Effects of Topical Steroids on Postoperative Pain in Patients Undergoing Stand-Alone Lateral Lumbar Interbody Fusion.

Background Postoperative pain in the psoas and femoral nerve distribution is common following lateral lumbar interbody fusion (LLIF) surgery. Topical steroids have shown beneficial effects in spinal surgery. The effect of topical steroids applied directly to the psoas and the traversing nerve complexes in patients undergoing stand-alone LLIF (SA-LLIF) is unclear. Purpose The purpose of this retrospective cohort study is to investigate the efficacy of topical steroids in reducing postoperative pain when applied to the psoas muscle during SA-LLIF. Methods Patients who underwent SA-LLIF were incl

Topical antibiotic therapy: current status and future prospects.

As we enter a new decade, topical antibiotics are the subject of much renewed interest and are being used on a wider scale than ever before. The reasons for using topical rather than oral therapy for a variety of dermatoses include the reduced risk of systemic side effects, the avoidance of resistance selection in the gut microflora, the higher achievable concentration of antibiotic at the site of action and the overall usage of less drug. Somewhat surprisingly, treatment costs are not reduced by the use of topical therapy. The number of antibiotics licensed for topical use has increased in re

Prescription of high-potency corticosteroid agents and clotrimazole-betamethasone dipropionate by pediatricians.

Family physicians, internists, and pediatricians are more likely to prescribe combination topical antifungal-topical corticosteroid preparations than are dermatologists. One such preparation, clotrimazole-betamethasone dipropionate, can cause atrophy because it has a high-potency corticosteroid component. We analyzed data from the National Ambulatory Medical Care Survey for visits to pediatricians from 1990 to 1994 and isolated visits at which a topical corticosteroid agent or clotrimazole-betamethasone dipropionate was prescribed. Pediatricians rarely prescribed single-agent, high-potency top

Minocycline effects in patients with active trachoma.

Minocycline has a unique solubility in lipids and may reach therapeutic concentrations in tears and saliva. In two consecutive prospective double-masked clinical trials that were carried out in two villages in Saudi Arabia, we assessed the effects of oral minocycline in the treatment of trachoma and compared its effects with those of topical tetracycline ointment in the first study and to tetracycline ointment and placebo in the second study. A total of 178 eyes in 96 patients were included. The age range was 7 to 14 years, with a mean age of 9 years. All patients underwent complete ophthalmol

The Telehealth MOM Study: Protocol for a randomized controlled trial of the Telehealth Multi-component Optimal Model (MOM) of postpartum care.

BACKGROUND: A substantial proportion of maternal morbidity and mortality occurs in the postpartum period. In 2018, the American College of Obstetricians and Gynecologists (ACOG) published new guidelines recommending that all women have contact with a maternity care provider by 3 weeks postpartum. Although this revised schedule addresses the evident need for enhanced postpartum care, it has not been tested in a clinical trial and implementation has been hampered by logistical and financial constraints. The goal of this study is to evaluate the effect on postpartum outcomes of the Telehealth Mul

Are open science practices in dentistry associated with higher Altmetric scores and citation rates?

AIM: Open science, a set of principles and practices, aims to make scientific research more accessible and accountable, benefiting scientists and society. This study evaluated whether adopting open science practices (OSPs) correlates with higher citation rates and Altmetric scores. METHODS: A random sample of randomised clinical trials (RCTs) on dental caries published between 2000 and 2022 was selected. A systematic PubMed search identified relevant RCTs, and data on OSPs - study registration, open methodology, open software, open scripts, open analysis plan, open data, open peer review, and

Postoperative drainage for 6, 12, or 24 h after burr-hole evacuation of chronic subdural hematoma in symptomatic patients (DRAIN-TIME 2): study protocol for a nationwide randomized controlled trial.

BACKGROUND: Chronic subdural hematoma (CSDH) is a common acute or subacute neurosurgical condition, typically treated by burr-hole evacuation and drainage. Recurrent CSDH occurs in 5-20% of cases and requires reoperation in symptomatic patients, sometimes repeatedly. Postoperative subdural drainage of maximal 48 h is effective in reducing recurrent hematomas. However, the shortest possible drainage time without increasing the recurrence rate is unknown. METHODS: DRAIN-TIME 2 is a Danish multi-center, randomized controlled trial of postoperative drainage time including all four neurosurgical d

Scientific language trends among Swedish urologists and surgeons 1900-1955.

PURPOSE: Before English took the lead as the prime scientific language among northern European urologists and surgeons, German was widely regarded as the "lingua franca". This shift has to date not been systematically reconstructed. This article provides insights into the question how political and social factors influence how physicians communicate with each other, what they read, and how the constellations of international scientific communities in medicine change over time. METHODS: Through a language analysis of more than 2000 articles, including their references, in major Swedish medical

Publication bias, with a focus on psychiatry: causes and solutions.

Publication bias undermines the integrity of the evidence base by inflating apparent drug efficacy and minimizing drug harms, thus skewing the risk-benefit ratio. This paper reviews the topic of publication bias with a focus on drugs prescribed for psychiatric conditions, especially depression, schizophrenia, bipolar disorder, and autism. Publication bias is pervasive; although psychiatry/psychology may be the most seriously afflicted field, it occurs throughout medicine and science. Responsibility lies with various parties (authors as well as journals, academia as well as industry), so the mo

Prostitutes and criminals: beginnings of eugenics in Croatia in the works of Fran Gundrum from Oriovac (1856-1919).

Fran Gundrum (1856-1919) was a Croatian physician, encyclopedist, and an advocate of medical enlightenment and healthy lifestyle. In order to identify and analyze Gundrum's ideas about the problems of prostitution and criminality, we studied all of his books, booklets, and articles published between 1905 and 1914. We showed that Gundrum's theories of heredity, morality, and sexual hygiene incorporated many of the important discussions of his time, especially those related to the Darwinian paradigm. Gundrum's project of collecting statistics on prostitutes was the first such study published on

Psychiatric disorders and aggression in the printed media: is there a link? A central European perspective.

BACKGROUND: A content analysis was used to describe the association between psychiatric disorders and aggression in the printed media in the Czech Republic and Slovakia. METHODS: Articles were chosen from the most widely read daily newspapers and magazines in both countries during five one-week periods in 2007. A coding manual was developed and a content analysis was performed. Aggressive behavior was assessed by two separate categories - the role of the mentally ill person in the violent act (perpetrator/victim) and the type of aggressive act (homicide, suicide). RESULTS: A total of 375 art

Inhaled, nasal and topical corticosteroid burden and association with corticosteroid-associated adverse events.

BACKGROUND: Patients with type 2 inflammation-associated diseases are often treated with multiple formulations of corticosteroids. Long-term risks of using combinations of local corticosteroids are poorly characterised. OBJECTIVE: Using Danish nationwide databases, investigate associations between local corticosteroid exposure and 11 corticosteroid exposure-associated conditions compared to non-users, and eventual dose-response relationship for exposed individuals. METHODS: Systemic corticosteroid-naïve adults who redeemed inhaled, nasal and/or topical corticosteroids 2000-2012 were matched

Rate of Adverse Events and Healthcare Costs Associated with the Topical Treatment of Rosacea.

BACKGROUND: Rosacea is a condition more common in women than in men, and in people aged ≥30 years than in younger patients. Adverse events associated with the use of topical medications for rosacea may lead to a lack of treatment adherence. Previous studies have reported low treatment adherence rates among patients with rosacea. OBJECTIVE: To describe the rate of treatment discontinuation resulting from adverse events and the associated healthcare costs among patients with rosacea who are receiving a topical medication. METHODS: We conducted a retrospective cohort study of patients diagnosed

Enabling meta-analysis in systematic reviews on carpal tunnel syndrome.

Possible solutions to the problems of clinical heterogeneity of outcome measures and inadequate reporting of results for randomized controlled trials (RCTs) on carpal tunnel syndrome (CTS) are presented. Meta-analysis was impeded by these problems in 2 systematic reviews concerning conservative and surgical treatment options for CTS. A solution to the problem of inadequate data presentation is to add explicit information on minimal requirements with regard to data presentation to guidelines for the reporting of studies. To resolve the problem of clinical heterogeneity of the outcomes there sho
