BACKGROUND: Pneumoperitoneum during laparoscopic cholecystectomy (LC) can cause hypercapnia, hypoxemia, hemodynamic changes and shoulder pain. General anesthesia (GA) enables the control of intraoperative pain and ventilation. The need for GA has been questioned by studies suggesting that neuraxial anesthesia (NA) is adequate for LC.
STUDY OBJECTIVE: To quantify the prevalence of intraoperative pain and to verify whether evidence on the maintenance of ventilation, circulation and surgical anesthesia during NA compared with GA is consistent.

Pressure threshold is the minimal pressure (force) which induces pain. The pressure threshold meter (PTM) is a force gauge with a rubber disc of 1 cm2 surface. The instrument has been proven to be useful in clinical practice for quantification of deep muscle tenderness. BACKGROUND: Cholelithiasis typically presents with right upper quadrant pain, as can pain from other right upper quadrant organs. Pain of cholelithiasis is often referred to tip of scapula. Professor Paddy Collins drew attention to fact that patients with gallstone pain would attempt to demonstrate this by placing their hand behind the back and thumb pointing upwards. BACKGROUND: Heterotopic pregnancy is a rare event that usually presents in the first 6-8 weeks of pregnancy.
CASE: A 35-year-old primigravida was referred to the hospital at 18+ weeks' gestational age for evaluation of chronic abdominal pain. Ultrasound revealed a heterotopic pregnancy. The patient underwent surgery for removal of the ectopic pregnancy.
CONCLUSION: Heterotopic pregnancy can present at any time during pregnancy and should be considered in any pregnant woman with abdominal pain.

A 21-year-old female patient with chronic abdominal pain was referred to a psychiatric outpatient clinic after gastroenterological and gynaecological pathogeneses had been excluded and a treatment with an antidepressant had had no beneficial effects. The mental state examination, however, revealed no psychopathology whatsoever. The patient was injected with a local anaesthetic loco dolenti which resulted in immediate pain relief. She was diagnosed with abdominal cutaneous nerve entrapment syndrome (ACNES); no psychiatric diagnosis was given.
