Position paper: appropriate use of pharmacotherapeutic agents by the orofacial pain dentist.

Orofacial Pain Dentistry is concerned with the prevention, evaluation, diagnosis, treatment, and management of persistent and recurrent orofacial pain disorders. The American Dental Association, through the Commission on Dental Accreditation (CODA), now recognizes Orofacial Pain as an area of advanced education in Dentistry. It is mandated by CODA that postgraduate orofacial pain programs be designed to provide advanced knowledge and skills beyond those of the standard curriculum leading to the DDS or DMD degrees. Postgraduate programs in orofacial pain must include specific curricular content

The Oral Microbiome and Systemic Health: Bridging the Gap Between Dentistry and Medicine.

The oral microbiome, consisting of a mixture of bacteria, fungi, and viruses, is an important contributor to oral and systemic health. Microbial balance disruptions are associated with oral pathologies like dental caries and periodontitis as well as systemic diseases such as cardiovascular diseases, adverse pregnancy outcomes, and respiratory diseases. This review explores the mechanistic pathways linking oral dysbiosis to systemic inflammation, endothelial dysfunction, and immune modulation. The roles of key microbial species in health and disease are analyzed, with an emphasis on how hematog

Management of Severe Riga-Fede Disease in a Child With MIRAGE Syndrome.

Riga-Fede disease (RFD) is a rare condition in infants and young children caused by repetitive trauma to the oral mucosa, often from newly erupted teeth. We present the case of a 13-month-old male child with a complex genetic disorder who developed persistent ulceration on the ventral tongue due to contact with mandibular incisors. Initial management included conservative smoothing of the teeth, which led to only partial improvement. Over time, the mucosal ulceration worsened, resulting in mucosal separation and soft tissue loss. A multidisciplinary surgical approach was pursued. Pediatric oto

Acceptance and awareness of southeastern and western private practice pediatric dentists of fluoride-free toothpastes: a survey study.

The American Academy of Pediatric Dentistry (AAPD) affirms that the use of fluoride, as an adjunct in the prevention of caries, is safe and effective. The AAPD encourages dentists, other healthcare providers, and parents to optimize fluoride exposures to reduce the risk of caries and to enhance the remineralization of affected teeth. However, there is resistance amongst patients towards fluoride overexposure and despite there being research on other effective remineralizing agents, most pediatric dentists primarily cater their practice to fluoride-based products. The objective of the study is

Sterilizing orthodontic appliances: A systematic review and meta-analysis on the available methods.

Infection control is essential to protect both the doctor and the patient by preventing the spread of infectious diseases. There is no exception in the field of dentistry, particularly in orthodontics, where numerous appliances are used for a variety of functions and also because the mouth cavity has the highest concentration of bacteria of any body part. Through this systematic review, we aimed to assess the various methods of sterilization employed in an orthodontic setting. Using relevant keywords, reference searches, and citation searches, the databases such as PubMed, MEDLINE, Web of Scie

Novel Cell Therapy Using Mesenchymal Stromal Cell Sheets for Medication-Related Osteonecrosis of the Jaw.

Despite medication-related osteonecrosis of the jaw (MRONJ) being first reported in 2003, the optimal treatment and prevention modalities for MRONJ are not clear. As a result, dentistry, oral surgery, and departments involved in the treatment of cancer and bone diseases are struggling with the management of MRONJ. Several cases of MRONJ cannot be managed by conventional treatment strategies recommended in various position papers. Therefore, studies have been conducted to investigate the efficacy of novel therapies for MRONJ. However, the optimal treatment is unknown. Several cell therapies inc

Intravenous lipid emulsion - rescued at LAST.

The accidental overdose of local anaesthetics may prove fatal. The commonly used amide local anaesthetics have varying adverse effects on the myocardium and beyond a certain dose all are capable of causing death. Local anaesthetics are the most frequently used drugs in dentistry and although uncommon, local anaesthetic systemic toxicity (LAST) accounts for a high proportion of mortalities in the dental office, with local anaesthetic-induced cardiac arrest particularly resistant to standard resuscitation methods. Over the last decade there has been convincing evidence of using intravenous lipid

Common implant-related advanced bone grafting complications: classification, etiology, and management.

Bone grafting has become an essential part of implant dentistry. Thanks to the advancement of grafting technology, areas that could only be managed with removable dentures or teeth-supported fixed prostheses can now be successfully treated with implant-supported prosthesis. Nonetheless, the complications associated with bone grafting procedures have slowly emerged to be one of the main challenges to many clinicians. A Medline literature search of articles published from 1984 to 2006 related to advanced bone grafting complications were selected and analyzed. To make it easier to understand, imp

Orofacial tuberculosis mimicry of odontogenic abscess: A diagnostic dilemma!

Orofacial tuberculosis is a rare extrapulmonary form of tuberculosis without pathognomic signs and symptoms. Due to its unusual clinical presentation, orofacial tuberculosis (TB) is more often misdiagnosed as other orofacial pathologies and presents a diagnostic challenge in dentistry. One such case is described in the present case report, where a paediatric patient presented with extraoral submandibular swelling that mimicked an odontogenic infection associated with a mandibular primary molar. The treatment plan was formulated considering the condition as an odontogenic infection, and an inci

Monitoring patient compliance to dental home care instructions with the mobile-based ecosystem.

BACKGROUND: Caries risk assessment and management from the cornerstone for successfully applying a minimum intervention dentistry philosophy in managing dental caries. Most applied work on caries prevention acknowledges the role of oral hygiene and dietary control strategies in reducing the prevalence and incidence of caries. What we need to address, however, is the necessity of initiating and sustaining behaviors that will ensure the implementation of strategies, that is "patient compliance." AIMS AND OBJECTIVES: To present a novel method to monitor oral health-care behavior on a day-to-day

Analysis of the Interaction Between HIV and Periodontopathic Bacteria That Reactivates HIV Replication in Latently Infected Cells.

The acquired immunodeficiency syndrome (AIDS) pandemic caused by the human immunodeficiency virus (HIV) is a major global health concern affecting 38 million people worldwide. HIV gene expression is the major determinant of the rate of viral replication leading to the progression of AIDS. The persistence of cellular reservoirs of HIV proviruses, despite prolonged treatment with antiretroviral drugs, represents the main obstacle preventing the eradication of HIV. Epigenetic silencing by histone deacetylase (HDAC) contributes to maintaining HIV transcriptional latency. However, the mechanism of

Allergic contact dermatitis from two-component acrylic resin in a manicurist and a dental hygienist.

BACKGROUNDS: Two-component acrylic resin used in nail art and dentistry can cause allergic contact dermatitis (ACD), but only a few reports from Japan have been published. CASES: A 35-year-old female manicurist (case 1) and 30-year-old female dental hygienist (case 2) were diagnosed with ACD caused by ethyl methacrylate and methyl methacrylate in a liquid monomer of two-component acrylic resin respectively. Case 1 was sensitized from direct skin contact with nail art acrylic products, which could have been avoided, and gave up both being a manicurist and a user. Onset in case 2 occurred when

[Translational Medicine and its application in stem cell research for periodontal regeneration].

Translational Medicine is an evolutional concept that encompasses the rapid translation of basic research for use in clinical disease diagnosis, prevention, treatment and finally in public health promotion. It follows the idea "from bench to bedside and back", and hence relies on cooperation between laboratory research and clinical care. Translation process is a complex process that requires both research and non-research activities. During the past ten years, there has been intense interest in the development of new clinical procedures, therapeutic molecules, and prototypes based on translati
