[Anterior-segment indocyanine green angiography in the management of anterior scleritis].

PURPOSE: To assess the value of anterior segment indocyanine green (ICG) angiography for the diagnosis, prognosis, and treatment of anterior scleritis. PATIENTS AND METHODS: Anterior segment ICG angiography was performed in five patients presenting with diffuse or nodular scleritis. Angiographic findings before and after treatment were described with special emphasis on dye leakage, vessel abnormalities, and perfusion defects. A systemic medical workup (laboratory tests, imaging investigations, and systemic examination) was performed in all cases to rule out underlying systemic disease. RESU…

Scleritis associated with hepatitis C: a case report and review of literature.

There has been an increase in the complications associated with hepatitis C. Scleritis is one of the complications of the condition that is not routinely suspected, and thus underdiagnosed or missed, leading to delay in initiation of treatment. This case report will bring that to the attention of ophthalmologists so necessary systemic therapy can be instituted in a timely fashion.

Use of topical steroid therapy in the management of nonnecrotizing anterior scleritis.

BACKGROUND: Nonnecrotizing anterior scleritis may be treated with a variety of therapies, including topical steroid therapy, systemic therapy with nonsteroidal antiinflammatory drugs (NSAIDs) and systemic steroid therapy. This study was carried out to determine the efficacy of topical steroid therapy in treating diffuse and nodular scleritis. METHODS: A phase I/II descriptive study was conducted. Thirty-two consecutive patients with nonnecrotizing anterior scleritis referred to a tertiary care ophthalmology cornea and uveitis practice in Ottawa were enrolled between September 1995 and Februar…

Treatment strategies for scleritis and uveitis associated with inflammatory bowel disease.

We treated 19 patients with anterior uveitis, episcleritis, or scleritis associated with inflammatory bowel disease. Adequate control of ocular inflammation was achieved in 16 patients (84%). Ocular inflammation was adequately controlled with corticosteroids alone, without systemic adverse effects, in only three patients, all of whom had anterior uveitis associated with ulcerative colitis. Systemic nonsteroidal anti-inflammatory drugs proved beneficial in six of seven patients, and one additional patient benefited from another anti-inflammatory drug (hydroxychloroquine sulfate). Systemic cytot…

Use of cyclosporin in the management of steroid dependent non-necrotising scleritis.

Many patients with non-necrotising scleritis can be treated adequately with non-steroidal anti-inflammatory drugs or steroids. But, as many of them are young, treatment may present problems if they require high doses of steroids to control the scleral inflammation and then relapse when the dosage is reduced. Five such patients were treated with a combination of steroids and low-dose cyclosporin therapy, and in all cases the scleritis remained under control on a much lower dose of steroids than when steroids were used alone. Cyclosporin is therefore a useful drug in the management of younger pa…

Anterior segment optical coherence tomography: A monitoring tool in anterior scleritis.

PURPOSE: The purpose of this study was to evaluate the role of anterior segment optical coherence tomography (ASOCT) in the diagnosis and management of anterior scleritis. PATIENTS AND METHODS: In this retrospective study, we examined 58 eyes of 44 patients with anterior scleritis. The unaffected eye served as a control. In bilateral cases, the less affected eye was taken as the control. ASOCT image over the inflamed area and over the corresponding same area in the control eye was taken. The images were analysed for the presence or absence of hyporeflective areas, and the mean total scleral t…

Association Between Thyroid Disease and Scleritis Using a Population-Based Method.

PURPOSE: To investigate the association between scleritis and thyroid eye disease in a U.S. Midwestern county population. METHODS: Retrospective, case-control study of all patients diagnosed with ocular inflammatory disease in Olmsted County, Minnesota from January 1, 2006, to December 31, 2015. Patients were identified using the Rochester Epidemiology Project database, which contains virtually all medical care in the county. Medical records were reviewed to confirm diagnosis of scleritis. A 3:1 age- and sex-matched control group was also identified using the database. Patient records for the…

Surgically-Induced Necrotizing Scleritis After Scleral Buckling With Stenotrophomonas maltophilia Infection.

Surgically induced necrotizing scleritis (SINS) is a rare inflammatory disease of the sclera that occurs following ocular surgery, specifically pterygium surgery and scleral buckling. Here, we report a case of SINS in a 78-year-old female patient after segmental scleral buckling for rhegmatogenous retinal detachment. The retina was restored after scleral buckling, and the postoperative course was uneventful. However, the patient developed ocular discharge and conjunctival hyperemia, indicating infection, after two months. The sclera became thinner and intraocular inflammation developed after b…

Posterior Scleritis Associated with Late-Onset Takayasu Arteritis: A Case Report.

PURPOSE: To describe a rare case of posterior scleritis associated with late-onset Takayasu arteritis. METHODS: A case report of a 75-year-old female presenting with posterior scleritis, eventually diagnosed with late-onset Takayasu arteritis was described. Several medical evaluation visits were conducted, including ophthalmological and rheumatological follow-up. Multiple diagnostic tests were carried out, and the cornerstones of the treatment were corticosteroids and the IL-6 inhibitor tocilizumab. RESULTS: The patient was admitted to our hospital's emergency room with right unilateral eyel…

IgG4-related disease presenting as recurrent scleritis combined with optic neuropathy.

BACKGROUND: We report a case of atypical presentation of IgG4-related disease (IgG4-RD) with recurrent scleritis and optic nerve involvement. CASE PRESENTATION: A 61-year-old male presented with ocular pain and injection in his left eye for 2 months. Ocular examination together with ancillary testing led to the diagnosis of scleritis, which relapsed in spite of several courses of steroid treatment. After cessation of steroid, the patient complained of severe retro-orbital pain and blurred vision. His best corrected vision was count finger, the pupil was mid-dilated and a relative afferent pup…

Health system wide "big data" analysis of rheumatologic conditions and scleritis.

BACKGROUND: The development of scleritis in the setting of autoimmune conditions has been well documented. Prior series have assessed the relationship between systemic autoimmune disorders and scleritis only in patients referred for rheumatologic or ocular inflammation. This can lead to a referral bias. We reviewed all charts within the electronic medical record (EMR) of a health system for patients with systemic autoimmune and scleritis diagnoses to determine the prevalence of both and which disorders had the highest relative risk of developing scleritis. METHODS: The EMR was searched for sc…

Atypical posterior scleritis mimicking an amelanotic choroidal melanoma. A case report.

Purpose: To describe a clinical case of atypical posterior scleritis mimicking an amelanotic choroidal melanoma. Method: Observational case report of a 54-year-old woman who presented to the emergency department with photophobia and blurred vision in her left eye for three days. The development of a raised hypopigmented lesion superior to the papilla with choroidal folds and without vitritis simulated an amelanotic choroidal melanoma. Differential diagnosis took into consideration other compatible entities, including choroidal masses or orbital pseudotumor. Results: The patient was subject to …

Eye metastasis in lung adenocarcinoma mimicking anterior scleritis: A case report.

BACKGROUND: The eye is a rare site for lung cancer metastasis. Indeed, ocular metastasis is one of the greatest challenges to quality of life in a cancer patient. Here we present a patient with lung adenocarcinoma and ocular metastasis. CASE SUMMARY: The patient was a 70-year-old man diagnosed with lung adenocarcinoma who developed eye metastasis mimicking anterior scleritis. Brain magnetic resonance imaging showed an abnormal signal in the right eye. Based on next generation sequencing of the surgical specimen, the patient was shown to have a KRAS point mutation (p.G12D). CONCLUSION: Multid…
