalies including absent pectoralis muscle and rib defects, even with malformations of the upper extremity that more often affects boys than girls. This case describes a 5-year-old and 4-month-old girl diagnosed with Poland syndrome that required chest wall reconstruction. An autologous rib transplantation and terylene patch were used for repair of the chest wall defect. The girl had a satisfactory postoperative recovery. This technique was an effective method of treatment of this condition.

Tracheoinnominate fistula (TIF) is a rare condition with significant potential for mortality if surgical intervention is not immediate. We present two cases of successfully managed TIF. Both cases involve ligation and resection of the innominate artery at the TIF followed by a pectoralis major muscle flap. In both cases, success was largely due to a high index of suspicion and immediate control of the bleeding with transport to the operating room for surgical repair. The history, aetiology, and pathogenesis of TIF are reviewed, yielding an algorithm for recommended management of TIF.

OBJECTIVE: To explore the effect of skin flaps and muscular flaps transplanted in the use of tissue defect repairing.
METHOD: Thirty cases of tissue defect were repaired or reconstructed by forearm flaps, pectoralis major muculocutaneous flaps, frontal flaps, nasolabial fold flaps or paranavel rectus abdominis muscle flaps.
RESULT: Twenty-nine cases were successful but one case was unsuccessful.
CONCLUSION: Skin flaps and muscular flaps reconstruction are safe and reliable. But the indication should be selected and the complication should be prevented.

Von Recklinghausen neurofibromatosis (NF1) is one of the most common autosomal dominant disorders and has one of the highest mutation rates of a single human locus. The NF1 gene has recently been mapped to proximal 17q. Poland anomaly consists of the combination of unilateral aplasia of the sternal head of the pectoralis major muscle and an ipsilateral anomaly of the hand. Although other defects may occur in patients with Poland anomaly, neurofibromatosis 1 has not been reported. We have seen a boy with these two disorders.
