Immune deficiencies in chronic intestinal pseudo-obstruction.

AIM: Chronic intestinal pseudo-obstruction has been associated with urinary disorders, myopathy, and ophthalmoplegia in adults and cholelithiasis in children. We observed a high percentage of total-parenteral-nutrition-dependent patients with pseudo-obstruction and recurrent infections requiring gammaglobulin infusions. METHODS: All records for 23 children with chronic intestinal pseudo-obstruction (10 females and 13 males, mean age 9.8 y +/- 4.9 y, range 4-24 y) referred for a nutritional evaluation from 1992 to 1995 were reviewed. Chronic intestinal pseudo-obstruction was diagnosed by clini

Congenital mandibular hypoplasia: analysis and classification.

Mandibular hypoplasia is a frequently encountered craniofacial difference and can be classified into three groups: congenital, developmental, and acquired. The focus of this article is on the congenital group, the majority of which is associated with syndromes. There have been numerous publications on patients with syndromic congenital mandibular hypoplasia; however, there has been no investigation and differentiation of the "nonsyndromic" patients. The purpose of this study was to analyze this subgroup of patients with nonsyndromic congenital mandibular hypoplasia to determine incidence, clin

Anesthetic implications of infants with mandibular hypoplasia treated with mandibular distraction osteogenesis.

OBJECTIVES: To document the incidence of difficult intubation following mandibular distraction osteogenesis (MDO) in children with severe mandibular hypoplasia. BACKGROUND: Syndromes associated with significant mandibular hypoplasia, especially Pierre Robin sequence, provide a challenge in airway management both in and out of the operating room. Mandibular advancement using mandibular distraction osteogenesis devices has been used in infants in an attempt to reduce the incidence of acute life-threatening airway obstruction. Whether MDO also reduces the incidence of difficult intubation has no

The craniofacial surgeon as amateur geneticist.

Craniofacial surgeons, by nature and training, focus on how to correct anomalies rather than on why they occur. Surgeons often leave diagnosis and etiopathogenic speculation to geneticists. Craniofacial surgeons should cross over the specialty line and learn to think like geneticists. This article reviews definitions of basic words in the genetic language and emphasizes the three diagnostic levels, phenotypic, pathogenic, and genetic, for the principal categories of craniofacial anomalies. Whenever possible, examples are given to illustrate how genetic knowledge can influence surgical strategy

Characterizing Mandibular Morphology in Robin Sequence-A 3D Statistical Shape Analysis.

BACKGROUND: Robin sequence (RS) is a congenital condition and constitutes the triad of micrognathia, glossoptosis, and upper airway obstruction. While micrognathia is a cardinal feature of RS, its assessment is largely subjective. The aim of the present study is to describe 3D mandibular morphology in patients with RS and age-related mandibular shape variation compared with an age-matched control group. METHODS: 3D reconstructions of the mandible were obtained from CT-imaging of children with isolated (iRS) and nonisolated RS (niRS). Principal Component Analysis was used to describe variation

True Incidence of Marginal Mandibular Nerve Palsy following Neonatal Mandibular Distraction Osteogenesis.

BACKGROUND: In children with Pierre Robin sequence (PRS), mandibular distraction osteogenesis (MDO) is routinely performed to alleviate airway obstruction; however, it involves risk of injury to the marginal mandibular nerve (MMN). The authors hypothesize that MMN palsy incidence following MDO, reported at 1% to 15%, is underestimated. This study investigates the true incidence of MMN palsy after MDO to better guide follow-up care and improve treatment of this complication. METHODS: A retrospective review of PRS patients who underwent MDO at a single, tertiary pediatric hospital between Septe

Abnormal liver function tests and improved survival in a child with splice mutation TARP syndrome.

TARP (talipes equinovarus, atrial septal defect (ASD), Robin sequence, persistent left superior vena cava) syndrome is a rare X-linked disorder affecting the RBM10 gene. It was previously viewed as universally fatal in the early neonatal period, however, recent cases have shown patients surviving beyond this stage. We present a male toddler diagnosed with TARP syndrome due to a a previously unreported splicing mutation c.2295+1G>A in the RBM10 gene. At birth, he had an ASD and Robin sequence, two of the eponymous features, as well as other associated phenotypic features. During infancy, he had

Working Towards an Appropriate Use of Ibuprofen in Children: An Evidence-Based Appraisal.

Ibuprofen is the most widely used non-steroidal anti-inflammatory drug (NSAID) for the treatment of inflammation, mild-to-moderate pain and fever in children, and is the only NSAID approved for use in children aged ≥3 months. Its efficacy and safety profile have led to its increasing use in paediatric care, even without medical prescription. However, an increase of suspected adverse reactions to ibuprofen has been noted in concomitance with the raised, often medically unsupervised, consumption of the drug. The purpose of this work was a critical review of the paediatric literature over the las

Surgical versus nonsurgical interventions to relieve upper airway obstruction in children with Pierre Robin sequence.

BACKGROUND: Newborns with Pierre Robin sequence (PRS) often experience chronic intermittent hypoxemia/hypoventilation associated with airway obstruction. The heterogeneity of the severity of upper airway obstruction makes management a challenge; the optimal intervention in individual cases is not clear. OBJECTIVE: To investigate the prevalence of surgical/nonsurgical interventions for PRS at two children's hospitals. Patient characteristics and outcomes were examined. METHODS: The present retrospective chart review identified 139 patients with PRS born between 2000 and 2010. Demographic info

A compound heterozygote SLC26A2 mutation resulting in robin sequence, mild limbs shortness, accelerated carpal ossification, and multiple epiphysial dysplasia in two Brazilian sisters. A new intermediate phenotype between diastrophic dysplasia and recessive multiple epiphyseal dysplasia.

Mutations in solute carrier family 26 (sulfate transporter), member 2 (SLC26A2) gene result in a spectrum of autosomal recessive chondrodysplasias that range from the mildest recessive form of multiple epiphysial dysplasia (rMED) through the most common diastrophic dysplasia (DTD) to lethal atelosteogenesis type II and achondrogenesis IB. The clinical variability has been ascribed to quantitative effect of mutations of the sulfate transporter activity. Here we describe two Brazilian sisters, born to healthy and non consanguineous parents, with Robin sequence, mild shortening of upper and lower

Diagnostic difficulties in cases of sudden death in infants with mandibular hypoplasia.

Infants with mandibular hypoplasia are at risk of sudden death from cardiorespiratory arrest secondary to upper airway obstruction. To evaluate diagnostic difficulties that may occur at autopsy in such infants, the autopsy files at the Adelaide Children's Hospital (ACH) for 36 years, 1959 to 1994, were reviewed. Eight cases were identified (age range, 2 days to 10 months; mean age, 2.2 months; male/female ratio, 5:3). In all cases, death was considered most likely due to airway obstruction related to mandibular hypoplasia or its treatment. Although death occurred in the hospital in five cases,

Lingual Abscess in an Adult Patient With Pierre Robin Sequence: A Case Report.

A lingual abscess is a rare but serious infection within the tongue parenchyma, posing significant risks due to potential airway obstruction. Despite advancements in oral hygiene and antibiotics, timely diagnosis and treatment are critical to prevent severe complications. In this case, we report a 29-year-old male with Pierre Robin sequence (PRS) who presented with a four-day history of severe tongue pain, swelling, decreased appetite, and fever, without any reported trauma. Examination revealed left-sided tongue swelling, poor oral hygiene, and notable Mallampati III classification. A neck CT

Modified mandible traction with wires to treat neonatal Pierre Robin sequence: A case report.

BACKGROUND: Pierre Robin sequence (PRS) is a congenital craniofacial deformity characterized by micrognathia, glossoptosis and airway obstruction. Some affected neonates are born with severe life-threatening upper airway obstruction that requires surgery. If without timely treatment, it is possible to cause not only organ damage and developmental abnormalities but also early newborn mortality. CASE PRESENTATION: In this report, a 51-hours-old neonate was diagnosed with PRS, who had severe upper airway obstruction and required surgery. We performed the modified mandible traction with wires at

Long-Term Occlusal Outcomes for Conservative Management in Patients With Robin Sequence.

INTRODUCTION: Robin sequence (RS), the triad of micrognathia, glossoptosis, and airway obstruction, is treated with conservative management in milder cases with the expectation that mandibular growth will normalize over time. Mandibular distraction osteogenesis (MDO) is a surgical intervention utilized in more severe cases. We aim to identify the long-term outcomes in conservatively managed RS patients to identify differences in mandibular growth and structural airway pathology. METHODS: A retrospective review of patients with RS at a tertiary pediatric hospital was conducted from 2002 to 202

Automated Computed Tomography Segmentation of the Pharyngeal Airway and Palate to Accelerate Tübingen Palatal Plate Fabrication in Pierre Robin Sequence.

Infants with Pierre Robin sequence frequently develop upper airway obstruction due to micrognathia and glossoptosis. The Tübingen palatal plate repositions the tongue base anteriorly to improve airway patency; however, conventional fabrication requires serial intraoral impressions and repeated nasoendoscopy, which prolongs airway compromise. Computed tomography (CT) enables single-session virtual spur design, yet manual pharyngeal airway and hard palate segmentation is labor-intensive, delaying treatment. The authors evaluated convolutional neural network-based automated segmentation to accele

Focused Investigation of Facial Nerve Dysfunction After Mandibular Distraction Osteogenesis for Robin Sequence.

Facial nerve dysfunction (FND) is a well-recognized but poorly documented complication of mandibular distraction osteogenesis (MDO) for Robin sequence (RS). This study aims to document the authors' experiences with FND and identify risk factors associated with this adverse event. A retrospective review of a prospectively gathered database was performed to identify patients with RS who underwent MDO at the authors' institution from March 2016 to June 2023. The authors included all infants with at least 3 months of follow-up after device removal. Data collected included patient demographics, inc
