Shaving Versus Resection on Recurrent Laryngeal Nerve in Thyroid Cancer Surgeries: A Systematic Review and Meta-Analysis.

BACKGROUND: Management of recurrent laryngeal nerve (RLN) invasion by thyroid cancer remains controversial between shaving the tumor and resecting it. We compared oncological and functional outcomes of the two techniques. METHODS: Following PRISMA guidelines, three reviewers searched seven databases for studies from 1995 to 2024 comparing shaving and resection of the RLN. Outcomes explored were overall survival, recurrence, preservation of vocal cord function, maximum phonation time, and other treatment-related morbidities if provided. RESULTS: A total of 13 retrospective cohorts (532 patien…

Novel voice prosthesis after total laryngectomy with laryngoplasty reconstruction.

BACKGROUND: Although many patients achieve serviceable speech after total laryngectomy (TL), others are limited by un-naturally low pitch. We describe a cadaveric study to provide proof of concept for a novel voice prosthesis after TL with free tissue laryngoplasty. METHODS: Devices were implanted into fresh frozen cadavers after TL and free tissue laryngoplasty. Phonation pressures were measured using a custom Blom-Singer Manometer (InHealth Technologies, Carpinteria, CA) and acoustic files were analyzed using Praat, a speech analysis software. RESULTS: Two fresh frozen cadavers were implan…

Computational modeling of phonatory dynamics in a tubular three-dimensional model of the human larynx.

Simulation of the phonatory flow-structure interaction has been conducted in a three-dimensional, tubular shaped laryngeal model that has been designed with a high level of realism with respect to the human laryngeal anatomy. A non-linear spring-based contact force model is also implemented for the purpose of representing contact in more general conditions, especially those associated with three-dimensional modeling of phonation in the presence of vocal fold pathologies. The model is used to study the effects of a moderate (20%) vocal-fold tension imbalance on the phonatory dynamics. The chara…

Estimation of impact stress using an aeroelastic model of voice production.

The maximum impact stress at the contact of the vocal folds achieved during the oscillation cycle was estimated in phonation using an aeroelastic model of voice production. Relations of impact stress to the lung pressure, fundamental frequency of self-oscillations, prephonatory glottal width, sound pressure level generated at the end of the glottis and vibration amplitude of the vocal folds were studied. Using the fundamental frequency, prephonatory glottal width, lung pressure and airflow rate values found in normal speech, maximum impact stress values of 2-3 kPa were obtained. The results fa…

Videolaryngostroboscopy following vertical partial laryngectomy.

Phonation after partial laryngeal ablative surgery has not often been examined. Videolaryngostroboscopic recordings made after vertical partial laryngectomy (VPL) were retrospectively reviewed and correlated with patient historical and operative factors. Among VPL patients (n = 42), the most common site of vibration during phonation was the contralateral false vocal fold (17/42 patients or 40.5%), followed by the contralateral arytenoid mucosa (10/42 or 23.8%) and the contralateral true vocal fold (8/42 patients or 19.0%). There was no overall difference in vocal quality judgment with respect …

Quantitative measurement of mucosal wave by high-speed photography in excised larynges.

The movement characteristics of mucosal waves of the vocal fold are important components in normal phonation. Quantitative studies of the mucosal wave have used stroboscopic techniques from a supraglottic view. The current study measured displacement of mucosal epithelium during experimental phonation by using high-speed photography from an infraglottic view. Effects of thyroarytenoid contraction, increased mean airflow rate, and variation of vocal fold length were examined in canine larynges. Top and bottom vocal fold "lip" amplitude, fundamental frequency, and phase difference were the depen…

Phonatory assessments in patients with slowly progressive neuromuscular disorders: a new tool for evaluating inspiratory muscle dysfunction.

INTRODUCTION: Neuromuscular diseases (NMD) are a group of pathologies that affect the functionality of skeletal muscles and one of the main causes of morbidity and mortality in these patients is respiratory failure secondary to inspiratory muscle dysfunction. The diagnosis of inspiratory muscle dysfunction requires a combination of vital capacity (VC), maximum inspiratory pressure (MIP) and sniff nasal inspiratory pressure (SNIP). To assess whether it is possible using 3 techniques of peak phonation time (MPT) measurement to define lung function (VC < 50%ref) and inspiratory muscle dysfunction…

Aerodynamic Threshold Measures for Reflecting Glottal Closure in Voice Disorders.

PURPOSE: Previous work suggested that phonation threshold pressure (PTP), phonation threshold flow (PTF), and phonation threshold power (PTW) could be effective aerodynamic measures for quantifying glottal incompetence. This study examined how these measures could reflect varying extent of incomplete glottal closure in individuals with voice disorders. METHOD: Thirty individuals formally diagnosed with glottal incompetence, including 10 with hypofunctional disorders (hypo group) and 20 with hyperfunctional disorders (hyper group), and 30 individuals with normal voice (control group) participa…
