KEY POINTS: Treatment with semaglutide improves kidney function and pathology in the mouse nephrotoxic serum nephritis model. Transcriptomics data demonstrated that semaglutide had anti-inflammatory and antifibrotic effects and indicated beneficial effects on renal hemodynamics. These beneficial effects of semaglutide occur independent of metabolic effects.
BACKGROUND: CKD is a significant public health issue, affecting approximately half a billion people globally. Key risk factors for CKD include obesity, hypertension, cardiovascular diseases, and diabetes. Glucagon-like peptide-1 receptor a…

BACKGROUND: Semaglutide, a glucagon-like peptide-1 (GLP-1) receptor agonist, has shown promise in managing hyperglycemia and reducing cardiovascular (CV) outcomes. However, its effects on arrhythmic, major CV, and microvascular outcomes remain uncertain. This systematic review and meta-analysis aimed to evaluate these outcomes in patients with type 2 diabetes (T2D) treated with semaglutide.
METHODS: We searched the PubMed, Embase, and Cochrane databases for eligible randomized controlled trials (RCTs) reported up to November 2024. We performed a meta-analysis via a random-effects model to est…

In 2024, a second brand of injectable semaglutide was marketed, authorized for the indication of weight loss, along with the first tirzepatide. Both are indicated as an adjunct to a low-calorie diet and increased physical activity for weight control, including weight loss and maintenance, in adults with a BMI greater than 27. In Community Pharmacy, we have a Professional Pharmaceutical Service in Nutrition, and it is of interest to study and compare the outcomes obtained by patients using these drugs versus those who only adopt lifestyle changes in diet and physical activity. This is especiall…

Fibroblast growth factor 21 (FGF21) analogs have significant therapeutic potential in metabolic dysfunction-associated steatohepatitis (MASH) but limited body weight effects in patients with MASH. This study investigated the effect of combined treatment with the FGF21 analog zalfermin and the glucagon-like peptide-1 receptor agonist semaglutide on body weight and plasma and liver biochemistry and histology in a mouse model of MASH. Amylin liver nonalcoholic steatohepatitis diet-induced obese-MASH mice with biopsy-confirmed MASH and fibrosis were administered (subcutaneous [SC], daily [QD]) veh…

A growing number of studies show that the diabetes drug Semaglutide is neuroprotective in Alzheimer's disease (AD) animal models, but its mode of action is not fully understood. In order to explore the mechanism of Semaglutide, 7-month-old APP/PS1/tau transgenic (3xTg) mice and wild-type (WT) mice were randomly divided into four groups: control group (WT + PBS), AD model group (3xTg + PBS), Semaglutide control group (WT + Semaglutide) and Semaglutide treatment group (3xTg + Semaglutide). Semaglutide (25 nmol/kg) or PBS was administered intraperitoneally once every two days for 30 days, followe…

OBJECTIVES: Many people with type 2 diabetes experience clinical inertia, remaining in poor glycaemic control on oral glucose-lowering medications rather than intensifying treatment with a glucagon-like peptide-1 receptor agonist, despite an efficacious, orally administered option, oral semaglutide, being available. The present study evaluated the long-term cost-effectiveness of initiating oral semaglutide versus continuing metformin plus sodium-glucose cotransporter-2 (SGLT-2) inhibitor therapy in the UK.
DESIGN: Outcomes were projected over patients' lifetimes using the IQVIA Core Diabetes …

Incretin-based agents are transforming obesity pharmacotherapy, and orforglipron, the first oral non-peptide GLP-1 receptor agonist to complete phase 3 testing, invites direct comparison with the injectable agents semaglutide and tirzepatide. We integrated clinical trial evidence, including the direct head-to-head SURMOUNT-5 trial, independently verified molecular descriptors, a fully specified drug-target-disease network with permutation-tested hub significance, hypergeometric pathway enrichment, and multi-criteria decision analysis (MCDA) across four weighting schemes with formal sensitivity…

INTRODUCTION: Obesity has reached epidemic proportions in South Asia, contributing significantly to metabolic diseases and economic burdens. Newer antiobesity medications (AOMs), such as semaglutide and tirzepatide, show promising efficacy but face barriers such as high costs and limited familiarity among healthcare professionals (HCPs) in low- and middle-income countries (LMICs). This study explored perceptions, prescribing practices, efficacy, safety concerns, and barriers to AOM use among HCPs managing obesity in South Asia.
METHODS: A cross-sectional online survey was conducted from May t…

BACKGROUND: Obesity is a chronic disease associated with substantial morbidity and healthcare costs. In Japan, the Optimal Use Promotion Guidelines (OUG) impose restrictions on patient eligibility for semaglutide 2.4 mg and limit treatment duration. This study evaluated the cost-effectiveness of semaglutide 2.4 mg in Japan under retreatment assumptions informed by OUG requirements.
METHODS: A Markov cohort model (Core Obesity Model) was used to evaluate lifetime clinical and economic outcomes from a Japanese public payer perspective. The Core Obesity Model assesses the associations between ri…

BACKGROUND: Multiple meta-analyses report that glucagon-like peptide-1 receptor agonists (GLP-1 RAs), and semaglutide in particular, are associated with lower incident atrial fibrillation (AF) and reduced post-ablation AF recurrence. These reviews have appeared in rapid succession, often drawing on the same trials.
OBJECTIVE: To systematically identify, appraise, and reconcile the published meta-analyses examining GLP-1 RAs/semaglutide and AF across two clinical questions-incident AF and post-ablation AF recurrence.
METHODS: We conducted an umbrella review following Preferred Reporting Items…

BACKGROUND: Rare sugar D-Allulose, a zero-calorie sweetener, markedly ameliorates obesity. It reportedly stimulates the release of endogenous glucagon-like peptide 1 (GLP-1) to activate vagal afferent and directly influences the neurons in hypothalamic arcuate nucleus (ARH), thus evoking vagal and central nervous routes. D-Allulose can now be produced substantially, being expected for diet therapy. Oral form GLP-1 receptor agonist (GLP-1RA), Oral semaglutide (O-Sema), without injection markedly ameliorates obesity. It evokes only central nervous route. Thus, these GLP-1-based substances utiliz…

BACKGROUND: Diabetes mellitus (DM) is a major risk factor for postoperative infection and wound complications in spine surgery, yet distinctions between Type 2 (T2DM) and Type 1 (T1DM) pathophysiology are rarely addressed. This study compares infectious burden, wound healing, and immune response among a murine model of spinal implant-associated infection of T2DM, T1DM, and nondiabetic control mice before and after metabolic intervention with the GLP-1 receptor agonist (GLP-1RA), semaglutide.
METHODS: Male C57BL/6J mice were rendered diabetic by streptozotocin induction (T1DM) or a high-fat, h…
