Recognising skin manifestations of rheumatological disease.

BACKGROUND: An understanding of skin conditions associated with rheumatic diseases ensures accurate diagnosis and management. Cutaneous manifestations of rheumatological disease are legion. OBJECTIVE: The aim of this article is to increase clinician familiarity with the dermatological manifestations of rheumatic conditions to enable accurate diagnosis and effective management. DISCUSSION: This article will address the skin manifestations of lupus erythematosus, rheumatoid arthritis, psoriatic arthritis, dermatomyositis and scleroderma, including their implications in diagnosis, prognosis and

Linear morphea with secondary mucinosis.

Secondary mucin deposition in the skin is a common feature of lupus erythematosus and dermatomyositis. In scleroderma, it occurs uncommonly or in small amount. We describe a 7-year-old boy with progressive, linear, bound-down plaques involving the thighs, lower abdomen and back with no systemic involvement. Histopathology showed features of scleroderma with abundant mucin deposition in the reticular dermis. This report highlights excessive mucin deposition in lesions of morphea.

Sclerosing panniculitis.

Sclerosing panniculitis is a chronic panniculitis characterized by skin induration and hyperpigmentation of one or both legs that often occurs in patients who have venous insufficiency. The indurated plaques are often painful and the legs frequently have a characteristic "inverted wine bottle" appearance. Histopathology is characterized by mostly lobular panniculitis with necrosis of fat and without vasculitis, and in fully developed lesions by septal sclerosis and membranocystic changes. Treatment of sclerosing panniculitis includes compression therapy with graded stockings and anabolic stero

Case study: periodic follow-up is necessary in morphea profunda to identify systemic evolution.

The term "morphea" includes a wide spectrum of clinical entities, varying from localized plaques of only cosmetic importance to deep lesions resulting in considerable morbidity for the patient. In fact, although survival rates are no different from that of the general population, localized scleroderma may be associated with development of substantial disability, as occurs in deep morphea and in pediatric patients (disabling pansclerotic morphea of children). We report a case of morphea profunda affecting a young man with severe, rapidly progressive, widespread skin involvement and focus on the

[Scleroderma en coup de sabre with intracranial involvement. A case report].

Scleroderma en coup de sabre is a descriptive term denoting linear scleroderma of the frontoparietal area of the face and scalp. This form of scleroderma is infrequent, mainly in pediatric patients, but not infrequently associates neurologic symptoms, especially epilepsy. We describe a 15-year-old girl with scleroderma en coup de sabre and a long-standing history of epilepsy. Cranial magnetic resonance examination reaffirms the clinical impression that inflammation is not solely confined to the skin in this form of linear scleroderma.
