Monitoring of intracranial pressure has become increasingly widespread in a variety of conditions. The information gained has allowed for the development of certain principles of management in patients with presumed or potential intracranial hypertension. These principles are applicable whether ICP monitoring is employed or not. Aggressive management of severe intracranial hypertension may improve survival; however, data from several studies are inconclusive. The author attempts to review developments in the field and assess the impact of this modality of therapy in pediatric neurosurgical pra…

Hypothalamic hypopituitarism has been associated with the intermittent intracranial hypertension of "normal-pressure" hydrocephalus. Six patients with this condition were studied endocrinologically; five showed evidence of hypothalamohypophyseal insufficiency, and, though only one needed treatment, all needed continued assessment of their endocrine state during follow-up. Thus, hypothalamohypophyseal dysfunction is more common than might be expected. Tests of anterior pituitary function should be preformed before surgical intervention, as such dysfunction may adversely affect survival and the …

Two groups of Korean patients with tuberculosis, matched in pairs for age and duration of chemotherapy, have been studied. One group had received ethambutol in a mean dosage of 14.4 mg/kg and the other antituberculosis chemotherapy not including ethambutol for at least 3 months. All patients had routine ophthalmological assessments on admission to the study, including examination of the fundi, visual fields and macular thresholds. Intra-ocular pressures were measured and a water-drinking test was then carried out. All the assessments were made by the same ophthalmologist who was at all times k…

A detailed review of the hormonal effects on intraocular pressure is presented. There is evidence that corticotropin, vasopressin, thyroxin, insulin, glucocorticoids and mineralocorticoids may play a role in the physiologic regulation of intraocular pressure. Growth hormone, melanocyte stimulating hormone, progesterone, estrogen, chorionic gonadotropin and relaxin may influence intraocular pressure when administered in pharmacologic doses. Whether the key to understanding primary open-angle glaucoma lies in recognizing abnormal endocrine mechanisms, especially involving glucocorticoids, remain…

The physiopathological conditions of the normal pressure hydrocephalus syndrome are analysed. Within a group of 21 patients with clinical sintomathology pertaining this entity, only were 10 selected with a clinical, radiological and cisternografic indubitable diagnosis. It has been noticed that in spite of the stricked selection only a 50% of them have been beneficed by the application of a low pressure valve. Then, the hypothesis that the disease is a multicausal syndrome appears, which by an identical physiopathologic process produces or exhibits a similar clinical, radiological and cisterno…

The losses of righting reflex produced by various doses of phenobarbital in mice at 1 atm O2 versus 1 atm O2 plus 102 atm He were determined. The resulting dose-response curve at pressure gave an ED50 that was 64 per cent larger than the ED50 at 1 atm. This increment is essentially the same as that found for gaseous anesthetics under similar test conditions. The quantitative similarity of the results of pressure reversals of barbiturate and inhalational anesthetics suggests that the mechanisms or sites of action of these agents are similar. However, the dose-response curve at 103 atm was steep…

Intracranial pressure (ICP) was continuously monitored by the Richmond technic of Vries and Becker in 17 patients undergoing elective craniotomy. This method entails the placement, underlocal anesthesia, of a hollow screw through the cranium into the subarachnoid space. The screw was connected to a Statham P23Db pressure transducer with heavy vinyl tubing and with pressures recorded on a Beckman Dynograph. The effects of 3 anesthetic technice-halothane, enflurane, and nitrous oxide-narcotic-relaxant-on ICP during induction and maintenance were compared with preinduction control pressures. Cont…

The syndrome of low intracranial pressure may develop in a variety of circumstances, e.g. after lumbar puncture, following head injury and intracranial operations. It sometimes occurs after viral meningitis. The case history is described of a woman in whom the disorder was unusually severe and persistent. Interesting radiological features were observed, the appearances being those of herniation of the brain towards the tentorial opening. Simple measures of treatment produced dramatic and lasting relief.
