BACKGROUND: Prosthetic valve endocarditis presents unique challenges for both diagnosis and treatment. A potential role of biofilm has been hypothesized in the pathogenesis of these infections.
METHODS: A patient with infective endocarditis involving a stentless (Freestyle) porcine prosthetic aortic valve with annular abscess and paravalvular leak 8 months after implantation is reported.
RESULTS: The infected valve did not show vegetations or perforations, but histiocytic inflammation was seen along the endocardial surfaces of the valve. Auramine-rhodamine staining revealed many acid-fast or

Stenotrophomonas maltophilia (formerly Xanthomonas maltophilia) is a gram-negative bacillus increasingly associated with serious nosocomial infections. Here, the case of a 69-year-old female patient who developed prosthetic valve endocarditis associated with this organism is described. A review of the literature revealed only 18 previous reports; eight involved native valves, the remainder prosthetic valves. Most cases were associated with risk factors, including intravenous drug abuse (6 patients), infected intravenous lines (4 patients) or a recent invasive procedure (3 patients). The course

BACKGROUND: Obesity has been suggested to be a potential risk factor for revision surgery following unicompartmental knee arthroplasty (UKA), however the orthopaedic literature is currently inconclusive.
METHODS: Data from the Australian Orthopaedic Association National Joint Replacement Registry (AOANJRR) were analysed for the period from 2015 to 2022. All primary UKA procedures performed for any diagnosis were identified and categorized into two patient groups: obese patients (BMI ≥ 30 kg/m2) and non-obese patients (BMI < 30 kg/m2). There were 26,472 primary UKA procedures included, of whic

OBJECTIVE: Biological augmentation and stabilization of high-grade bone defects with structural allografts from donor femur halfs.
INDICATIONS: Severe bone defects with aseptic loosening of hip prosthesis, periprosthetic femoral fracture or non-union, possibly even in cases of a healed infection.
CONTRAINDICATIONS: Local or systemic infection.
SURGICAL TECHNIQUE: The two modeled strut allografts are temporary fixed epiperiostal anterolateral and -medial with wire cerclages, while protecting the vascular supplying linea aspera of the femur. With the thus stabilized femur, the leg can be plac

BACKGROUND: The risk of periprosthetic joint infection (PJI), a serious complication after arthroplasty, has not changed for years. Interventions such as eradication of Staphylococcus aureus and antibiotic bone cement are used to diminish infection risk but despite these efforts, the percentage of infection in TKA remains constant. Antimicrobial drapes have a dual action, acting both as a physical and antimicrobial barrier to counter bacterial contamination of the surgical wound. To study the effect of antimicrobial drapes, we used intraoperative contamination as a proxy for infection in our i

Pneumoccal infection of a total knee replacement is rare. We present a case of hematogeous pneumoccocal infection of a revision total knee arthroplasty as a complication of disseminated pneumoccocal sepsis. The patient was successfully treated with two stage reimplantation in addition to 8 weeks of intravenous moxifloxacin and penicillin, and remains infection free and functions well over 3 years postoperatively.

BACKGROUND: The most frequently occurring adverse events in individuals with a transfemoral amputation treated with a bone-anchored prosthesis are soft tissue infections and stoma-related complications. These soft tissue complications are believed to be influenced by surgical technique and implant design, but little is known about the effect of changes to treatment on these events.
QUESTIONS/PURPOSES: (1) What is the result of surgical technique and implant modifications on the incidence of soft tissue infections and stoma-related complications in transfemoral bone-anchored prosthesis users,

Background and purpose - Prosthetic-joint infection (PJI) is the most serious complication of arthroplasty, and accurate identification of a potentially responsible microorganism is essential for successful antibiotic treatment. We therefore determined the diagnostic accuracy of sonication and compared it with tissue culture as a screening tool in detecting prosthetic joint infection in revision arthroplasty. Patients and methods - 252 consecutive revision arthroplasty cases were enrolled. These cases were determined as being suspected or unsuspected of having infection according to standard c

BACKGROUND: Chronic periprosthetic joint infection (PJI) after total knee arthroplasty (TKA) is most commonly addressed with a 2-stage exchange procedure. The purpose of this study is to examine the natural history of patients who have undergone prosthesis removal and spacer placement and evaluate risk factors for outcomes other than reimplantation.
METHODS: Patients who underwent removal of an infected TKA and placement of an antibiotic spacer for PJI were identified in a Medicare database. Patients with a study outcome within 1 year were then identified: (1) in hospital mortality, (2) knee

In patients with congenital heart diseases, new procedures, such as transcatheter valve replacement, have been associated with a non-negligible incidence of infective endocarditis (IE): up to 4% patient-year IE incidence. Prosthetic IE after percutaneous tricuspid valve replacement (PTVR) has been scarcely reported. We report the first pediatric case of IE after percutaneous tricuspid Melody™ valve-in-valve implantation in a boy who was diagnosed with Eisenmenger syndrome, related to patent ductus arteriosus. This first pediatric case of late IE (4 years) after PTVR is added to eight previousl

BACKGROUND: Endocarditis is a rare complication of transcatheter aortic valve implantation (TAVI), with an estimated 1-year incidence of 0.50% [Circulation 2015]. However, ensuing consequences are often dire, and its surgical treatment poses unique problems, due to the frequent underlying frailty of TAVI recipients.
CASE REPORT: We report the case of an 84-year-old woman, who developed Staphylococcus aureus prosthetic valve endocarditis (PVE) 7 days after transfemoral TAVI (ACURATE neo™, Boston Scientific or Symetis, further complicated by an aortic annular abscess with fistulization into the

The field of prosthetic urology demonstrates the striking impact that simple devices can have on quality of life. Penile prosthesis and artificial urinary sphincter implantation are the cornerstone procedures on which this specialty focuses. Modern research largely concentrates on decreasing the rates of complication and infection, as the current devices offer superior rates of satisfaction when revision is not necessary. These techniques are also able to salvage sexual function and continence in more difficult patient populations including female-to-male transgender individuals, those with is

BACKGROUND: For almost 30 years, bone-anchored prostheses have offered an alternative solution to prosthetic sockets by attaching the artificial limb directly to the femoral residuum by means of an osseointegration implant. Osseointegration implant surgery was introduced in our center in 2009. The aim of the present study is to report on safety, prosthesis-wearing time, and health-related quality-of-life (HRQoL) for patients with femoral bone-anchored prostheses during a 5-year follow-up period.
METHODS: All patients who underwent implantation of a press-fit osseointegration implant between M

As treatment options for erectile dysfunction (ED) continue to expand, and with more attractive alternatives such as effective systemic treatment becoming available, the number of men presenting for treatment of ED is increasing exponentially. Since a subset of these men continue to require surgical therapy, there is a potential for the number of operations for the treatment of ED and related disorders to actually increase. Areas in the surgical treatment of ED where improvements are needed are identified, including: measures to prevent penile prosthesis infections, better penile implants, imp

INTRODUCTION: Bordetella holmesii is a Gram-negative coccobacillus involved in different infections mostly described in case reports. Prosthetic joint infections in relation to this pathogen are rare. Here, we present the third case of B. holmesii in a patient without anatomical or functional spleen dysfunction.
CASE REPORT: The patient was a 62-year-old female with a total knee prosthesis implanted in 1997 that required multiple replacements of the femoral component due to aseptic loosening in the past years. The patient was admitted to our hospital for an elective replacement surgery due to

INTRODUCTION: Rheumatoid arthritis (RA) is one of the major indications of total hip (THA) or knee (TKA) arthroplasty. International studies have suggested that RA is a risk factor for prosthesis infections.
OBJECTIVES: To compare patients with RA and patients with osteoarthritis (OA) of other etiologies with regard to the incidence of prosthesis, incisional, and other systemic postoperative infections in THA and TKA.
METHODS: Retrospective, comparative cohort of patients followed up after undergoing THA or TKA at the Hospital SARAH-Brasília, from 1996 to 2007.
RESULTS: Seventy-five arthrop
