Guidance for the referral of children and young people for consideration of neurosurgical interventions in the management of posture and tone (NIMPTs): a UK-wide consensus report.

OBJECTIVES: Guidance is currently lacking in the UK regarding which children and young people (CAYP) should be referred for consideration for neurosurgical interventions in the management of posture and tone (NIMPTs). We aimed to establish a consensus among centres providing these interventions (namely intrathecal baclofen (ITB), deep brain stimulation (DBS) and selective dorsal rhizotomy (SDR)) as to which CAYP should be referred for consideration of these interventions. METHOD: Centres in the UK offering NIMPTs were identified and contacted. Suggestions for referral criteria were requested …

A history of the Jannetta procedure.

Peter Jannetta was a neurosurgery resident when he proposed the neurovascular compression theory. He built upon the astute observations of Dandy, Gardner, and others who, in the era before the operating microscope, had successfully ventured into the posterior fossa. In 1965, Jannetta performed cranial nerve microdissections for dental students and identified the trigeminal portio intermedia. He proposed that preservation of these sensory fibers may avoid complete facial numbness, and together with Robert Rand developed a subtemporal transtentorial approach for selective rhizotomy for trigemina…

C2 nerve dysfunction associated with C1 lateral mass screw fixation.

The C1 lateral mass screw technique is widely used for atlantoaxial fixation. However, C2 nerve dysfunction may occur as a complication of this procedure, compromising the quality of life of affected patients. This is a review of the topic of C2 nerve dysfunction associated with C1 lateral mass screw fixation and related research developments. The C2 nerve root is located in the space bordered superiorly by the posterior arch of C1 , inferiorly by the C2 lamina, anteriorly by the lateral atlantoaxial joint capsule, and posteriorly by the anterior edge of the ligamentum flavum. Some surgeons su

Glossopharyngeal neuralgia, more accurately called vago-glossopharyngeal neuralgia (VGPN) because of the frequent association with pain irra … ar decompression, since vascular compression is the main cause of the neuralgia. Percutaneous thermorhizotomy at the foramen jugularis (pars nervosa) is only indicated as a second option, because of unavoidable sensorimotor deficits in the ninth and tenth nerves. The trigeminal vasculature pathology in patients with neuralgia.

OBJECTIVE: To examine the possible pathological changes of the trigeminal vasculature in patients with neuralgia. BACKGROUND: Such a study has never been performed before. The alterations of the trigeminal vessels could have important pathophysiological implications in the trigeminal neuralgia pathogenesis. METHODS: The biopsy specimens for the electronmicroscopic (EM) and immunohistochemical examination were taken during a partial rhizotomy in 6 patients with trigeminal neuralgia and in 2 persons with trigeminal neuropathy. In addition, the 32 normal trigeminal nerves were used as the contr…

Evaluation of surgical procedures for trigeminal neuralgia.

Trigeminal neuralgia is a type of facial pain that is difficult to treat. The pain can be excruciating and debilitating. The wide range of treatments currently used for trigeminal neuralgia is ample evidence that there is no simple answer to how it should be managed. This review will evaluate the current surgical procedures used for the treatment of trigeminal neuralgia. A critical analysis of the evidence-based studies to date was done to evaluate and compare the efficacy of the different surgical procedures. Arguments for and against the use of surgery for trigeminal neuralgia are presented.

Abducens nerve palsy after radiofrequency rhizolysis for trigeminal neuralgia: case report.

OBJECTIVE AND IMPORTANCE: Abducens nerve palsy is a rare and reversible complication associated with percutaneous radiofrequency trigeminal rhizolysis. CLINICAL PRESENTATION: An 86-year-old man developed an abducens nerve palsy immediately after undergoing percutaneous radiofrequency trigeminal rhizolysis for severe trigeminal neuralgia involving all three divisions of the trigeminal nerve. The palsy resolved spontaneously after 2 months. CONCLUSION: This case, in combination with previous reports, suggests that cases of transient sixth nerve palsy associated with percutaneous radiofrequency

Controversies in Trigeminal Neuralgia: A Review.

Trigeminal neuralgia (TN) is an area full of controversies. The anatomical distribution of various sensory fibers in the sensory root of the … trigeminal REZ, which carry the touch and proprioceptive fibers, paved the way for partial sensory rhizotomy by posterior fossa approach. Similarly, Jannetta's concept of microvascular compression at the REZ paved the way for the popular microvascular decompression surgery. The search for less risky, less invasive procedures has paved the way for the vario …

Laser speckle imaging to improve clinical outcomes for patients with trigeminal neuralgia undergoing radiofrequency thermocoagulation.

OBJECTIVE: Percutaneous treatments for trigeminal neuralgia are safe, simple, and effective for achieving good pain control. Procedural risks could be minimized by using noninvasive imaging techniques to improve the placement of the radiofrequency thermocoagulation probe into the trigeminal ganglion. Positioning of a probe is crucial to maximize pain relief and to minimize unwanted side effects, such as denervation in unaffected areas. This investigation examined the use of laser speckle imaging during probe placement in an animal model. METHODS: This preclinical safety study used nonhuman pr

Bilateral trigeminal neuralgia: a therapeutic dilemma.

In order to illustrate the inherent problems of managing bilateral trigeminal neuralgia a retrospective study of the 16 cases of bilateral trigeminal neuralgia, out of just over 300 cases of trigeminal neuralgia, treated over a 14-year period, has been performed. All the patients, presented with a typical history of trigeminal neuralgia and underwent surgical exploration. Pain relief was initially achieved in all cases; however, only four remained cured, three have become pain free after additional rhizotomy, a further one after peripheral cryotherapy and four with medical treatment. Four pati…

Surgery for the treatment of overactive bladder.

OBJECTIVES: To describe surgical options that may be indicated when conservative management of overactive bladder is unsuccessful. METHODS: The literature on current endoscopic and open surgical procedures is reviewed. RESULTS: The endoscopic approach of hydrodistention under anesthesia may be therapeutic, but normally offers only temporary relief of symptoms. Among open surgical procedures, cystolysis has not withstood the test of time and is no longer used. Partial rhizotomy can reduce reflex bladder contractility while preserving other important urogenital functions. Peripheral denervatio…

Percutaneous Surgical Approaches in Multiple Sclerosis-Related Trigeminal Neuralgia: A Systematic Review and Meta-analysis.

OBJECTIVE: The prevalence of trigeminal neuralgia (TN) in the setting of multiple sclerosis (MS) is reported to be 20-fold higher than in the general population. In these patients, TN can be medically refractory and often requires neurosurgical interventions. Ablative percutaneous approaches such as balloon compression (BC), glycerol rhizolysis (GR), and radiofrequency ablation (RF) can be used and often provide rapid pain relief. The aim of this meta-analysis was to compare the safety and efficacy profile of these approaches. METHODS: This study was performed according to the PRISMA guidelin

Identifying the trigeminal nerve branches for transovale radiofrequency thermolesion: "no pain, no stress".

BACKGROUND: Radiofrequency thermorhizotomy of the trigeminal nerve is a known treatment of trigeminal neuralgia. Analysis of verbal responses to electric stimulation of the trigeminal rootlets has been the only method available to localize the affected branch, but patient discomfort may lead to unreliable verbal responses, resulting in increased morbidity or even therapeutic failure. Orthodromically elicited evoked potentials of the trigeminal nerve have also been used, but their application is tedious and results may vary. OBJECTIVE: To develop an electrophysiological method for intraoperati…

Rhizolysis: an unusual postoperative complication.

Percutaneous retrogasserian glycerol rhizolysis, introduced in 1981, has become established in the management of trigeminal neuralgia secondary to multiple sclerosis and pontine infarction. More recently, this technique was established as safe and reliable in managing pain originating from the zygapophyseal joints, with successful relief of pain in 67% of patients treated for lower back pain. In this article, we report a case of complete paralysis of lumbar paravertebral muscle function caused by severe multifidus degeneration after 3-level lumbar rhizolysis. This paralysis resulted in kyphosi
