Quantification of sleep disordered breathing by computerized analysis of oximetry, heart rate and snoring.

Intermittent snoring and cyclic oscillations of heart rate and oxyhaemoglobin saturation (Sao2) are characteristic features of the obstructi … ) and desaturations (RDID) with the apnoea plus hypopnoea index (AHI) from simultaneously performed polysomnography. The study population consisted of 53 snorers with an AHI of 19.0 +/- 2.6 (median +/- SEM; range 0.7-87.8). Whereas both RDI's from MESAM correlated rather weakly with the AHI from polysomnography (RDIH: r = 0.32, p less than 0.05; RDIS: r = 0.33, p …

Accuracy of the Pulsox-Me500, a pulse oximeter with built-in accelerometer, as an obstructive sleep apnea screening device: A pilot study.

The Pulsox-Me500, a pulse oximeter equipped with a built-in three-axis accelerometer, was recently developed by Konica Minolta, Inc. However, its accuracy of screening obstructive sleep apnea (OSA) has not been investigated, and this was the aim of our study. Participants simultaneously used the Pulsox-Me500 and a SleepScope (a portable one-channel electroencephalography device) in the residential setting and wore this device during polysomnography in the inpatient setting. An interclass correlation coefficient (ICC) (2,1) value of 0.4-<0.75 and ≥ 0.75 was considered fair to good and excellent…

Sleep Disordered Breathing in Joubert Syndrome: Disease Spectrum in a Pediatric Cohort.

BACKGROUND: Joubert syndrome (JS) is a rare, autosomal recessive condition with multisystem manifestations. Sleep disordered breathing (SDB) is a prominent feature, with patients characteristically presenting with a mixture of hyperpnea and apnea. Reports describing whether the respiratory abnormalities change as patients grow older are lacking. METHODS: We describe the clinical course of 21 children with JS followed over 16 years in a tertiary pediatric hospital. RESULTS: Fourteen children underwent polysomnography, while 11 had multiple repeat studies. Patients exhibited a range of SDB, in…

Central sleep apnea caused by low dosage of baclofen: a case report.

BACKGROUND: Baclofen can relax muscle spasticity by inhibiting the transmission of both mono- and polysynaptic reflexes at the spinal cord. It has been increasingly used off-label for the management of spinal cord lesions or neurologic disorders. However, the expansion use has led to an increase in baclofen-associated complications. CASE PRESENTATION: We report the case of a 60-year-old Chinese female patient who suffered from nocturnal choking and daytime sleepiness. Polysomnography identified central sleep apnea after initiation of low-dosage baclofen (10 mg two times a day) treatment, and …

Evaluation of the central sleep apnea in asymptomatic children with Chiari 1 malformation: an open question.

INTRODUCTION: Type I is the most common Chiari malformation in children. In this condition, the lower part of the cerebellum, but not the brain stem, extends into the foramen magnum at the base of the skull leading to intermittent brain hypertension. In symptomatic children, central sleep apneas are shown in polysomnography evaluation. A central apnea index of 1/h or more is considered abnormal, but >5/h is clearly considered pathological. Therefore, central sleep apnea evaluation in pediatric age may show great age-related variability. METHOD AND SUBJECTS: We present three patients who were …

The Kushida Index as a screening tool for obstructive sleep apnoea-hypopnoea syndrome.

OBJECTIVES: To test the validity of the Kushida Index for screening for sleep apnoea in a West of Scotland adult population. METHODS: Specific intra-oral measurements and respiratory polysomnography were carried out on 71 patients in this prospective study. The intra-oral measurements were applied to the Kushida formula to obtain a value for the Kushida Index. This value was compared to the diagnosis obtained using polysomnography in the conventional manner. RESULTS: The sensitivity of the Kushida Index in this present study was 68% (95% CI 50-81) and the specificity was 71% (95% CI 52-84). …

Arnold-Chiari malformation presenting as sleep apnea syndrome.

Sleep apnea syndrome is a recognized manifestation of Arnold-Chiari malformation that almost invariably has been described in association with other neurological findings. We report a 39-year-old man who presented with severe daytime hypersomnolence. A detailed neurological examination was completely normal. Polysomnography revealed moderately severe mixed central and obstructive sleep apnea and hypopnea with persistence of central apnea and hypersomnolence despite nasal continuous positive airway pressure therapy. A history of severe headaches precipitated by sustained laughter prompted the c…

Prevalence of every night snoring and obstructive sleep apnoeas among 30-69-year-old men in Bologna, Italy.

An epidemiological survey of the prevalence of snoring and sleep apnoeas was performed on 3479 30-69-year-old men living in Bologna, north-east Italy. First a postal questionnaire was sent. It was returned with appropriate answers by 1170. A 20% random sample of those who did not answer were invited by telephone to return the questionnaire. Among these groups 119 (10%) and 19 (5.6%) respectively answered that they always snored. A random sample of 40 every-night snorers were studied by polysomnography. Based on the frequency of every-night snoring and the results of polysomnography we estimate…

Accuracy of Detecting Obstructive Sleep Apnea Using Ambulatory Sleep Studies in Patients With Syndromic Craniosynostosis.

STUDY OBJECTIVES: Obstructive sleep apnea (OSA) is seen in up to two-third of the patients with syndromic craniosynostosis. Gold standard to diagnose OSA is the hospital-based polysomnography, although alternatively ambulatory home sleep apnea devices are available. Aim of this study was to assess (1) accuracy of ambulatory sleep studies, (2) clinical decision making following sleep studies, and (3) course of OSA during long-term follow-up. MATERIALS AND METHODS: A retrospective cohort study was performed in children with syndromic craniosynostosis, of whom polysomnographies and home sleep ap…

Quantitative assessment of the quality of home sleep studies: A computer-assisted approach.

Home monitoring is the most practical means of collecting sleep data in large-scale research investigations. Because the portion of recording time with poor-quality data is higher than in attended polysomnography, a quantitative assessment of the quality of each signal should be recommended. Currently, only qualitative or semi-quantitative assessments are carried out, likely because of the lack of computer-based applications to carry out this task efficiently. This paper presents an innovative computer-assisted procedure designed to perform a quantitative quality assessment of standard respira…
