PURPOSE: Sporadic hemiplegic migraine is a rare form of migraine associated with motor weakness during the aura phase. While the aura usually lasts less than 1 hour, patients with sporadic hemiplegic migraine frequently have prolonged weakness.
CASE REPORT: A 60-year-old male had sporadic hemiplegic migraine after a head injury at the age of 14. He presented to our emergency department with a typical migraine attack except prolonged right limbs weakness and numbness (>1 day). Brain magnetic resonance imaging showed an acute infarction in the left posterior medial pons. He recovered completely…

Hyperemesis gravidarum can impair correct absorption of an adequate amount of thiamine and can cause electrolyte imbalance. This study inves … peat MRI showed a marked improvement of WE-related alterations and a new hyperintense lesion in the pons, suggestive of central pontine myelinolysis. No sign or symptom due to involvement of the pons was present.

OBJECTIVE: To study the clinical features, neuro-radiology and treatment of intracranial subependymomas.
METHODS: A total of 22 subependymomas were reviewed with regards to clinical presentation, neuro-imaging features, treatment, histopathological features and long-term follow-up. There were 13 males and 9 females with an age range of 5 to 54 years old. Fourteen tumors were located in lateral ventricles, 1 located on frontal-parietal lobe, 1 on mesencephalon, 2 on pons, 2 in fourth ventricle and 1 on cerebellum. One tumor was located in temporal lobe, mesencephalon, thalamus and pons.

Rapid eye movement (REM) sleep behavior disorder (RBD) is frequently seen in patients with neurodegenerative diseases. It can also be seen in brainstem lesions involving the pons. However, RBD in patients with a pure pontine infarction has been rarely reported. A 68-year-old man had a two-month history of violent behavior during sleep. His nocturnal behavior was screaming, thrashing of arms, punching or kicking his spouse, and falling out of bed associated with nightmares of being attacked by what he described as, "terrible animals frightening me." No remarkable neurological signs were found. …

OBJECTIVE: To determine the ischemic lesions distribution and extension of patients with basilar artery thrombosis by the means of magnetic … urred in pontes, cerebellums and mesencephalons than medullas, thalami and occipital lobes. Basilar pons, cerebral crus and cerebellum hemisphere were more susceptible than pontine tegmentum, vermis, midbrain tegmentum and tectum.
CONCLUSIONS: When the basilar artery is occluded, basilar pons, cerebral crus and cerebellum hemisphere were most suscepti …

We herein report the case of a 53-year-old woman with Behcet's disease and an acute T2-hyperintense lesion in left side of the pons. Echo-planar "trace" diffusion MR imaging revealed high signal intensity changes at the lesion site on b = 1000 s/mm(2) images, initially suggesting restricted diffusion. On corresponding apparent diffusion coefficient maps, however, the lesion had high signal intensity and high apparent diffusion coefficient values (1.22 x 10(-3) mm(2)/s), compared with the contralateral normal side of the pons (0.86 x 10(-3) mm(2)/s) and compared with the normal temporal white m…

A 4-yr-old girl with a ganglioglioma involving the cerebellum and pons presented with CO2 retention and arterial O2 desaturation during wakefulness and sleep. Despite the presence of a regular respiratory pattern during wakefulness, NREM sleep was associated with the appearance of a sustained periodic respiratory pattern characterized by clusters of usually two breaths separated by an 8 to 10-s expiratory pause. At sleep onset, this Biot's respiratory pattern appeared immediately after a sigh. Breathing frequency during NREM sleep became regular with administration of supplemental O2 and an at…

A 30-year-old man patient presented with the progressive weakness and pain in the back, arms and legs, sensory loss, facial weakness, diplopia, difficulty of speech and swallowing since 4 months. MRI revealed syringomyelia and syringocephaly involving medulla (syringobulbia), pons (syringopontia) and midbrain (syringomesencephaly) with intrasyringeal hemorrhage.

BACKGROUND: In this study, we aimed at determining the dynamic thiol/disulfide homeostasis and oxidant balance, and investigating the relation of these parameters to the severity of the disease and the serum calcium levels.
METHODS: 55 patients with iatrogenic hypoparathyroidism follow-ups and 40 healthy volunteers were included in the study. The blood dynamic thiol/sulfide balance, Total Antioxidant Status (TAS), Total Oxidant Status (TOS), Paraoxonase Enzyme Activity (PON) levels were measured in serum samples.
RESULTS: In our study, it was found that the disulfide, disulfide/native thiol,

BACKGROUND: Bilateral medial medullary infarction (MMI) is uncommon and bilateral medial pons infarction (MPI) is even rarer. "Heart appearance" on magnetic resonance imaging (MRI) is a characteristic presentation of bilateral medial medullary infarction (MMI).
CASE PRESENTATION: We present 67-year-old Chinese diabetic and hypertensive female patient affected with "heart appearance-like" infarction in bilateral ponto-medullary junction on MRI. Abnormal signal was observed in the bilateral ponto-medullary junction on T1, T2, fluid-attenuated inversion recovery and apparent diffusion coefficien…

We encountered a case of traumatic basilar artery dissection presenting with "locked-in" syndrome. A 46-year-old-man fell approximately 2 meters and landed in an inverted position. He was admitted to our hospital after 4 days. He was in coma on admission, and neurological examination revealed that his pupils were 2 mm in size and reactive, the corneal reflex of both sides was absent, the pharyngeal reflex was absent, and quadriplegia was present. CT and MRI showed infarction at the ventral portion of the pons. 3D-CT and CT using a bone algorithm showed fractures of the upper clivus, the left p…

Trigeminal neuralgia (TN) is most commonly caused by vascular compression of the trigeminal nerve root entry zone. Secondary trigeminal neuralgia due to ischemic lesion of the pons is very rare. Here we report a patient with a pontine infarct transecting the central trigeminal pathways resulting with trigeminal neuralgia.

A 50-year-old woman had primary position upbeating nystagmus, abnormalities of the lower cranial nerves, mild cerebellar signs, and CSF lymphocytosis. CT and MRI showed enlargement of the pons. Eye movement recordings revealed impaired pursuit and optokinetic nystagmus, and visual-vestibular interactions were absent. The patient improved spontaneously.

An English springer spaniel was presented for right-sided atrophy of the muscles of mastication, analgesia and paralysis of the face, and vestibular dysfunction. Neurological signs were consistent with a lesion involving the pons and rostral medulla resulting in deficits in the function of the trigeminal, facial, and vestibular nerves. MRI disclosed a right-sided extraparenchymal mass consistent with a trigeminal nerve sheath neoplasm that was compressing and invading the pons and medulla. Atrophy of the muscles of mastication, innervated by the trigeminal nerve, was also observed on MRI. Addi…

The neurological examination revealed right hemiparesis and hemi-hypoesthesia. A brain magnetic resonance imaging scan revealed a cerebellopontine lesion, arising from th…

We report a 73-year-old man with alternating abducent hemiplegia (Raymond syndrome) and contralateral supranuclear facial nerve palsy. On admission, he showed lateral gaze palsy of the right eye, left supranuclear facial nerve palsy, dysarthria and left hemiparesis. Brain MRI showed an infarct that was located in the paramedian and lateral area in the base of the caudal pons on the right side. MRA showed a mild stenosis of the basilar artery. Hemiplegia and supranuclear facial nerve palsy were considered to be caused by the involvement of corticospinal tract and corticobulbar tract that run at…
