[Resistance of Leishmania infantum to Glucantime: risk factors and therapeutic management].

BACKGROUND: Resistance to antimonial drugs is rarely observed in immunocompetent patients. CASE REPORT: A 1-year-old girl was admitted suffering from persistent fever. A diagnosis of visceral leishmaniasis was made. The patient was given two courses of meglumine antimoniate (Glucantime) (60 mg/kg/d for 15 days) and one course of 12 injections of pentamidine (4 mg/kg). She relapsed 8 months later and failed to respond to Glucantime. Immunological tests performed during the relapse showed a suppression of the T cell response to Leishmania antigen and no production of interferon gamma. The patie

The nursing role in epidemiology, risk management, and patient-public education.

Epidemiology encompasses the identification of infectious disease, the vectors of transmission, the sources of contamination, the patient population exposed or involved, containment practices, and preventative measures to stop further transmission of the disease. Epidemic keratoconjunctivitis is a common infectious disease in ophthalmology. It can be contracted from interpersonal exposure, contact with contaminated items, or iatrogenic transmission from the health-care settings and providers. The interview process, plus patient education, is the method of epidemiologic assessment that acts as

Identifying patients with epilepsy at high risk of cardiac death: signs, risk factors and initial management of high risk of cardiac death.

People with epilepsy have a three-fold increased risk of dying prematurely, and a significant proportion is due to sudden cardiac death or acute myocardial infarctions. The causes of increased cardiovascular morbidity and mortality in epilepsy are manifold and include acute or remote effects of epileptic seizures, the longstanding epilepsy itself or antiseizure treatments. Seizure-related cardiac arrhythmias are common and comprise bradyarrhythmia and asystole, atrial fibrillation and ventricular tachycardia. The most frequent clinically relevant seizure-related arrhythmia is ictal asystole th

Kratom Toxicity: Pharmacologic Mechanisms, Clinical Risks, and Management Considerations.

Kratom (Mitragyna speciosa) is an opioid-like botanical increasingly used for self-management of pain and opioid withdrawal and has been increasingly implicated in emergency department presentations for acute toxicity. Despite its growing use, kratom remains poorly understood in clinical practice, particularly with respect to its complex, metabolite-dependent pharmacology. Product heterogeneity, variable alkaloid composition, and frequent polysubstance use further complicate clinical recognition and management, raising important patient safety concerns. This narrative review synthesizes the pu

When Mania Outruns the Law: Managing Risk in the Liminal Zone.

Patients with mania are often acutely unwell yet do not meet criteria for involuntary hospitalization. This creates a clinical and ethical dilemma the authors refer to as the liminal zone. These patients refuse treatment despite clear foreseeable harms, but the legal threshold for certification is not met. This situation produces moral discomfort for clinicians, disrupts team consensus, and is often equally distressing for families and friends who recognize impending consequences but lack formal authority to intervene. This article outlines practical approaches for managing liminal-zone patien

Is Appendiceal Cancer a Risk of Nonoperative Management of Pediatric Uncomplicated Appendicitis?

INTRODUCTION: Nonoperative management (NOM) of pediatric uncomplicated appendicitis is becoming increasingly common. As most appendiceal cancer in children is incidentally diagnosed after appendectomy, there is a theoretical risk of missed appendiceal cancer with NOM. The objective of this study was to determine the incidence of appendiceal cancer in children who otherwise met the criteria for NOM. METHODS: This study is a secondary analysis of a multi-intuitional, nonrandomized interventional trial comparing surgery to NOM of uncomplicated appendicitis in children 7 to 17 y across ten tertia

Cardiovascular risk in psoriatic arthritis: How can we manage it?

It is now widely recognized that patients with psoriatic arthritis (PsA) have a higher risk of cardiovascular disease (CVD) when compared with the general population. A number of factors contribute to this increased risk. Skin and articular inflammation have been shown to be independently associated with CVD in PsA patients. Metabolic syndrome and all its components are significantly more frequent in PsA patients than in healthy populations. Depression, which is not uncommon in psoriatic subjects, seems to increase the CV risk. Finally, corticosteroids and non-steroidal anti-inflammatory drugs

Dual lives at risk: managing haemoperitoneum and splenic injury in pregnancy.

A pregnant woman in her 20s, at 31 weeks gestation, presented with severe abdominal pain, reduced fetal movements and mild vaginal bleeding following a fall. Examination revealed pallor, a tense abdomen and absent fetal movements. Point-of-care ultrasound indicated haemoperitoneum and placental abruption. Emergency laparotomy and lower segment caesarean section were performed, delivering a 1 kg neonate with Apgar scores of 5 and 7. Intraoperative findings included 3 L haemoperitoneum, placental abruption and splenic hilar vascular injury, necessitating splenectomy. The mother received blood tr

NOVEL AGENTS IN THE MANAGEMENT OF DIABETES AND RISK OF WORSENING DIABETIC RETINOPATHY.

PURPOSE: Novel therapies for diabetes have potent effects on glycemic control, obesity, and cardiovascular risk reduction, but some, including the popular drug semaglutide, have also been implicated in worsening of diabetic retinopathy (DR). Given the ubiquity of these new agents, understanding the risks to vision is important. Here, we review the data for several newly available agents in terms of systemic efficacy and retinal safety. METHODS: Literature review. RESULTS: Novel antihyperglycemic treatments include incretin mimetics and enhancers, sodium-glucose cotransporter inhibitors, long

An overview of risk factors, management and prevention of cochlear implant infections.

PURPOSE OF REVIEW: With cochlear implantation becoming increasingly performed worldwide, an understanding of the risk factors, preventive measures, and management of cochlear implant (CI) infection remains important given the significant morbidity and cost it conveys. RECENT FINDINGS: At the turn of the 21st century there was a decrease in rates of CI infection, particularly meningitis, following the discontinuation of positioner use for CI. However, in more recent years rates of CI infection have remained largely static. Recently, studies evaluating preventive measures such as pneumococcal v

Managing the risks of an asbestos bulk storage facility at a research institute.

The South African National Institute for Occupational Health (NIOH), formerly the Pneumoconiosis Research Unit, has previously milled about 544 kg of anthophyllite, crocidolite, amosite and chrysotile asbestos fibre materials. This endeavour came about in an attempt to address a recommendation, made by the International Union Against Cancer (UICC), to make asbestos standard reference samples available for research. Some of these reference samples, as well as the bulk, unprocessed materials are still within the care of the NIOH and can be obtained for the purpose of Public Health research under
