AIM: To emphasize the role of clinical suspect and echographic approach to heterotopic pregnancy in young women complaining of acute abdominal pain.
MATERIAL OF STUDY: Tubal pregnancy in a young woman has been reported. Diagnostic clinical suspect, also supported by serum hormonal substances (beta HCG), has been confirmed by US in urgency; transabdominal investigation requires skilled operator, preferably surgically trained, to identify early study of heterotopic pregnancy (fifth week of gestation), which has undergone left salpingectomy in our experience.

Enterosalpingeal fistula is a rare complication of Crohn's disease which is rarely diagnosed preoperatively. We describe a new case of this complication suspected by CT scan and confirmed by hysterosalpingogram and contrast from the ileum. This case is about a 50-year-old woman suffering from ileal Crohn's disease diagnosed two years previously and actually complicated by stenosis and entero-salpingeal fistula. The treatment consisted on resection of the ileocaecal region with salpingectomy. Postoperative course was uneventful.

A case of spontaneous heterotopic pregnancy is reported that presented with 10 weeks of amenorrhea. Transvaginal ultrasound showed an intra-uterine gestational sac as well as left-sided tubal pregnancy. Operative laparoscopy and left salpingectomy was performed while the intrauterine pregnancy continued and resulted in a liveborn vaginal delivery at 36 week of gestation. The promising neonatal outcome suggests that laparoscopy could be an appropriate method to manage some carefully selected patients with heterotopic pregnancy.

A case of simultaneous ectopic and intra-uterine pregnancy following in vitro fertilization and embryo transfer (IVF-ET) is described. On day 56 after ET the tubal pregnancy ruptured, with massive hemoperitoneum. Following salpingectomy, the intra-uterine pregnancy continued to term when delivery was carried out by cesarean section owing to non-progress of labor and suspected fetal distress. The pitfalls in diagnosing a simultaneous pregnancy of this kind following IVF-ET are discussed.

We present a case of chemotherapy refractory spindle cell rhabdomyosarcoma of the lower urinary tract in a 15-month-old female that ultimately required consolidative surgery with cystectomy, urethrectomy, ovarian-sparing hysterectomy, bilateral salpingectomy, anterior vaginal wall resection, and bilateral pelvic lymph node dissection. Genitourinary reconstruction was performed by ileal conduit creation and vaginoplasty. After completion of her maintenance postoperative chemotherapy regimen, the patient has remained disease-free for approximately 27 months.

Endometriosis is the presence of functioning endometrium outside the uterus. Endometriosis rarely occurs in the abdominal wall. Majority of abdominal wall endometriosis occur in or adjacent to surgical scars, following caesarean section or hysterectomy. Laparotomy scar endometriosis following salpingectomy for ectopic pregnancy has rarely been reported. We report a case of scar endometriosis following laparotomy for chronic ectopic, and diagnosed by fine needle aspiration cytology (FNAC). Excision biopsy confirmed the FNAC diagnosis of scar endometriosis.

A 27 year old patient was admitted with heterotopic pregnancy. Ultrasound scan done by her general practitioner had shown a 14 week viable intra-uterine pregnancy and fluid in the paracolic gutters. An exploratory laparatomy was done and she was found to have a ruptured right-sided ectopic pregnancy. Right partial salpingectomy was done. She had an uneventful post operative course. Ultrasound scan done at 18 weeks showed that the foetus was growing well.

Abdominal ectopic pregnancy is an uncommon form of ectopic pregnancy with few cases reported in Nigeria. We present a case report of abdominal ectopic pregnancy with haemoperitoneum in an unbooked multipara at advanced gestational age. She subsequently had an emergency exploratory laparotomy and right salpingectomy. Diagnosis of this condition can be challenging due to non-specific clinical symptoms, while definitive management can also pose a challenge due to the site of placental attachment.

A 32-year-old Nigerian woman, who became pregnant after undergoing in vitro fertilization, was admitted with nausea and abdominal pain. She had a history of two miscarriages and infertility because of tubal blockage treated by salpingectomy. One week prior, she presented to an outside hospital with premature rupture of membranes resulting in stillborn delivery of twins. Endometrial cultures from dilatation and curettage grew Escherichia coli, and she was started on a fluoroquinolone for chorioamnionitis.

Ectopic pregnancy occurs in 2% of all pregnancies and is a potentially life-threatening emergency. A high level of clinical suspicion is required for any pregnant patient who presents with vaginal bleeding and/or pelvic pain. Workup should begin with immediate triage based on vital signs, a pregnancy test, and transvaginal ultrasound. Ectopic pregnancy can be treated either medically with methotrexate or surgically with either salpingectomy or salpingostomy. Carefully counseled, asymptomatic patients may be candidates for expectant management.

Serous adenofibroma of the fallopian tube is a rare, benign tumour of the female genital tract. They are usually small, asymptomatic and incidentally diagnosed during a surgery for another gynaecological condition. This report presents an atypical case of a 17-year-old girl with a tubal serous adenofibroma that presented with a palpable mass occupying the entire abdomen accompanied by urinary symptoms. She underwent a laparoscopic surgery with drainage of 1800 mL of yellow, citrine liquid from the cyst and left salpingectomy with no complications.

Human pasteurellosis with a gynecologic tropism is rare. A 54 year-old woman presented with sepsis due to a uterine abscess from pasteurella multocida (PM) secondarily complicated by gluteal abscess. She had a history of adenomyosis and had previously undergone hysteroscopic sterilization with micro-inserts. Treatment consisted in antibiotherapy, laparoscopic hysterectomy with salpingectomy and abscess drainage, which resulted in complete recovery. This is the first case of uterine abscess from PM with a second septic location.

Tubal torsion is a very rare event, especially in pregnancy. We present a case of a patient of 20 weeks gestation that was admitted to our clinic with acute abdomen. Radiological and biochemical investigations did not reveal the cause of abdominal pain which resulted in laparatomic exploration. During the operation, the paratubal cystic mass, previously explored by ultrasonographic examination, and the left fallopian tube were found twisted among themselves. Salpingectomy was performed due to the necrotic appearance of the fallopian tube.

We report a case of biopsy-proven renal cortical necrosis in a 42-year-old man after bioprosthetic aortic valve replacement despite prophylactic anticoagulation. He had antiphospholipid syndrome with chronic thrombotic microangiopathy and triple-positive antibodies, leading to renal cortical necrosis manifested as severe non-reversible dialysis-dependent acute kidney injury. We identified 13 previously published reports of renal cortical necrosis due to antiphospholipid syndrome: 3 occurred postpartum, 2 after kidney transplantation, and 1 following salpingectomy for ectopic pregnancy.

Red degeneration of uterine fibroids is a known complication during pregnancy, but is exceedingly rare in the postpartum period. We present the case of a postpartum female who developed acute abdominal pain, a palpable mass, and elevated tumor markers. Imaging raised suspicion for postpartum red degeneration of a uterine leiomyoma, undetected during antenatal scans. She underwent total abdominal hysterectomy with bilateral salpingectomy, and histopathology confirmed the diagnosis. This case highlights the diagnostic challenges associated with postpartum fibroid degeneration.
