The FDA's risk/benefit calculus in the approvals of Qsymia and Belviq: treating an obesity epidemic while avoiding another fen-phen.

As obesity rates continue to rise in the United States, both physicians and patients have demanded more safe and effective drug treatment options. However, following the fen-phen/Redux and sibutramine failures, the FDA has been hesitant to approve any anti-obesity drugs, despite the magnitude of the epidemic. Some have argued that these public embarrassments have led the FDA to overestimate the risks and underestimate the benefits when deciding whether to approve new anti-obesity drugs. On June 27, 2012, the FDA approved Belviq for chronic weight management, making it the first anti-obesity dr

New developments in the pharmacologic treatment of obesity.

Obesity, defined as a body mass index (BMI, kg/m(2) ) >30, is a significant public health problem. It's estimated that 50 percent of the U.S. population will be classified as obese by the year 2030. Due to associated health complications and rising health care costs related to obesity, new treatment options are being explored. For people who need additional treatment beyond lifestyle modification, new pharmacologic options have been developed that may assist in reducing BMI. Health care providers and patients should consider each person's individual health history and consider both the potenti

[The obesity epidemics--do diet pills have a place in the treatment?].

BACKGROUND: Many experts consider obesity a chronic disease that may require long-term therapy. A loss of 5-15% of body weight is associated with improvements in cardiovascular risk factors and morbidity. However, most studies show that the majority of patients who lose weight relapse. Patients may be unable to maintain a low energy intake when confronted with an almost limitless supply of food. Moreover, a number of physiological mechanisms favour a set point for body weight, that may be altered with anti-obesity drugs. MATERIAL AND RESULTS: In the current paper we describe actions and effec

Valve replacement for appetite suppressant-induced valvular heart disease.

Valvular heart disease associated with the use of appetite-suppressant medication is a recently described clinical entity. Although the mechanism of valvular injury remains elusive pathologically, the valvular abnormalities resemble those observed in carcinoid syndrome. The incidence of clinically evident valvular heart disease is low with short-term (less than 3 months) exposure to appetite-suppressant drugs. Prolonged exposure to higher doses in addition to combination drug therapy confers an excess risk for valvular pathologic changes. We report the case of a patient with severe mitral regu

Chlorphentermine-induced biochemical alterations in mitochondrial membranes.

The effects of chlorphentermine (CP) on bioenergetics and monoamine oxidase activity in rat liver mitochondria were examined. Oxidation rates of isocitrate, glutamate and succinate were investigated in the presence of CP (0.5-5.0 mM). In the presence of low concentrations (0.5-2.0 mM) CP decreased respiratory control ratio and ADP/O ratio, and stimulated state 4 respiration. State 3 respiration and uncoupled state were unaffected. The same pattern of changes was found after in vivo CP treatment for 1 or 2 weeks (30 mg/kg i.p., 5 days/week). CP also increased respiration in state 3 that had bee…

Abused drug-induced intracranial self-stimulation is correlated with the alteration of dopamine transporter availability in the medial prefrontal cortex and nucleus accumbens of mice.

Intracranial self-stimulation (ICSS) of the medial forebrain bundle in mice is an experimental model use to assess the relative potential of reward-seeking behaviors. Here, we used the ICSS model to evaluate the abuse potential of 18 abused drugs: 3-Fluoroethamphetamine (3-FEA); methylphenidate; cocaine; dextroamphetamine; alpha-Pyrrolidinobutyrophenone (α-PBT); 4'-Fluoro-4-methylaminorex (4-FPO); methamphetamine; larocaine; phentermine; paramethoxymethamphetamine (PMMA); phendimetrazine; N-(1-adamantyl)-1-pentyl-1H-indazole-3-carboxamide (AKB-48); Naphthalen-1-yl-(4-pentyloxynaphthalen-1-yl)m…

Pharmacotherapy for obesity.

BACKGROUND: Obesity is a serious, chronic, relapsing disease of energy regulation, with strong genetic and early-life environmental determinants. Pharmacotherapy can be a useful adjunct to lifestyle intervention in effecting and maintaining clinically meaningful weight loss. OBJECTIVE: The aim of this article is to discuss the role of pharmacotherapy in obesity management. The efficacy, side effects and contraindications of available weight-loss medications are reviewed. DISCUSSION: Long-term pharmacotherapy options, which can be effective in providing moderate weight loss, are available to

Medical management of obesity.

The prevalence of obesity and obesity-related disorders, such as insulin resistance and hyperinsulinaemia, which form the basis for the Metabolic Syndrome, is still increasing and reaching epidemic proportions. Excess weight, particularly abdominal obesity, elevates multiple cardiovascular and metabolic risk factors, including Type 2 diabetes, hypertension, dyslipidaemia and cardiovascular disease. Therefore, appropriate management of obesity and prevention of obesity-related diseases with high mortality and morbidity is of great importance for public health. Obesity management goals should en

Pharmacologic therapies for obesity.

This article examines the transitions in pharmacological therapy for obesity. It reviews the current options approved by the Food and Drug Administration and several drugs approved for other indications that can be used to treat obesity as well. Because weight regulation is complex and redundant systems protect against perceived starvation, optimal treatment of obesity in individual patients will likely require different combinations of behavioral, nutritional, pharmacologic, endoscopic, and surgical therapies.

Three cases of comprehensive dietary therapy and pharmacotherapy of patients with complex obesity-related diseases.

The effect of weight loss with anorectic medications on sleep apnea, non-insulin-dependent diabetes, and steatohepatitis is illustrated in three cases from practice in a clinical nutrition setting. Prevention of obesity, a chronic disorder, is preferable, but when obesity becomes a major obstacle in the care of patients with respiratory, cardiovascular, and metabolic disorders and osteoarthritis, an intense course of weight reduction using anorectic medications under medical and dietetic guidance is essential for patients' survival and reduction of medical cost.

Chlorphentermine suppresses the phosphatidylinositol pathway in concanavalin A-activated mouse splenic lymphocytes.

We have previously demonstrated that the chlorphentermine (CP)1-induced impairment in lymphocyte blastogenesis involves drug-induced inhibition of an event which occurs very early during lymphocyte activation. An early event, which is associated with mitogen-induced lymphocyte activation, involves the hydrolysis of phosphatidylinositol by phospholipase C to yield inositol phosphates and diacylglycerol as products. Inositol phosphates and diacylglycerol then function as mediators of a trans-membrane signal for the continuation of the cellular response. It was the purpose of the present study to…
