Management of Vertical Root Fracture Using Intentional Reimplantation and Radicectomy: A Case Report.

This case report describes the management of a clinically suspected vertical root fracture (VRF) using intentional reimplantation combined with radicectomy and documents the 36-month outcome with cone-beam computed tomography (CBCT). A 63-year-old man presented with spontaneous pain and intense pain on mastication associated with tooth 46. Clinical examination showed grade II mobility, positive percussion and buccal palpation, and an isolated 10-mm periodontal defect with bleeding and suppuration on the buccal aspect of the mesial root. Periapical radiography showed mesial-root underfilling an

Risk factors for the development of medication-related osteonecrosis of the jaw and effects of tooth extraction with local infection.

BACKGROUND/PURPOSE: Tooth extraction has been avoided in patients receiving antiresorptive agent (ARA) therapy. This study aimed to investigate dental findings associated with medication-related osteonecrosis of the jaw (MRONJ) development in patients. MATERIALS AND METHODS: First, in patients treated with high-dose ARAs, the relationship between dental findings and MRONJ development was examined. Next, in patients with MRONJ undergoing surgery, the relationship between dental findings and MRONJ occurring at a site distant from the initial site was examined. RESULTS: MRONJ occurred in 13 of

The extent of pyroptosis varies in different stages of apical periodontitis.

Apical periodontitis (AP) is an inflammation affecting the periapical region of tooth root. Microbial pathogens activate inflammasomes and promote the production of pro-inflammatory cytokines. Caspase-1-mediated pyroptosis is a possible mechanism involved in the initiation and progression of AP. The purpose of this study was to evaluate the role of caspase-1 and pyroptosis on AP at different stages. Human periapical inflammatory tissue was collected to study chronic AP stage. Human periodontal ligament fibroblasts (hPDLFs) were stimulated with lipopolysaccharide in vitro for 24h to simulate ea

Histological assessment of pulp condition after apical vital root transection in one root of multirooted teeth in dogs: a preliminary study.

One of the most important aspects in surgery is the healing process after the periapical surgery. Past studies have shown occasional encounters with vital root resection and have noted varying degrees of pulpal response after root resection in periodontal disease. The purpose of this investigation was to observe the pulpal and periapical responses to intentional apical vital root transection in one root of multirooted teeth of German-Canadian dogs over a 6-month postoperative period. This is an experimental study performed on left maxillary and mandibular quadrants of four adult German-Canadia

Systemic Factors Affecting Prognosis and Outcome of Endodontic Therapy.

The successful outcome of endodontic treatment is dependent on the immune response and the reparative potential of the individual. Alteration in the host immune response is a common characteristic shared by both apical periodontitis and systemic diseases. Although infection-induced periapical lesions occur in a localized environment, numerous epidemiologic studies in the last few decades have investigated the potential association between endodontic disease pathogenesis and systemic diseases. The goal of this review is to identify common systematic factors and discuss the effect they may or ma

Differential in situ distribution of interleukin-8, monocyte chemoattractant protein-1 and Rantes in human chronic periapical granuloma.

In situ distribution of three prototype chemokines interleukin (IL)-8, monocyte chemoattractant protein (MCP)-1 and Rantes was determined in chronic human periapical granulomas by immunohistochemistry using monoclonal antibodies. IL-8 was found primarily in the cytoplasm of the Malassez epithelial cells. MCP-1 immunoreactivity was confined to the endothelial cells that lined small venules. Each of the three investigated chemokines, including Rantes, exhibited a characteristic binding pattern to the extracellular matrix of the lesion. The observed chemokines may play a role in establishing the

Specialized postcapillary venules in human gingival tissue.

High endothelial venules (HEV) are specialized postcapillary venules that play a central role in lymphocyte migration and recirculation. This study examined the occurrence and morphology of HEV-like vessels in healthy gingiva and in lesions of chronic gingivitis in children, experimental gingivitis in adults, adult periodontitis, and in periapical granulomas. HEV-like vessels were absent in gingival health, but present during the development of inflammation and in chronic inflammation. The morphology of these HEV-like vessels in the oral cavity resembled that of lymphoid HEV. Thus gingival HEV

The prevalence and healing of apical periodontitis in patients with autoimmune diseases.

Apical periodontitis (AP) is a common and clinically significant oral health condition, associated with an inflammatory response to infections within the root canal system. As patients retain natural teeth for longer, managing AP becomes more complex. Whilst generally effective, endodontic treatment outcomes can vary considerably in individuals with systemic health conditions, such as autoimmune diseases. The intersection of systemic inflammation, immune dysfunction and pharmacological treatments of the different diseases raises important questions about how autoimmune diseases influence AP pr

High-performing cross-dataset machine learning reveals robust microbiota alteration in secondary apical periodontitis.

Multiple research groups have consistently underscored the intricate interplay between the microbiome and apical periodontitis. However, the presence of variability in experimental design and quantitative assessment have added a layer of complexity, making it challenging to comprehensively assess the relationship. Through an unbiased methodological refinement analysis, we re-analyzed 4 microbiota studies including 120 apical samples from infected teeth (with/without root canal treatment), healthy teeth, using meta-analysis and machine learning. With high-performing machine-learning models, we

Effectiveness of adjunct therapy for the treatment of apical periodontitis: A systematic review and meta-analysis.

BACKGROUND: Adjunct therapy refers to any intracanal procedure going beyond chemomechanical preparation with instruments and traditionally delivered irrigants (excluding interim dressings). It is not clear whether and which of these adjunct therapies have a significant impact on the outcome of root canal treatment [healing of apical periodontitis (AP) and other patient-related outcomes]. OBJECTIVES: This systematic review aimed to analyse available evidence on the effectiveness of adjunct therapy for the treatment of AP in permanent teeth, according to a population, intervention, comparison,

Patient-related outcomes and Oral Health-Related Quality of Life in endodontics.

There is increased interest in outcome measures that complement the classic goal of endodontics, being the prevention and healing of apical periodontitis. Possible outcome measures can be subdivided into patient-centred outcomes (PCOs), encompassing data collected by nonpatients aiming to assess the effectiveness of an intervention and patient-reported outcomes (PROs), which are reported directly by the patient. PROs can be subsequently classed into PRO measures that ascertain patients' view of their symptoms, their functional status, and their health-related quality of life, and patient-repor

Specialized pro-resolving lipid mediators in endodontics: a narrative review.

Endodontics is the branch of dentistry concerned with the morphology, physiology, and pathology of the human dental pulp and periradicular tissues. Human dental pulp is a highly dynamic tissue equipped with a network of resident immunocompetent cells that play major roles in the defense against pathogens and during tissue injury. However, the efficiency of these mechanisms during dental pulp inflammation (pulpitis) varies due to anatomical and physiological restrictions. Uncontrolled, excessive, or unresolved inflammation can lead to pulp tissue necrosis and subsequent bone infections called a
