Rethinking Goods and Services Tax (GST) and Academic Research in India.

The Goods and Services Tax (GST), introduced in India on July 1, 2017, was envisioned as a landmark reform to unify indirect taxation, enhance transparency, and reinforce fiscal federalism. While celebrated for its efficiency, its uniform application to research‑related activities has generated significant concern across academia, medicine, and public health. Conference registration fees and continuing medical education (CME) costs have risen sharply, restricting opportunities for skill development among doctoral students, postdoctoral fellows, clinicians, and public health professionals. Advancing Hepatitis C Elimination in Africa: Insights from Egypt.

The hepatitis C virus (HCV) poses a significant risk to global public health and is linked to life-threatening clinical outcomes. According to the WHO, there are an estimated 58 million people worldwide who have a chronic hepatitis C virus (HCV) infection; there are 1.5 million new cases and more than 350,000 fatalities from HCV-related illnesses each year. Even though there are numerous diagnostic techniques, the lack of funding, inadequate healthcare infrastructure, and low public awareness of the Hepatitis C virus can make diagnosis and treatment difficult to obtain throughout the continent

Early Check: The First 5 Years.

Evidence of net benefit and feasibility of implementation is necessary for a health condition to become part of routine newborn screening. But acquiring data to support informed decision-making is difficult without a nimble and coordinated research capacity grounded in public health practice, a problem exacerbated in the case of rare disorders. To address this concern, we built and implemented Early Check to test the feasibility of a research resource that could adapt to changing needs for data. During the first 5 years, we (1) built a partnership among an independent scientific research insti

Environmental health during extreme heat: challenges for the practice of safe physical activity in childhood and adolescence.

This essay discusses evidence and evidence-based strategies to ensure the safety, well-being, and continuity of physical activity for children and adolescents during extreme heat. It emphasizes the adaptation of environments, professional training, and equity in public policies. High temperatures and sedentary behaviors are not isolated challenges, and they create a critical scenario for the health of children and adolescents when combined, with impacts ranging from thermal regulation to mental health, motor behavior, and risk of obesity. The increasing frequency of extreme heatwaves imposes a

Intracellular Parasitic Infections Caused by Plasmodium falciparum, Leishmania spp., Toxoplasma gondii, Echinococcus multilocularis, Among Key Pathogens: Global Burden, Transmission Dynamics, and Vaccine Advances-A Narrative Review with Contextual Insights from Armenia.

Intracellular parasitic infections continue to pose significant public health and veterinary challenges globally, driven by their ability to evade immune responses, persist within host cells, and spread through complex transmission pathways. Caused by a diverse array of protozoan, helminthic, and arthropod-borne parasites, these infections, such as toxoplasmosis, leishmaniasis, and tick-borne diseases, remain prevalent across many regions, often exacerbated by environmental, socio-economic, and ecological factors. This review explores the current knowledge on intracellular parasitic diseases,

Perceived barriers and facilitators of implementing a sustained smartphone-based telemonitoring program for pregnant women at high-risk for pre-eclampsia in the public and private sectors in Pakistan.

BACKGROUND: In Pakistan, a smartphone-based telemonitoring (TM) program (Raabta) has been designed to support pregnant women with high risk for preeclampsia (HRPE) in Pakistan. However, implementing TM interventions is often challenging, particularly in low-resource settings, given the complexity of healthcare environments and variations in public and private health sectors. This study explores the potential barriers and facilitators for a sustained implementation of the Raabta program in public and private sector hospitals in Pakistan. METHODS: Using a qualitative description design, 57 semi

A topography-sensitive 2SFCA approach to measuring accessibility to active living infrastructure and its association with health-enhancing physical activity.

INTRODUCTION: Pedestrian accessibility to neighborhood infrastructure is associated with Physical Activity (PA), yet the built environment is often evaluated as separate facility types, while accessibility measures may not jointly capture population demand and topographic walking burden. This study therefore examined PA in relation to a composite Active Living Infrastructure (ALI) measure that integrates five facility types and applies a Topography-Sensitive Two-Step Floating Catchment Area (T-S2SFCA) model, which weighs facility supply against population demand while adjusting walking distanc

Addressing women's health needs and improving birth outcomes: results from a peer-to-peer state Medicaid learning project.

High rates of maternal mortality, infant mortality, and preterm births, as well as continuing disparities in pregnancy outcomes, have prompted a number of state Medicaid agencies to focus on improving the quality and continuity of care delivered to women of childbearing age. As part of a peer-to-peer learning project, seven Medicaid agencies worked to develop the programs, policies, and infrastructures needed to identify and reduce women's health risks either prior to or between pregnancies. The states also identified public health strategies. These strategies led to a policy checklist to help

An assessment of the telehealth needs and health-care priorities of Tanna Island: a remote, under-served and vulnerable population.

We surveyed eight Canadian physicians who had each provided medical care for six months on the remote and under-served island of Tanna in Vanuatu. The most frequently encountered medical problems on Tanna were infectious diseases (tuberculosis, hepatitis, abscesses, malaria, pneumonia, typhoid fever, meningitis and skin infections). When physicians were asked about the top three health-care priorities, they ranked tuberculosis control, clean water and improved health-care delivery/communication between hospital and outposts as most important. The key issues were: (1) basic public health needs

The intersection of sickle cell disease, stigma, and pain in Africa.

Sickle cell disease (SCD) is a significant public health concern in sub-Saharan Africa, where it is the most prevalent genetic disorder, presenting numerous health care and sociocultural challenges. A case study of a young girl from Ghana's Ashanti region illustrates the stigma surrounding SCD, driven by traditional beliefs and misconceptions that perceive SCD as a spiritual affliction. This stigma results in social ostracism and discrimination, impacting affected individuals and their families. Despite the severe and unpredictable pain associated with SCD, effective management is often hamper
