Cauda equina syndrome and dural ectasia: rare manifestations in chronic ankylosing spondylitis.

Cauda equina syndrome (CES) is a rare manifestation in patients with long-standing ankylosing spondylitis (AS). We report a 57-year-old male patient with a 30-year history of AS who developed CES in the past 4 years. The CT and MRI examinations showed unique appearances of dural ectasia, multiple dorsal dural diverticula, erosion of the vertebral posterior elements, tethering of the conus medullaris to the dorsal aspect of the spinal canal and adhesion of the nerve roots of the cauda equina to the wall of the dural sac. A large dural defect was found at surgery. De-adhesion of the tethered con

[Diabetic amyotrophy: favorable outcome following corticosteroid therapy?].

Diabetic amyotrophy or lombosacral radiculoplexus neuropathy is a rare complication associated with early-stage diabetes. Thigh pain, quadricipital amyotrophy, proximal weakness of lower limbs and weight loss are the main symptoms of the disease. As neurological damage is related to inflammatory microvasculitis, corticosteroid therapy may be considered as the first line therapy. We report a 54-year-old patient with type 2 diabetes affected with severe diabetic amyotrophy. Following intravenous corticosteroid therapy, the patient reported a rapid pain relief and gained muscle strength.

Cauda equina syndrome: a review of the current clinical and medico-legal position.

Cauda equina syndrome (CES) is a rare condition with a disproportionately high medico-legal profile. It occurs most frequently following a large central lumbar disc herniation, prolapse or sequestration. Review of the literature indicates that around 50-70% of patients have urinary retention (CES-R) on presentation with 30-50% having an incomplete syndrome (CES-I). The latter group, especially if the history is less than a few days, usually requires emergency MRI to confirm the diagnosis followed by prompt decompression by a suitably experienced surgeon. Every effort should be made to avoid CE

An unusual case of penetrating injury to the spine resulting in cauda equina syndrome: case presentation and a review of the literature.

STUDY DESIGN: Case study. OBJECTIVE: We present an unusual case of cauda equina syndrome due to a penetrating injury in which the brake caliper of a motorcycle lodged in the lumbar canal of the operator of the vehicle after a road accident and provide a review of the literature on penetrating injury to the spine. SUMMARY OF BACKGROUND DATA: While the large majority of penetrating spinal injuries are due to gunshot wounds, penetrating injury to the spine causing cauda equina syndrome is rare. METHODS: We report the case of a 42-year-old man involved in a motorcycle accident in which the brak

Return of normal urological and neurological function after revision surgery for spondyloptosis. Case report.

The authors report on the return of neurological and urological function in an adolescent after revision surgery for spondyloptosis 5 years after the index procedure for high-grade spondylolisthesis. This 16-year-old girl with Grade 3 spondylolisthesis was initially treated with a posterolateral reduction and fusion. Following surgery, cauda equina syndrome symptoms developed and did not resolve despite subsequent surgical decompression. Five years later, because of worsening radicular pain, an inability to walk for significant distances, and no resolution of persistent bladder dysfunction, th

Dumbbell-shaped Hodgkin's disease with cauda equina compression mimicking a herniated inter-vertebral disc, a case report.

Hodgkin's disease may involve the spine as a setting of the advanced disease. An initial manifestation of Hodgkin's disease in spine is extremely rare and the major involved sites usually are the thoracic or cervical spine. The mechanisms of pathogenesis for the formation of an epidural mass during Hodgkin's disease are hematogenous dissemination from nodal sites or local infiltration of lymphomatous tissue. We document here a case of a 16 year-old boy who suffered from incomplete voiding due to dumbbell-shaped retroperitoneal Hodgkin's disease with cauda equina compression. He was successfull

[Lumbago and radicular complaints: not always a herniated disk or degenerative stenosis of the spinal canal. A differential diagnosis of infrequent diseases].

Low back ache and pain in the legs are not always due to disc displacement and lumbar spinal degenerative changes. Some infrequent, but really not very rare diseases are presented in order to avoid mistakes which can have serious consequences for the patients. Degenerative changes of the lumbar spine are very common, not only in aged people. A superficial examination of the patient with back ache and/or pain in the legs can lead to a fallacious tie-up between such lesions and disturbances and complaints which are not connected to them.

Lumbar decompression surgery for cauda equina syndrome - comparison of complication rates between daytime and overnight operating.

PURPOSE: To investigate the incidence of complications from lumbar decompression ± discectomy surgery for cauda equina syndrome (CES), assessing whether time of day is associated with a change in the incidence of complications. METHODS: Electronic clinical and operative notes for all lumbar decompression operations undertaken at our institution for CES over a 2-year time period were retrospectively reviewed. "Overnight" surgery was defined as any surgery occurring between 18:00 and 08:00 on any day. Clinicopathological characteristics, surgical technique, and peri/post-operative complications

The lived experience of Cauda Equina Syndrome: a qualitative analysis.

STUDY DESIGN: An exploratory qualitative analysis, using semi-structured interviews to investigate the lived experience of Cauda Equina Syndrome (CES). OBJECTIVES: To address the paucity of psychological research into CES and explore patient experiences of living with the injury. SETTING: The study was conducted in the United Kingdom. Recruitment was via two National Health Service spinal services in the South East of England and an online CES charity. METHODS: An interpretative phenomenological analysis (IPA) methodology was employed. Eleven participants took part in the study and complete

Partial cauda equina syndrome after an uneventful minimally invasive microdiscectomy in a patient with Crohn's disease.

Cauda equina syndrome is a serious condition resulting from dysfunction of the lumbosacral nerve roots and characterized by impairment of bladder, bowel, sexual and lower limb functions. We report the case of a 48-year-old woman who had Crohn's disease for more than twenty years. The patient was undergoing immunotherapy with infliximab and developed a partial cauda equina syndrome after an uneventful minimally invasive microdiscectomy (L5-S1) that completely cured her sciatica. A postoperative magnetic resonance imaging examination showed root clumping but no compressive lesion. We discuss a p

Metastatic thymoma presenting as spontaneous epidural lumbar haematoma.

We report the case of a 44-year-old man who was found to have metastatic thymoma to his lumbar spine presenting as a spontaneous epidural haematoma. The man presented with back pain and cauda equina like symptoms in the absence of trauma, antiplatelet or anticoagulant agents. Following a laminectomy and excision of the epidural collection he made a full neurological recovery. Histopathology of the haematoma demonstrated metastatic thymoma. To the best of our knowledge, this is the first such case of metastatic thymoma to the lumbar spine presenting as a spontaneous epidural collection. We beli

The Evaluation of Digital Rectal Examination for Assessment of Anal Tone in Suspected Cauda Equina Syndrome.

STUDY DESIGN: Seventy-five doctors completed a questionnaire documenting their grade, specialty, and experience in performing digital rectal examination (DRE). A model anus, using a pressure transducer surrounding an artificial canal, was assembled and calibrated. Participants performed 4 DREs on the model (with a break between attempts) and predicted tone as "reduced" or "normal" (35 and 60 mm Hg, respectively), followed by a "squeeze" test. Thirty health care assistants partook as a control group with no training in DRE. OBJECTIVE: Our main objective was to investigate the validity of digit

Acute myelopathy or cauda equina syndrome in HIV-positive adults in a tuberculosis endemic setting: MRI, clinical, and pathologic findings.

BACKGROUND AND PURPOSE: Cape Town is the center of an HIV-tuberculosis coepidemic. This study's aim was to highlight the importance and to describe the MR imaging features of tuberculosis in acute myelopathy and cauda equina syndrome in HIV-positive adults. To accomplish this we retrospectively reviewed the MR imaging and clinico-pathologic findings of HIV-positive patients presenting to our hospital with recent onset paraplegia and sphincter dysfunction over a 4-year period, 2008-2011. MATERIALS & METHODS: MR imaging, CD4 count, and CSF analysis and pathology were correlated in 216 cases. R

Lumbar solitary osteochondroma presenting with cauda equina syndrome: a case report.

Although spinal osteochondromas can cause various clinical signs, the spinal cord or nerve root compression by solitary lumbar osteochondromas are rare clinical entities. We present a 62-year-old female patient with lower-back pain, progressive left leg paresis, numbness on the both lower extremities and urinary incontinence. The patient's clinical picture made us suspect the possibility of cauda equina syndrome. Radiological examination revealed a lesion originating from the left inferior articular facet of the second lumbar vertebrae. Urgent surgical decompression was performed and the lesio
