Increased prevalence of "Retinal Ischemic Perivascular Lesions" in retinal artery occlusion: A silent vascular biomarker in the fellow eye.

PURPOSE: To determine the prevalence of retinal ischemic perivascular lesions (RIPLs) in the unaffected fellow eyes of patients with retinal artery occlusion (RAO) and to evaluate their association with systemic vascular risk factors. METHODS: A total of 80 eyes were included: 40 unaffected fellow eyes of patients diagnosed with RAO and 40 eyes of age-matched healthy control subjects. All participants underwent optical coherence tomography angiography imaging. RIPLs were identified and quantified in the fellow eyes of RAO patients and in control eyes. Quantitative vascular density analysis of…

Systemic Inflammation and Structural Retinal Damage on OCT in Acute Non-Arteritic Central Retinal Artery Occlusion-A Retrospective Study.

Central retinal artery occlusion (CRAO) is an acute ophthalmic emergency causing irreversible vision loss due to retinal ischemia. Inflammation is increasingly recognized as a contributor to vascular occlusions, but the relationship between systemic inflammatory biomarkers and structural retinal damage in CRAO remains unclear. This retrospective study included 37 patients with non-arteritic CRAO presenting within 168 h of symptom onset. Optical coherence tomography (OCT) classified ischemic severity as mild, moderate, or severe based on inner retinal morphology. Systemic inflammatory indices, …

Establishment of the reproducible branch retinal artery occlusion mouse model and intravital longitudinal imaging of the retinal CX3CR1-GFP+ cells after spontaneous arterial recanalization.

Animal models of retinal artery occlusion (RAO) have been widely used in many studies. However, most of these studies prefer using a central retinal artery occlusion (CRAO) which is a typical global ischemia model of the retina, due to the technical limitation of producing single vessel targeted modeling with real-time imaging. A focal ischemia model, such as branch retinal artery occlusion (BRAO), is also needed for explaining interactions, including the immunological reaction between the ischemic retina and adjacent healthy retina. Accordingly, a relevant model for clinical RAO patients has …

Bilateral internal carotid artery and vertebral artery dissections with retinal artery occlusion after a roller coaster ride - case report and a review.

We present the first case of a woman with no significant medical history who developed dissections of bilateral carotid and bilateral vertebral arteries, as well as a retinal artery occlusion, after a roller coaster ride. A 35-year-old woman developed right-sided neck pain followed by a frontal headache immediately after a roller coaster ride. Five days after the incident, she developed complete loss of vision in her right eye for two hours. Subsequently, the vision improved but remained significantly decreased. On presentation, her visual acuity was 20/200 in the right and 20/20 in the left e…

Branch retinal artery occlusion (BRAO) combined with branch retinal vein occlusion (BRVO) and optic disc neovascularization associated with HIV and CMV retinitis.

Two vaso-occlusive events, branch retinal artery occlusion (BRAO) and branch retinal vein occlusion (BRVO), were observed in the retina of an HIV-infected patient with cytomegalovirus (CMV) retinitis who developed neovascularization of the disc (NVD). Although BRVO and reversible NVD have been reported in association with CMV retinitis, we have seen no reports of concomitant BRAO. CMV damages endothelial cells and causes an occlusive vasculitis. In HIV-infected individuals, damaged endothelial cells and rheologic problems result in increased blood viscosity. HIV infection has also been associa…

Optic disk and retinal nerve fiber layer damage after transient central retinal artery occlusion: an experimental study in rhesus monkeys.

PURPOSE: To evaluate the retinal tolerance time to acute ischemic insult in middle-aged or elderly rhesus monkeys with pre-existing atherosclerosis and arterial hypertension. METHODS: In 39 eyes of 39 middle-aged and elderly rhesus monkeys with a mean age of 19.5 +/- 2. 8 years, occlusion of the central retinal artery was produced by temporary clamping of the central retinal artery at its site of entry into the dural sheath of the optic nerve for 97 to 300 minutes. Stereoscopic color fundus photography and fluorescein fundus angiography were performed before central retinal artery occlusion a…

Internal Carotid Artery Ligation: A Murine Model for Acute Retinal Ischemic Injury.

Accumulating evidence indicates that retinal ischemia constitutes a pivotal pathological trigger of visual impairment. The cerebral arterial circle (circle of Willis), formed by the vertebral and internal carotid arteries, provides cerebral blood perfusion. As a direct branch originating from the internal carotid artery (ICA), the ophthalmic artery supplies blood to the ocular globe. Based on the anatomical features of the ocular vasculature, a retinal ischemic injury model was successfully generated in C57BL/6 mice via selective left ICA ligation. Morphological and vascular analyses demonstra

Central Retinal Artery Occlusion Following Intradialytic Hypotension in End-Stage Renal Disease.

Central retinal artery occlusion (CRAO) is a vision-threatening ophthalmic emergency requiring rapid recognition and evaluation. Although embolic etiologies predominate, low-flow ischemia related to hemodynamic instability represents an underrecognized mechanism. We report the case of a 66-year-old woman with end-stage renal disease (ESRD) on hemodialysis who experienced recurrent transient right-eye visual symptoms near the end of dialysis sessions. She had chronic intradialytic hypotension, with systolic blood pressures ranging from 90 to 100 mmHg and post-dialysis pressures averaging 90/50 …

RETINAL ISCHEMIC PERIVASCULAR LESIONS ARE ASSOCIATED WITH INCREASED CAROTID ARTERY STENOTIC DEGREE.

PURPOSE: Retinal ischemic perivascular lesions (RIPLs) have been reported as potential biomarkers for cardiovascular diseases and stroke. The authors aimed to investigate the RIPLs in patients with carotid artery stenosis (CAS) and their association with stenotic degree. METHODS: Patients with unilateral CAS or carotid artery occlusion were recruited. Optical coherence tomography was conducted for assessing the presence, number, and distribution of RIPLs. Optical coherence tomography angiography was used for evaluating microvascular density in superficial vascular complex and deep vascular co

Unilateral Central Retinal Artery Occlusion Revealing Giant Cell Arteritis: A Case Report.

Giant cell arteritis (GCA), also known as temporal arteritis, is a granulomatous vasculitis that affects large and medium-sized blood vessels. While the exact etiology remains unclear, the inflammation predominantly involves the aorta and its branches, particularly the orbital arteries. Among these, the posterior ciliary arteries (PCAs), which supply the choroid, optic nerve head, and cilioretinal artery, are especially prone to involvement. Ocular manifestations of GCA occur frequently and may result in significant visual loss. Ophthalmic conditions associated with GCA include arteritic anter…

Central Retinal Artery Occlusion After Intravitreal Injection of Faricimab: A Case Report.

Faricimab is a relatively novel agent as a bispecific antibody that inhibits both vascular endothelial growth factor-A and angiopoietin-2. Although it's effective for several retinal diseases, several rare adverse events, such as retinal vasculitis, have been reported. We present a case of central retinal artery occlusion (CRAO) following intravitreal injection of faricimab in a 74-year-old woman treated for central retinal vein occlusion. A sudden decrease in visual acuity occurred 24 hours after the second injection. Her best-corrected visual acuity was counting fingers at 30 cm. No anterior…

Retinal artery/arteriole occlusion risks after endovascular treatment for unruptured intracranial aneurysm.

BACKGROUND: To evaluate the association between retinal artery/arteriole occlusion (RAO) and unruptured intracranial aneurysm (UIA). METHODS: Incident UIA patients from a nationwide cohort (n=253 240) were categorised into three groups based on subsequent treatment: observation (n=208 993), microsurgical clipping (n=14 168) and endovascular treatment (EVT) groups (n=30 079). The incidence and the incident time of RAO were analysed. HRs of RAO and associated risk factors were evaluated. Additionally, a hospital cohort comprising 2569 consecutive UIA patients treated at a tertiary hospital was …

A Triggering Event of Central Retinal Artery Occlusion With Concurrent Ischemic Stroke.

We report a case of central retinal artery occlusion with concurrent ischemic stroke in a young patient. A 34-year-old Malay gentleman, an ex-smoker with underlying dyslipidemia, however, not on medication or follow-up, presented with acute, generalized, and painless right eye blurring of vision for one day. He also complained of on-and-off headaches for the past three months prior to the presentation. Visual acuity assessment demonstrated hand movement in the right eye, whereas in the left eye, it was 6/6, along with a right eye relative afferent pupillary defect. His right eye showed reduced…
