Minimally invasive approaches for implant-supported overdentures in the atrophied mandible.

OBJECTIVES: The study compares three minimally invasive approaches for the retention of implant supported mandibular complete dentures, particularly focusing on patient satisfaction. BACKGROUND: The McGill Consensus Statement recommends restoration of the edentulous mandible with an overdenture retained on two implants. Alternatively, less invasive treatment concepts with shorter treatment times have been developed for critical cases. MATERIAL AND METHODS: Thirty-nine patients (with a total of 78 implants) with advanced mandibular bone atrophy were randomly assigned to three groups: "single

An alternative multiple pontic design for a fixed implant-supported prosthesis.

In situations of moderate residual ridge resorption where multiple tooth replacement is needed, and where the patient desires a fixed implant-supported restoration, it is challenging to design a pontic-tissue interface. The semiconvex multiple pontic design described in this article, with its mucosal contact exerted with moderate pressure, is proposed to circumvent the problems encountered with the plaque accumulation, maintenance conditions, phonetics, and compromised esthetics frequently encountered in these patients. The use of a screw-retained, implant-supported restoration is also emphasi

Clinical outcome of dental implants placed with high insertion torques (up to 176 Ncm).

BACKGROUND: Primary stability can be improved by using a tapered implant in a slightly underprepared implant site. This may lead to high compression forces and elevated insertion torques. It has been postulated that disturbance of the local microcirculation may occur, leading to necrosis of the osteocytes and bone resorption. PURPOSE: Report on the clinical outcome of 42 implants placed with an insertion torque equal or greater than 70 Ncm and evaluate bone levels around these implants. MATERIALS AND METHODS: This prospective study included 48 patients treated with 66 4.5 mm diameter Tapered

Management of an implant case that required retreatment: a case report.

The presented case represents how the lack of a preoperative panoramic radiograph or any radiograph contributed to incorrect case planning, poor implant selection, and the careless surgical placement of an implant into or dangerously close to the lower left mental foramen, resulting in a paresthesia of 9 months duration. Two implants were surgically removed and the bone defects were grafted with a mixture of irradiated cancellous bone plus Biogran. One implant was sectioned and intentionally "put to sleep." The case was successfully completed with a lower bar overdenture supported by 4 screw i

Cement failure loads of 4 provisional luting agents used for the cementation of implant-supported fixed partial dentures.

The major disadvantage of cement-retained implant-supported fixed partial dentures (FPDs) is difficulty in retrievability. The retentive strengths of the provisional luting agents usually employed in these cases are therefore an important consideration. This study evaluated the cement failure loads of 4 provisional luting agents used for the cementation of FPDs supported by 2 implants or 4 implants. Nogenol luting agent exhibited the lowest retentive values in both types of FPD. ImProv proved to be the most retentive cement of all tested. Temp Bond NE and Temp Bond presented significantly diff

Effect of implant number, attachment type, and framework material on stress distribution in mandibular overdentures: a finite element analysis.

BACKGROUND: Using finite element analysis, we evaluated the effects of implant number, attachment type, and framework material on stress distribution in bone and in implant and prosthetic components of mandibular overdentures in completely edentulous mandibles. METHODS: Straumann implants (4.1 mm diameter and 12 mm length) were placed in a completely edentulous mandible in two configurations. For the two-implant models, implants were positioned axially in the lateral–canine region while for the three-implant models, one implant was placed at the midline and two were positioned bilaterally in t

A 3-year follow-up of ceria-stabilized zirconia/alumina nanocomposite (Ce-TZP/A) frameworks for fixed dental prostheses.

PURPOSE: Zirconia/alumina nanocomposite stabilized with cerium oxide (Ce-TZP/A) shows significantly higher mechanical strength than yttrium-oxide-partially-stabilized zirconia (Y-TZP) and allows post-sintered machining that does not require any subsequent treatment. The aim of this prospective study was to assess the clinical performance of veneered Ce-TZP/A frameworks for fixed dental prostheses. METHODS: Fifteen patients with 22 Ce-TZP/A fixed prostheses were included in this study. The fixed dental prostheses were cemented with resin cement (baseline) and were evaluated at baseline and at

Potential damage to bone-implant interface when measuring initial implant stability.

BACKGROUND: An electronically controlled mechanical tapping device (MTD) to measure implant stability has been studied extensively for its ability to measure initial stability at the time of surgical placement and predict an implant's survival prognosis, with few reported complications. METHODS: Initial stability data, measured repeatedly with a resonance frequency analysis device and the MTD, for seven of 28 implants placed in jawbones of four fresh human cadavers and the histologic images of bone-implant interfaces are presented as evidence for potential damage to the bone-implant interface

Case Report: Advanced prosthodontic rehabilitation using implant-assisted partial denture guided by the neutral zone concept after fibular reconstruction.

Rehabilitation of partially edentulous patients following fibular free-flap reconstruction presents significant functional and biomechanical challenges due to altered soft tissue morphology, reduced vestibular depth, and compromised load-bearing capacity. In cases involving extensive tissue loss, fixed prosthetic designs may be limited in their ability to support the tongue and cheeks, while implant-assisted removable partial dentures (IARPDs) offer better hygiene access and prosthetic flexibility. This case report describes a prosthetically driven approach that integrates CBCT-guided planning

Re-revision rate after anatomic-to-anatomic revision of total shoulder arthroplasty: a descriptive study of the Dutch national registry.

BACKGROUND: Anatomical total shoulder arthroplasty (aTSA) is typically reserved for end-stage glenohumeral arthritis with an intact rotator cuff and adequate glenoid bone stock. With the exponential increase in total shoulder arthroplasty surgeries, complications and the need for re-revision surgery have also increased. Typically, aTSA is revised to a reversed prosthesis, but in some cases the anatomic configuration is maintained. Revision surgery is associated with a higher risk of complications and re-revisions than primary surgery. However, the literature on re-revision surgery after aTSA i

Commercially manufactured spacers for the treatment of periprosthetic joint infection of the hip.

BACKGROUND: Periprosthetic joint infection remains a common and serious complication after hip arthroplasty. To improve function and patient comfort after joint removal in two-stage revision, commercially manufactured spacers for the hip joint allow retention of the anatomical joint geometry thereby limiting soft tissue contraction and allow mobilization. INDICATIONS: Periprosthetic joint infection of the hip, septic arthritis with severe destruction of the hip cartilage and/or bone requiring arthroplasty. CONTRAINDICATIONS: Allergies to polymethylmethacrylate (PMMA) or antibiotics, severe h

Microbiologic epidemiology depending on time to occurrence of prosthetic joint infection: a prospective cohort study.

OBJECTIVES: The high microbiologic diversity encountered in prosthetic joint infection (PJI) makes the choice of empirical antimicrobial therapies challenging, especially in cases of implant retention or one-stage exchange. Despite the risk of dysbiosis and toxicity, the combination of vancomycin with a broad-spectrum β-lactam is currently recommended in all cases, even if Gram-negative bacilli (GNB) might be less represented in late PJI. In this context, this study aimed to describe the microbiologic epidemiology of PJI according to the chronology of infection. METHODS: This prospective coho
