OBJECT: The mechanism responsible for exceptional examples of intraneural ganglia with extensive longitudinal involvement has not been understood. Such cases of intraneural cysts, seemingly remote from a joint, have been thought not to have articular connections. Decompression and attempted resection of the cyst has led to intraneural recurrence and poor neurological recovery. The purpose of this report is not only to clarify the pathogenesis of these cysts, but also to discuss their treatment based on modern concepts of intraneural ganglia.
METHODS: Two examples of extreme longitudinal propa

The aetiology of painful dorsal wrist ganglia remains obscure. In a prospective study we investigated the link between a painful dorsal wrist ganglion and wrist joint abnormality with wrist arthroscopy before excision of the ganglion. Of 16 wrists arthroscoped 12 were abnormal, 10 had an abnormal scapholunate joint, and two had abnormal lunatetriquetral joints. We think that painful dorsal wrist ganglia, like popliteal cysts in the knee, are markers of underlying joint abnormalities. Surgeons who treat painful ganglia should be aware of a possible underlying cause so that they can target treat…

Schwannoma in the popliteal fossa is still rare, often diagnosed late because it grows slowly and has no symptoms. It is often misdiagnosed with connective tissue tumors or with neurological disorders originating in the spine or disorders of the peroneal nerve. Schwannoma within the common peroneal nerve is still rare in the popliteal fossa, with most tumor sizes around 2 cm in diameter due to their smaller size of nerve but can cause neurologic disturbance, especially when it is large. And over a long time, it can cause serious complaints like neurological deficits and make surgery difficult

The presence of a Baker's cyst may diminish the therapeutic effect of intra-articular 90Y treatment to the knee joint. SPECT (single photon emission computed tomography) was used in 10 patients with Baker's cysts, treated with intra-articular 90Y for persistent synovitis of the knee, to measure the amount of radioisotope within the cyst. Comparison was made with the arthrographic appearances. The likelihood or extent of uptake into Baker's cysts of 90Y injected into the synovial cavity of the knee was not predictable from arthrograms. Deposition of 90Y in cysts was variable, ranging from littl

OBJECTIVE: The formation and propagation of intraneural ganglion cysts (INGCs) is being elucidated by the unified articular theory. Its principles include a connection for joint fluid to egress from a synovial joint via an articular branch to a parent nerve and cyst following the path of least resistance, dependent on pressures and pressure fluxes. The occurrence of so-called extreme peroneal or tibial INGCs in the popliteal fossa extending to the sciatic nerve has been reported. One rarely described variant with a circumferential cyst within the subparaneurial compartment outside the epineuri

OBJECTIVE: To demonstrate that tibial intraneural ganglia in the popliteal fossa are derived from the posterior portion of the superior tibiofibular joint, in a mechanism similar to that of peroneal intraneural ganglia, which have recently been shown to arise from the anterior portion of the same joint.
DESIGN: Retrospective clinical study and prospective anatomic study.
MATERIALS: The clinical records and MRI findings of three patients with tibial intraneural ganglion cysts were analyzed and compared with those of one patient with a tibial extraneural ganglion cyst and one volunteer. Seven

RATIONALE: Synovial cysts are well known in rheumatoid arthritis (RA), and most common in the popliteal fossa. They may produce lots of local symptoms and complaints, which may present initially as an unrelated clinical condition. Few studies have reported multiple extra-articular synovial cysts (MESCs) in the RA patients. Early diagnosis is crucial for patient treatment.
PATIENT CONCERNS: A 50-year-old man without any special clinical histories found a soya bean size bump at the left elbow medially, then multiple lumps were found at bilateral elbows and gradually increasing. No pain, no acti

Immunity to Toxoplasma gondii, as measured by oocyst shedding, was studied in cats. In 3 trials, 12 3-mo-old T. gondii-free cats were fed tissue cysts of the ME-49 strain of T. gondii. All cats shed T. gondii oocysts for approximately 1 wk starting 3-5 days after ingesting tissue cysts. One cat became ill because of toxoplasmic pneumonia and was killed 17 days after inoculation. The remaining cats remained clinically normal. Approximately 3 mo after primary infection, these 11 cats (immune) and 11 age-matched or littermate uninfected cats (nonimmune) were challenged orally with tissue cysts of

BACKGROUND: During the past nine years three cases of cystic adventitial disease of the popliteal artery have been diagnosed and treated in our medical institution. Different approaches were used in the treatment in all these cases. The purpose of this report is to summarise the current knowledge of the etiology, presentation, diagnostics and treatment of this condition, with the addition of new cases.
METHODS: Information about three new cases is presented and discussed together with that from the relevant publications obtained from the Pubmed database. Results. In the first case resection w

BACKGROUND: Associated joint disorders with popliteal cysts were stated approximately between the ranges of 41-83 % in all reported cases. Combined treatment strategies that eliminate intra-articular pathologies and cyst- associated valve mechanisms are thought to be a good option in treatment of the disease. In this study, our main objective is to present clinical results of our combined treatment results, which includes posterior cyst excision with supine arthroscopic intervention, targeting intra-articular pathologies on recalcitrant cases.
METHODS: One hundred three knees of 100 patients …

INTRODUCTION: Ganglionic cysts are mostly found around the wrists, but they can occur in any part of the body. They are also common around the knees as popliteal cysts and meniscal cysts. Intra-articular cysts of the knee, however, are much rarer, with only sporadic cases being reported till 1990. Most of these cysts are asymptomatic and detected during routine magnetic resonance imaging (MRI) and arthroscopy for other pathologies. Most of the time, they are reported in relation to the anterior cruciate ligament (ACL) and rarely in relation to the posterior cruciate ligament, menisci, and very…

BACKGROUND: Cystic adventitial disease (CAD) is an important and rare non-atherosclerotic cause of intermittent claudication and critical limb ischemia. Since the first case of CAD involving the external iliac artery was described by Atkins and Key in 1947, approximately 300 additional cases have been reported.
OBJECTIVES: The aim of this article is to report a rare vascular disorder, predominantly seen in young healthy men with minimal cardiovascular risk factors.
METHODS: We report a rare case of cystic adventitial disease of a young policeman. To confirm the diagnosis, an ultrasonography

CASE: An 87-year-old woman presented with a popliteal skin lesion due to polyethylene wear 13 years following a left total knee arthroplasty. Excisional biopsy through a posterior approach was performed. Histology showed inflammation with granulomas and birefringent foreign body particles in the skin. Cultures remained negative, and revision knee arthroplasty was performed.
CONCLUSIONS: A popliteal skin lesion due to polyethylene wear disease in total knee arthroplasty has not previously been described. Patients with an atypical inflammation of the skin with an underlying joint implant should

BACKGROUND: Cystic adventitial disease (CAD) is a rare condition in which mucinous cyst forms within the adventitia of arteries and veins. The management of CAD still remains unclear and a wide range of imaging and treatment options has been described. The purpose of this study is to propose an update of etiology, clinical diagnosis, imaging modalities and treatment options in the setting of CAD described on arterial territory.
METHODS: We performed a systematic review including studies reporting case or case series of CAD, searching across the Medline, Embase and Cochrane databases.

Adventitial cystic disease (ACD) is an uncommon condition with only around 300 cases reported in the literature. ACD consists of a collection of gelatinous material within a cyst, that is adjacent or surrounding a vessel. In the last five years three cases of ACD have been observed: the first case was a 48-year-old man, ex nicotine abuser, with a four-month history of progressive claudication; the second case was a 55-year-old man, ex nicotine abuser, with ischemic heart failure and a one-year history of progressive claudication; the third case was a 70-year-old man, with diabetes, dislipidemi

Peroneal nerve ganglion cysts typically present because of a palpable mass or symptoms and signs of entrapment neuropathy, including pain, diminished sensation, and motor weakness. Surgical treatment is usually successful when performed early, but, when diagnosis is delayed, intraneural growth and invasion may cause irreversible axonal injury and footdrop. This case report illustrates the importance of timely diagnosis when treating a ganglion of the peroneal nerve and reviews the appropriate workup, differential diagnosis, and treatment.
