Pneumococcal vaccination coverages among low-, intermediate-, and high-risk adults in Catalonia.

There is scarce data about pneumococcal vaccination coverages among adults in recent years. We investigated current pneumococcal vaccination coverages in Catalonia, Spain, with a cross-sectional population-based study including 2,033,465 individuals aged 50 y or older assigned to the Catalonian Health Institute at 01/01/2015 (date of survey). A previously validated institutional research clinical Database was used to classify study subjects by their vaccination status for both 23-valent pneumococcal polysaccharide vaccine (PPSV23) and 13-valent pneumococcal conjugate vaccine (PCV13), to identi

Antibody response to pneumococcal and influenza vaccination in patients with rheumatoid arthritis receiving abatacept.

BACKGROUND: Patients with rheumatoid arthritis (RA), including those treated with biologics, are at increased risk of some vaccine-preventable infections. We evaluated the antibody response to standard 23-valent pneumococcal polysaccharide vaccine (PPSV23) and the 2011-2012 trivalent seasonal influenza vaccine in adults with RA receiving subcutaneous (SC) abatacept and background disease-modifying anti-rheumatic drugs (DMARDs). METHODS: Two multicenter, open-label sub-studies enrolled patients from the ACQUIRE (pneumococcal and influenza) and ATTUNE (pneumococcal) studies at any point during

Safety, tolerability, and immunogenicity of 15-valent pneumococcal conjugate vaccine in healthy adults.

BACKGROUND: Pneumococcal disease remains an important health priority despite successful implementation of pneumococcal conjugate vaccines (PCVs) in infant immunization programs, mainly due to the emergence of diseases caused by serotypes not included in licensed PCVs. A 15-valent pneumococcal conjugate vaccine (PCV-15) containing the 7 serotypes (4, 6B, 9V, 14, 18C, 19F, and 23F) included in licensed PCV-7 available at study initiation plus 8 additional serotypes (1, 3, 5, 6A, 7F, 19A, 22F, 33F) was developed and evaluated in healthy adults 18-45 years of age. METHODS: Sixty subjects receive

Nearly complete elimination of the 7-valent pneumococcal conjugate vaccine serotypes in Tennessee.

BACKGROUND: Invasive pneumococcal disease (IPD) rates decreased after 7-valent pneumococcal conjugate vaccine (PCV) introduction in 2000. We assessed whether previously described decreases were sustained. METHODS: Active laboratory-based surveillance identified IPD cases in 5 Tennessee Counties. For each case, clinical data were collected, and antibiotic susceptibility testing and serotyping were performed. Penicillin resistance was defined as intermediate- or high-level resistance to penicillin. Serotypes were classified as PCV7, PCV13 (6 additional serotypes not in PCV7), pneumococcal polys

Limb swelling in a pediatric sickle cell patient after revaccination with pneumococcal vaccine.

CASE (DESCRIPTION): This report describes an incident of limb swelling after revaccination with the 23-valent pneumococcal polysaccharide vaccine (PPSV23). A 5-year old female with sickle cell disease experienced severe arm swelling with edema to the extent that it was difficult to put a shirt over the arm. The day prior, she received PPSV23 concomitantly with the meningococcal vaccine. She had received the PPSV23 vaccine 2.5 years prior to the event. The patient was managed with analgesics, antiemetics, and antibiotic prophylaxis and the arm swelling resolved 24 h later. A decreased length of

Pre-vaccination nasopharyngeal pneumococcal carriage in a Nigerian population: epidemiology and population biology.

BACKGROUND: Introduction of pneumococcal vaccines in Nigeria is a priority as part of the Accelerated Vaccine Introduction Initiative (AVI) of the Global Alliance for Vaccines and Immunisation (GAVI). However, country data on the burden of pneumococcal disease (IPD) is limited and coverage by available conjugate vaccines is unknown. This study was carried out to describe the pre vaccination epidemiology and population biology of pneumococcal carriage in Nigeria. METHODS: This was a cross sectional survey. Nasopharyngeal swabs (NPS) were obtained from a population sample in 14 contiguous peri-…

Impact of universal pneumococcal vaccination on hospitalizations for pneumonia and meningitis in children in Montevideo, Uruguay.

BACKGROUND: In March 2008, Uruguay included PCV7 into the routine vaccination program, in a 2 + 1 schedule for children <2 years of age. Catch-up immunization was offered to children born in 2007. Greater than 95% of children received their first and second doses. The aim of this study was to assess the effect of this strategy. METHODS: Annual hospitalization rates (per 10,000 discharges) for community-acquired pneumonia (CAP) in children <14 years of age and pneumococcal meningitis are described prior to PCV7 vaccination (2005-2007), during the year of implementation (2008) and following vac

The development of 13-valent pneumococcal conjugate vaccine and its possible use in adults.

INTRODUCTION: Worldwide, Streptococcus pneumoniae causes significant morbidity and mortality. The 23-valent pneumococcal polysaccharide vaccine (PPSV23) has been recommended for use in persons aged 65 years and over and in adults with certain chronic medical conditions. Pneumococcal conjugate vaccines (PCVs) have been developed for use in infants and children aged less than 5 years, and are being studied for use in adult populations. AREAS COVERED: The different types of pneumococcal vaccines are discussed. Studies comparing PPSV23 and PCVs, as well as the results of the widespread use of 7-v…

Risk factors for invasive pneumococcal disease in children in the era of conjugate vaccine use.

OBJECTIVE: We conducted a case-control study to evaluate risk factors for invasive pneumococcal disease (IPD) among children who were aged 3 to 59 months in the era of pneumococcal conjugate vaccine (PCV7). METHODS: IPD cases were identified through routine surveillance during 2001-2004. We matched a median of 3 control subjects to each case patient by age and zip code. We calculated odds ratios for potential risk factors for vaccine-type and non-vaccine-type IPD by using multivariable conditional logistic regression. RESULTS: We enrolled 782 case patients (45% vaccine-type IPD) and 2512 mat

Insidious presentation of pediatric pneumococcal meningitis: alive and well in the post vaccine era.

Routine childhood vaccination against H influenza type b and S pneumoniae has dramatically reduced the incidence of pediatric bacterial meningitis. Because of the decreased incidence of this disease, individual emergency physicians will have limited opportunity to experience the diverse clinical manifestations of this disease. Insidious presentations of this diagnosis still occur but would now be considered rare events for emergency physicians. It is imperative to recognize that young infants with bacterial meningitis can present without fever or irritability and manifest signs and symptoms co

Outcomes of febrile children without localising signs after pneumococcal conjugate vaccine.

BACKGROUND: Evaluation of children with fever without localising signs (FWLS) has barely changed in the USA since 1993 despite reduced invasive disease after the introduction of Haemophilus influenzae type b conjugate vaccine and conjugate pneumococcal vaccine (PCV7). PCV7 is now recommended in the UK for children under 2 years of age, and new NICE guidelines have been issued for managing feverish children in the UK in anticipation of PCV7's efficacy. We compared rates of bacterial infections in children aged 3-36 months with FWLS in the pre- and post-PCV7 eras to define current trends and eva

Changing interest among physicians toward pneumococcal vaccination throughout the twentieth century.

Interest in using pneumococcal vaccination among physicians has come full circle since the early twentieth century. Interest first arose during an epidemic among South African gold miners when a potential therapeutic agent, ethylhydrocupreine, caused significant toxicities. When a safe and effective vaccine became available in 1946, interest in using it paradoxically waned as its introduction coincided with the advent of penicillin. Physicians believed that this effective new antibiotic would change pneumonia from a dreaded infection to a casual, infrequent occurrence. The manufacturer was una

Efficacy of polysaccharide pneumococcal vaccine in adults in more developed countries: the state of the evidence.

We review studies on the efficacy against disease caused by Streptococcus pneumoniae of the 23-valent polysaccharide pneumococcal vaccine in adult populations in the more developed countries. Meta-analyses of primary vaccine trials have attempted to reduce uncertainty from lack of power. Vaccine efficacy calculated from studies in South African gold-miners and in Papua New Guinea, with high attack rates and differing serotype patterns, cannot automatically be applied to more developed countries. Meta-analyses will overestimate a protective effect if this clinical heterogeneity is ignored. Meta

Decline in invasive pneumococcal disease after the introduction of protein-polysaccharide conjugate vaccine.

BACKGROUND: In early 2000, a protein-polysaccharide conjugate vaccine targeting seven pneumococcal serotypes was licensed in the United States for use in young children. METHODS: We examined population-based data from the Active Bacterial Core Surveillance of the Centers for Disease Control and Prevention to evaluate changes in the burden of invasive disease, defined by isolation of Streptococcus pneumoniae from a normally sterile site. Serotyping and susceptibility testing of isolates were performed. We assessed trends using data from seven geographic areas with continuous participation from

Activation of cell-mediated immunity following immunization with pneumococcal conjugate or polysaccharide vaccine.

The immunogenicity of pneumococcal polysaccharide (PS) vaccines can be improved by conjugating PS to a polypeptide carrier that alters the immune response from T-cell independent to T-cell dependent. In order to study the influence of PS or protein antigens as inducers of cell-mediated responses, 30 adults were immunized with a 23-valent pneumococcal PS vaccine (PS-group) or an 11-valent, tetanus and diphtheria mixed carrier conjugate vaccine with (adjuvant group) or without aluminium adjuvant (nonadjuvant group). Cell-mediated responses were analyzed on days 0, 14 and 28 after vaccination by

Vaccination update. Hib, hepatitis, polio, varicella, influenza, pneumococcal and meningococcal disease.

Immunization of infants and children is the most effective strategy for decreasing the incidence of some infectious diseases. Most invasive disease due to Haemophilus influenzae type b occurs before age 5 years, and routine vaccination of infants for hepatitis B is currently recommended because selective immunization of high-risk persons has not been feasible. Decades of use of poliovirus vaccine has effectively eliminated cases of wild-virus infection, although some vaccine-related cases still occur. The newly approved varicella vaccine appears to be a cost-effective way to decrease infection
