Measurement of intracellular versus extracellular prostate-specific antigen levels in peripheral macrophages: a new approach to noninvasive diagnosis of prostate cancer.

Although prostate-specific antigen (PSA) is a useful screening marker in prostate cancer, it has limited specificity. Previously it was shown that the amount of surface-bound PSA present on circulating macrophages was different between patients with localized prostate cancer and those with metastatic prostate cancer. It was recently demonstrated that intracellular PSA in macrophages can be measured by flow cytometry. In the context of searching for a noninvasive, highly reliable method for prostate cancer diagnosis, we assessed the extent to which extracellular (ie, surface-bound) and intracel…

The predictive value of multiparametric magnetic resonance imaging in enhancing prostate-specific antigen assessment for prostate cancer: a cross-sectional study.

BACKGROUND: Prostate-specific antigen (PSA) testing and transrectal ultrasound (TRUS)-guided biopsy remain standard tools for diagnosing prostate cancer (PCa), but both have limitations in sensitivity and specificity, leading to overdiagnosis or missed detection of clinically significant cancers. Multiparametric magnetic resonance imaging (mp-MRI) has emerged as a valuable imaging modality for PCa detection and risk stratification, yet the optimal integration of mp-MRI findings with PSA levels for pre-biopsy assessment remains to be fully established. This study aims to evaluate the predictive…

Confirmation of Prostate-specific Antigen (PSA) Values Improves Prostate Cancer Screening in Early Middle-Aged Men - Data from the PROBASE Trial.

BACKGROUND AND OBJECTIVE: To assess the effectiveness of a confirmatory prostate-specific antigen (PSA) test as a safeguard against unnecessary work-up and to improve the specificity of prostate cancer (PCa) screening. METHODS: Overall, 72,157 PSA values corresponding to screening rounds at age 45-55 yr in the PROBASE trial were analyzed. Positive PSA tests were reported as the proportion of men with an initial or confirmatory PSA ≥3 ng/ml. The cumulative PCa number over 6 yr was described for men with and without confirmatory PSA decrease at baseline screening (age 45 yr). Sankey diagrams we…

Predicting Long-Term Risk for Prostate Cancer Mortality Following a Prostate-Specific Antigen Screening Test: Prognostic Model Development and External Validation.

BACKGROUND: Despite the scale of prostate-specific antigen (PSA) testing for prostate cancer (PCa) screening, prediction models do not predict time-to-event end points or adjust for patient life expectancy. OBJECTIVE: To develop, externally validate, and compare to existing tools a novel prognostic model for risk for prostate cancer-specific mortality (PCSM) after a PSA test. DESIGN: Prognostic model development in the PCa screening group of the Prostate, Lung, Colorectal and Ovarian (PLCO) Cancer Screening Trial; external validation in a Veterans Affairs (VA) population of patients undergoi…

A Phase II Trial Using Grape Seed Extract for Prostate Cancer Patients With Nonmetastatic Prostate-Specific Antigen Progression After Local Therapy.

Prostate cancer (PCa) is a major cause of cancer-related death among American men. While androgen deprivation therapy (ADT) is commonly used in the treatment of rising prostate-specific antigen (PSA) levels after primary therapy, its adverse systemic side effects support exploring low-impact treatment options during the observation period of nonmetastatic PSA-only progression. Grape seed extract (GSE) is a nontoxic phytochemical that has shown anti-PCa efficacy in preclinical studies. Herein, we report the results of a completed Phase II clinical trial (CT.gov-NCT03087903) which investigated t…

Correlation between total prostate specific antigen and histological grading of prostate cancer in Kenyan mission hospital: a five-year retrospective review.

BACKGROUND: Prostate cancer (CaP) is the leading non-cutaneous cancer in men of African descent, with the higher mortality rates found in sub-Saharan Africa. Early diagnosis, staging, and management of prostate cancer could help curb its mortality rate in SSA. However, access to precise radiological imaging for staging purposes is limited in our setting. We sought to evaluate the correlation between total prostate specific antigen (tPSA) and histological grading of CaP in our resource-limited setting. METHOD: We conducted a retrospective review of records of patients treated for biopsy-proven

Combined Prostate-specific Membrane Antigen Positron Emission Tomography and Multiparametric Magnetic Resonance Imaging for the Diagnosis of Clinically Significant Prostate Cancer.

BACKGROUND AND OBJECTIVE: More than half of men who undergo a prostate biopsy based on positive multiparametric magnetic resonance imaging (mpMRI) findings do not have clinically significant prostate cancer (csPCa). Prostate-specific membrane antigen (PSMA) positron emission tomography (PET) may complement mpMRI to better triage men with suspected prostate cancer (PCa) and reduce the number of unnecessary biopsies performed. A diagnostic test accuracy systematic review and meta-analysis was performed to determine the diagnostic accuracy of combined imaging for csPCa detection with pairwise com

Transrectal versus transperineal prostate biopsy for cancer detection in patients with gray-zone prostate-specific antigen: a multicenter, real-world study.

Knowledge about the effect of different prostate biopsy approaches on the prostate cancer detection rate (CDR) in patients with gray-zone prostate-specific antigen (PSA) is limited. We performed this study to compare the CDR among patients who underwent different biopsy approaches and had rising PSA levels in the gray zone. Two hundred and twenty-two patients who underwent transrectal prostate biopsy (TRB) and 216 patients who underwent transperineal prostate biopsy (TPB) between June 2016 and September 2022 were reviewed in this study. In addition, 110 patients who received additional targete…

Biological variation estimates for prostate specific antigen from the European Biological Variation Study; consequences for diagnosis and monitoring of prostate cancer.

BACKGROUND: Prostate-specific antigen (PSA) is central in the diagnosis of prostate cancer. However, high-quality biological variation (BV) estimates for PSA are scarce. Here BV estimates from the European Biological Variation Study (EuBIVAS) for total (tPSA), free (fPSA), conjugated PSA (cPSA), and percent free PSA (%fPSA) are provided. METHOD: EuBIVAS samples were collected weekly from thirty-seven healthy males (22-59 years) for 10 weeks. All samples, stored at -80 °C, were measured in duplicate with a Roche Cobas e801. Outlier and homogeneity analysis were performed followed by CV-ANOVA t…

Phase I Study of CTT1057, an 18F-Labeled Imaging Agent with Phosphoramidate Core Targeting Prostate-Specific Membrane Antigen in Prostate Cancer.

Agents targeting prostate-specific membrane antigen (PSMA) comprise a rapidly emerging class of radiopharmaceuticals for diagnostic imaging of prostate cancer. Unlike most other PSMA agents with a urea backbone, CTT1057 is based on a phosphoramidate scaffold that irreversibly binds to PSMA. We conducted a first-in-humans phase I study of CTT1057 in patients with localized and metastatic prostate cancer. Methods: Two patient cohorts were recruited. Cohort A patients had biopsy-proven localized prostate cancer preceding radical prostatectomy, and cohort B patients had metastatic castration-resis

Undetectable prostate specific antigen at 6-12 months: a new marker for early success in hormonally treated patients after prostate brachytherapy.

BACKGROUND: The concept of a prostate-specific antigen (PSA) "nadir" has been used as a predictive marker for treatment success in patients treated with radiotherapy for localized prostate carcinoma. However, this approach is not applicable in patients who are concomitantly treated with short-term hormonal therapies. To address this, the authors sought to develop a new predictive marker in such patients after prostate brachytherapy (BT). METHODS: Between March 1997 and November 2002, 194 men with clinical Stage T1A-T3N0M0 prostate carcinoma (according to the 1992 International Union Against C…

Screening for prostate cancer using prostate-specific antigen alone as a first-line checkup parameter: results of the health checkup system.

BACKGROUND: The incidence of prostate cancer in Japan is not very high but it is the most increasing malignant tumor form. To decrease the mortality from cancer, detection of early cancer and early treatment are most effective. As a primary screening for prostate cancer, measurement of serum prostate-specific antigen(PSA) added to the health checkup system has not been assessed. METHODS: Among males who received a health checkup during a 30-month period, serum PSA levels were measured in males who desired prostate cancer screening. The cut-off value for PSA was 4.0 ng/ml. Males with serum PSA…
