"Wire syndrome"-induced mucogingival defect management using the laterally closed tunnel technique.

Wire syndrome is an iatrogenic complication, characterized by unexpected tooth movements during orthodontic retention, even without detachment of the retention wire. It can cause esthetic and functional damage through adverse mucogingival changes. This case report describes the management of gingival recession induced by wire syndrome using the laterally closed tunnel technique with a connective tissue graft. A diagnosis of biofilm-induced gingivitis on a reduced periodontium was established after clinical and radiographic evaluation. The graft was laterally stabilized, and suspended sutures e…

Salivary and serum alkaline phosphatase as biomarkers in type 2 diabetes mellitus: Association with periodontal health and disease.

Chronic periodontitis in patients with type 2 diabetes mellitus (T2DM) requires reliable biomarkers for early detection and monitoring. Therefore, it is of interest to evaluate serum and salivary alkaline phosphatase (ALP) levels in 90 participants divided into healthy controls, diabetics with healthy periodontium and diabetics with chronic periodontitis. Salivary and serum ALP levels increased progressively across the groups and were significantly highest in diabetics with periodontitis (p < 0.001). Strong correlations were observed between salivary ALP, serum ALP and periodontal parameters s…

Compromised Teeth Preserve or Extract: A Review of the Literature.

Multiple systems and associated factors have been described in the literature to assess the prognosis of teeth with periodontal disease. Now … ing the factors that should guide decision-making to maintain or extract a tooth with a compromised periodontium. Then, we propose a schematic diagram of prognostic indicators to reflect the main factors to consider and the survival rate that each one represents when preserving or extracting a tooth.

[BMAL1 gene regulates the osteogenic differentiation of bone marrow mesenchymal stem cells].

Periodontitis is a chronic infective disease characterized as the destruction of the supporting tissues of the teeth. Bone marrow mesenchymal stem cells, which are ideal adult stem cells for the regeneration of supporting tissues, may play important roles in restoring the structure and function of the periodontium and in promoting the treatment of periodontal disease. As a consequence, the characteristics, especially osteogenic differentiation mechanism, of these stem cells have been extensively investigated. The regulation of the physiological behavior of these stem cells is associated with B…

Orthodontic-surgical treatment of bilateral maxillary canine impaction.

A 13-year-old female patient reported with the chief complaint of irregular front teeth. She had a skeletal Class III and Angle's Class I malocclusion with hyperdivergent growth pattern and bilateral impaction of maxillary canines. Surgical exposure of the impacted teeth and orthodontic alignment was planned. The surgical exposure was done by a minimally invasive modified window technique. Orthodontic treatment of impacted canines without causing significant morbidity to the adjacent teeth and periodontium is a challenge. The bilaterally impacted maxillary canines were successfully aligned and…

[Combined therapy of chronic catarrhal gingivitis using low intensity laser light].

Periodontal diseases, one of which is chronic catarrhal gingivitis (CCG), represent a medicosocial problem. Standard therapy often fails to bring about stable results. Addition of low-energy laser exposure at wavelengths of 0.63 micron and 0.85-0.89 micron to therapeutic complexes helps attain stable results, which is confirmed by clinical, functional, immunological, and x-ray data and densitometry. Laser therapy exerts a multifactorial pathogenetic effect, eliminating inflammation in periodontal tissue, normalizing microcirculation in the periodontium, improving mineral saturation of osseous …

Periodontal diseases in HIV-infected patients.

Periodontal diseases may be the first clinical sign of human immunodeficiency virus (HIV)-infection. Since the immunosuppression and subsequent susceptibility may alter the responses of the oral tissues as well as the microflora, both periodontal treatment and result of therapy may be modified. The periodontal diseases in HIV-seropositive patients include common as well as less conventional forms of gingivitis and periodontitis, and bacterial, mycotic and viral infections are seen. Neoplasias may also involve the periodontium; most common are Kaposi's sarcoma and non-Hodgkin's lymphoma. Recent…

Hyaluronic acid in Dentoalveolar regeneration: Biological rationale and clinical applications.

BACKGROUND: Hyaluronic acid (HA) is found in different locations in the periodontium, including mineralized tissues (i.e., cementum and alveolar bone) and non-mineralized tissues (i.e., gingiva and periodontal ligament). In addition, it seems to play an essential part in regulating the underlying mechanisms involved in tissue inflammatory reactions and wound healing. HA has the potential to regulate periodontal tissue regeneration and treat periodontal disease. AIM: The current review of the literature was conducted to assess how HA plays its part in periodontal therapy and examine the contem…

Cyclosporine A and amlodipine induced gingival overgrowth in a kidney transplant recipient: case presentation with literature review.

Drug-induced gingival overgrowth is a condition caused by side effects of treatment with one of three types of drugs: phenytoin (used in epilepsy treatment), cyclosporine A (used in transplantology after allogenic organ transplants) and calcium channel blockers (used in the treatment of hypertension). Gingival overgrowth leads to inflammation within the gums and periodontium and can amplify the existing periodontal disease leading to tooth loss. Patients who have undergone kidney transplant are given immunosuppressants to prevent transplant rejection and mostly it is accompanied with calcium c…

Shield the socket: Procedure, case report and classification.

The extraction of a tooth leads to a cascade of events which results in resorption of the alveolar bone around the socket. The buccal bone loss that occurs postextraction leads to vertical and horizontal bone loss. It requires complex hard and soft-tissue reconstruction to achieve esthetically pleasing results in such cases. In the socket-shield technique (SST) the root is bisected, and the buccal two-third of the root is preserved in the socket so that the periodontium along with the bundle bone and the buccal bone remains intact. A classification of SST technique is proposed depending on the…

A novel treatment for propagated crown fractures.

INTRODUCTION: A major complication after endodontic treatment is persistent pain on biting or chewing. Although unsuccessful endodontic treatment can account for such pain, pain to chewing and biting can also be a result of an unsupported or propagated crown fracture. Crown fractures that extend apically result in deterioration of the localized periodontium and ultimate loss of the tooth. To date, no predictable treatment for apically extending crown fractures exists. METHODS: Three cases of nonsurgical excision of a propagated crown fracture and the repair of the subsequent iatrogenic perfor…

Oral myiasis caused by Cochliomyia hominivorax treated nonsurgically with nitrofurazone: report of 2 cases.

Accidental oral myiasis are caused by ingestion of fly's eggs or direct oviposition over a compromised area of the host. Here the authors report 2 cases of accidental oral myiasis caused by direct oviposition over an edentulous area in the first patient and in the periodontium in the second patient. Both cases were treated successfully with topical applications of nitrofurazone during a 3-day period. No surgical procedures were required. Flushing the wound with nitrofurazone caused the maggots to endure a liquid-filled anaerobic environment. Healing was uneventful in the 2 cases presented and …
