Combined emphysema and cancer, from radiology to prognosis in patients treated by immunotherapy: An ancillary study of a prospective multicenter phase II IFCT-1802 SAVIMMUNE.

INTRODUCTION: In studies evaluating the efficacy of anti-programmed cell death (ligand)1 (anti-PD-(L)1) in patients with non-small-cell lung cancer (NSCLC), smokers tend to have better clinical outcomes than non-smokers. However, whether co-existing emphysema is associated with differences in clinical outcomes, regardless of smoking status remains unclear. METHODS: In a single-arm, prospective, multicenter, phase II trial, patients aged 18-75 years with metastatic NSCLC, PD-L1≥25% and a performance status (PS) of 2-3 received durvalumab until progression or toxicity. Pre-planned analyses clas

Paraseptal Emphysema and Interstitial Lung Abnormalities in AGES-Reykjavik Study.

BACKGROUND: Paraseptal emphysema (PSE) is often underrecognized because it has minimal impact on pulmonary function. Recent studies suggest its frequent coexistence with interstitial lung abnormalities (ILA) and potential clinical significance, yet population-based data on its prevalence and prognostic impact remain unclear. PURPOSE: To examine the associations between visual emphysema subtypes and ILA, and to evaluate the prognostic impact of ILA in individuals with emphysema subtypes. MATERIALS AND METHODS: We analyzed 5,059 participants from Age Gene/Environment Susceptibility Reykjavik S

[Lung emphysema].

1. Elastic recoil, size of the perfused surface for gas exchange and flow resistance of the small airways influence the symptomatology and lung function of pulmonary emphysema independently of each other. 2. The loss of elastic force and surface for gas exchange are predominant in panlobular emphysema. Ventilation-perfusion disturbances due to increased airway resistance during inspiration and expiration predominate in chronic obstructive bronchitis with centrilobular emphysema. 3. In the presence of identical values for vital capacity and FEV1.0, arterial blood gas studies during rest and exe…

Prevalence and Effects of Emphysema in Never-Smokers with Rheumatoid Arthritis Interstitial Lung Disease.

AIMS: Autoimmune conditions such as rheumatoid arthritis-related interstitial lung disease (RA-ILD) have been linked to the existence of emphysema in never-smokers. We aimed to quantify emphysema prevalence in RA-ILD never-smokers and investigate whether combined pulmonary fibrosis and emphysema (CPFE) results in a worsened prognosis independent of baseline disease extent. METHODS: RA-ILD patients presenting to the Royal Brompton Hospital (n=90) and Asan Medical Center (n=155) had CT's evaluated for a definite usual interstitial pneumonia (UIP) pattern, and visual extents of emphysema and ILD

Clinical correlations of computed tomography imaging in chronic obstructive pulmonary disease.

Chronic obstructive pulmonary disease is a complex disease with heterogeneous presentation, progression, and structural abnormality that is currently graded solely on clinical and physiologic parameters. Thoracic computed tomography imaging holds promise for phenotyping in chronic obstructive pulmonary disease, but despite increasing availability, this methodology has yet to be incorporated into clinical guidelines or routine clinical practice. However, the unique clinical implications of emphysema and airways disease are becoming clearer. Emphysema has a strong association with more rapid dis

Congenital Lobar Emphysema in Children: Case Series.

Recurrent wheezing is very common in infants. When these symptoms appear early without a free interval, a pulmonary malformation should be investigated. Congenital lobar emphysema is a rare abnormality of the lower respiratory tract. Here, we report a case series of six cases of congenital lobar emphysema between 2015 and 2023. Clinical and radiological data were collected according to an operating sheet previously established in our pediatric pneumo-allergology unit. They all had recurrent wheezing and dyspnea. Chest radiography and chest CT were consistent with the diagnosis of congenital lo

Pneumomediastinum and subcutaneous emphysema secondary to amyopathic dermatomyositis with cryptogenic organizing pneumonia in invasive breast cancer: a case report and review of literature.

Amyopathic dermatomyositis (ADM) is recognized as a variant phenotype of dermatomyositis and characterized by typical skin manifestations without evidence of muscular inflammation. While interstitial lung disease (ILD) is occasionally found as one of the lung manifestations in ADM patients, the development of a pneumomediastinum and/or subcutaneous emphysema in this disease entity is one of the extremely rare pulmonary complications. These latter complicated pulmonary manifestations have been usually reported in idiopathic ADM with ILD without any other associated medical conditions. We report

Smoking and pulmonary fibrosis: novel insights.

The relationship between smoking and pulmonary fibrosis is under debate and intense investigation. The aim of this paper is to review the existing literature and identify further areas of research interest. Recently the negative influence of cigarette smoking on IPF outcome was highlighted, as non-smokers exhibit a better survival than ex-smokers and combined current- and ex-smokers. In patients with non-specific interstitial pneumonia (NSIP), a high prevalence of emphysema was recently demonstrated, providing an indirect support for a smoking pathogenetic hypothesis in NSIP. The coexistence o

Integrated Bioinformatics and Experimental Validation to Identify Shared Sialylation-Related Signatures in Emphysematous and Fibrotic Lung Disease.

Emphysema and pulmonary fibrosis represent distinct patterns of chronic lung remodeling but may share molecular and cellular abnormalities. In this study, we analyzed public transcriptomic datasets of chronic obstructive pulmonary disease, emphysema, and idiopathic pulmonary fibrosis from the Gene Expression Omnibus database to explore shared disease-related signatures. Weighted gene co-expression network analysis and differential expression analysis were used to identify common molecular alterations, followed by functional enrichment, protein-protein interaction network construction, hub gene

An atypical case of congenital lobar emphysema in an adult, non-smoker patient presenting with pneumothorax.

Congenital lobar emphysema (CLE) is a developmental anomaly of the lower respiratory tract characterized by hyperinflation of one or more pulmonary lobes in the absence of extrinsic bronchial obstruction. We present a case of a 24-year-old male, nonsmoker who presented with shortness of breath and severe left sided chest pain. A chest x-ray was significant for a very large left-sided pneumothorax and chest CT showed lobar emphysematous changes. Video assisted thoracoscopic surgery (VATS) and lobectomy was subsequently performed after persistence of pneumothorax despite chest tube insertion and

[Clinical Case: Micro-drainage of subcutaneous emphysema Grade V through a fenestrated peripheral venous catheter in critical ill].

BACKGROUND: Approximately 30% of surgical procedures in the United States require orotracheal intubation. Generally laryngeal and tracheal injuries are rare. However, when they do occur, they can lead to major complications. Subcutaneous emphysema is the most common clinical presentation and is mostly treated conservatively and self-limiting. However, when it is Grade V, control strategies should be considered. There are reports in the literature on micro-drainage of subcutaneous emphysema through a fenestrated peripheral venous catheter with satisfactory results. CLINICAL CASE: We present th

Pulmonary hyperinflation due to gas trapping and pulmonary artery size: The MESA COPD Study.

BACKGROUND: Pulmonary hypertension is associated with increased morbidity and mortality in chronic obstructive pulmonary disease (COPD). Since pulmonary artery (PA) size increases in pulmonary hypertension, we measured PA cross-sectional area using magnetic resonance imaging (MRI) to test the hypothesis that pulmonary hyperinflation due to gas trapping is associated with PA cross-sectional area in COPD. METHODS: The MESA COPD Study recruited participants with COPD and controls from two population-based cohort studies ages 50-79 years with 10 or more pack-years and free of clinical cardiovascu

Surgical Approaches to Treating Emphysema: Lung Volume Reduction Surgery, Bullectomy, and Lung Transplantation.

Chronic obstructive pulmonary disease (COPD) is a common and morbid progressive disease where treatment is focused on improving dyspnea, reducing exacerbations, attenuating comorbidities, and improving quality of life. Surgical therapy can be beneficial to a carefully selected subset of individuals and is the subject of this review. The National Emphysema Treatment Trial (NETT) has not only demonstrated the efficacy of lung volume reduction surgery (LVRS) but has also provided many lessons regarding advanced emphysema. NETT demonstrated that LVRS improves exercise performance, quality of life,

Long-term risk of emphysema in patients with farmer's lung and matched control farmers.

We evaluated the long-term outcome of farmer's lung (FL) patients and matched control farmers using high-resolution computed tomography (HRCT). The study population consisted of 88 FL patients and 83 control farmers, matched by age, sex, and smoking habits. The mean time after the first diagnosed episode of FL was 14 yr. The great majority, 82%, of the studied subjects were nonsmokers. Clinical studies included HRCT, spirometry, and pulmonary diffusing capacity. Emphysema was found significantly more often (23%) in FL patients than in control farmers (7%) (p = 0.006). The presence of emphysema

Functional Benefits of Bilateral Emphysema Reduction Surgery in an Adolescent.

Acquired emphysema is a rare pathology in pediatrics. We report the case of a patient born at term with a neonatal respiratory distress, which had required mechanical ventilation. She developed gradually chronic obstructive pulmonary disease with severe emphysematous lesions, respiratory failure and functional impairments. Bilateral emphysema resection, performed at 16 years old, allowed major functional benefits at rest and during exercise. We present the results of respiratory functional evaluations, walk tests and maximal exercise tests (including measure of dynamic hyperinflation) before a

Stronger Associations of Centrilobular Than Paraseptal Emphysema With Longitudinal Changes in Diffusing Capacity and Mortality in COPD.

BACKGROUND: The factors associated with longitudinal changes in diffusing capacity remain unclear among patients with COPD. Centrilobular emphysema (CLE) and paraseptal emphysema (PSE) are major emphysema subtypes that may have distinct clinical-physiological impacts in these patients. RESEARCH QUESTION: Are CLE and PSE differently associated with longitudinal changes in diffusing capacity and mortality in patients with COPD? STUDY DESIGN AND METHODS: This pooled analysis included 399 patients with COPD from two prospective observational COPD cohorts. CLE and PSE were visually assessed on CT

A Robust Emphysema Severity Measure Based on Disease Subtypes.

RATIONALE AND OBJECTIVES: We propose a novel single index for the quantification of emphysema severity based on an aggregation of multiple computed tomographic features evident in the lung parenchyma of smokers. Our goal was to demonstrate that this single index provides complementary information to the current standard measure of emphysema, percent emphysema (percent low attenuation areas [LAA%]), and may be superior in its association with clinically relevant outcomes. MATERIALS AND METHODS: The inputs to our algorithm were objective assessments of multiple emphysema subtypes (normal tissue

Farmer's lung. Long-term outcome and lack of predictive value of bronchoalveolar lavage fibrosing factors.

This study evaluates the long-term outcome of farmer's lung (FL), adding high-resolution computed tomograms (HRCT) to previously reported procedures and verifying whether bronchoalveolar lavage (BAL) fluid markers or substrates of fibrosis (hyaluronic acid, Type III procollagen, fibronectin, and fibroblast growth factors) (FF) predict outcome. A total of 33 subjects with a history of FL dating back at least 6 yr were evaluated with pulmonary function tests, chest x-ray (CXR), and HRCT. All subjects had an initial evaluation, which included a BAL, 6 yr before the current study. Subjects were th

[Asymptomatic mediastinal and neck emphysema after Kussmaul's respiration and purging].

This is a case report of a young woman with diabetes mellitus type 1. She was admitted with severe diabetic ketoacidosis. Asymptomatic "surgical emphysema", pneumomediastinum and bilateral pneumothoraces were accidently discovered. The emphysema had probably occurred due to laboured breathing and groaning during her diabetic ketoacidosis. No treatment was needed.

Subconjunctival emphysema as a complication of PEEP.

This report describes the unusual case of a four-year-old girl who developed subconjunctival emphysema as a complication of mechanical ventilatory support with high positive expiratory pressure (PEEP)--which was applied after she sustained multiple injuries in a motor vehicle accident. The patient's left lower palpebral conjunctiva ballooned outward with air collection subconjunctivally, but it was crepitant and deflatable with light pressure. Saline, garamycin ointment, and Lacrilube were used to prevent drying. The condition persisted during the period when the patient required high PEEP (mo
