The main clinical manifestations of 42 cases of Q fever occurring in an outbreak in Valmaseda, Spain during March, April and May 1982 are described in this paper. The benign character of the acute disease is emphasized. Further epidemiologic studies must be done to determine the incidence of Q fever in the Basque country.

There are very few reports in the literature on mass seroepidemiological studies of the prevalence of Q fever antibodies. Those to be found are mostly concerned with focal epidemics of Q fever. Out of more than 2000 serum specimens from blood donors analyzed at random in Switzerland in 1982 for Q fever antibodies, 3.5% gave seropositive results (CBR titer at least 1:10). The virus isolated by Derrick from cases of fever produces characteristic pathogenic effects on monkeys and mice. A well-defined febrile reaction, during which the blood is infective for guinea-pigs, follows subcutaneous inoculation of virus in the monkey. Mice inoculated intraperitoneally show enlargement of spleen and liver with characteristic histological changes. In sections and smears of infected mouse liver and spleen large numbers of rickettsial organisms are visible. These occur in relatively large intracytoplasmic microcolonies. A patient with Q fever endocarditis, which is almost unknown in the United States, was followed for a total of 32 months; the study was begun 3 1/2 months before aortic valve replacement. Diagnosis was confirmed by serology, visualization of Coxiella burnetii in excised aortic valve tissue by direct and immunofluorescence staining, and isolation of C. burnetii from aortic valve tissue. Seventy-eight British soldiers stationed in the Eastern Sovereign Base Area (ESBA) in Cyprus contracted Q fever in the period December 1974 to June 1975. Pneumonia developed in 59% of cases. Of 31 patients tested, 81% had biochemical evidence of hepatitis although only one became clinically jaundiced. Three patients (4%) suffered pericarditis. Treatment with tetracycline had no apparent effect on the course of the disease.
