Takotsubo cardiomyopathy following aeromonas hydrophila septic shock after traumatic injury: a case report.

BACKGROUND: Aeromonas hydrophila is a Gram-negative bacillus commonly found in aquatic environments. It is capable of causing skin and soft tissue infections, and in immunocompromised individuals, it may lead to severe systemic infections and septic shock. Takotsubo cardiomyopathy (TTS) is a rare cardiac syndrome typically precipitated by acute stressors. CASE INTRODUCTION: This report describes a case of a patient who experienced cardiac arrest following trauma sustained in a road traffic accident, complicated by lower limb infection. The infection progressed rapidly, culminating in cardiac

Traumatic colopleural fistula presenting as tension fecohemopneumothorax and septic shock: A case report.

A colopleural fistula represents a rare pathological communication between the colon and the pleural cavity, typically resulting from trauma, malignancy, or inflammatory bowel disease. One of the exceedingly rare complications of a fistula is fecopneumothorax, characterized by the presence of gas and fecal matter in the pleural space, which can lead to fatal sepsis if not diagnosed promptly. We report an instance of a traumatic colopleural fistula in a 49-year-old male who suffered multiple gunshot wounds to the chest. Despite emergent surgical intervention and intensive critical care manageme

MKK3 depletion attenuates intestinal injury after traumatic hemorrhagic shock by restoring mitochondrial function.

BACKGROUND: Traumatic hemorrhagic shock (THS) is a complex pathophysiological process resulting in multiple organ failure. Intestinal barrier dysfunction is one of the mechanisms implicated in multiple organ failure. The present study aimed to explore the regulatory role of mitogen-activated protein kinase kinase 3 (MKK3) in THS-induced intestinal injury and to elucidate its potential mechanism. METHODS: Rats were subjected to trauma and hemorrhage to establish a THS animal model. MKK3-targeted lentiviral vectors were injected via the tail vein 72 h before modeling. Twelve hours post-modeling

The Effects of Genetic 3-Mercaptopyruvate Sulfurtransferase Deficiency in Murine Traumatic-Hemorrhagic Shock.

INTRODUCTION: Hemorrhagic shock is a major cause of death after trauma. An additional blunt chest trauma independently contributes to mortality upon the development of an acute lung injury (ALI) by aggravating pathophysiological consequences of hemorrhagic shock. The maintenance of hydrogen sulfide availability is known to play an important role during hemorrhage and ALI. We therefore tested the impact of a genetic 3-mercaptopyruvate sulfurtransferase mutation (Δ3-MST) in a resuscitated murine model of traumatic-hemorrhagic shock. METHODS: Anesthetized wild-type (WT) and Δ3-MST mice underwent

Understanding traumatic shock: out-of-hospital hypotension with and without other physiologic compromise.

BACKGROUND: Among trauma patients with out-of-hospital hypotension, we evaluated the predictive value of systolic blood pressure (SBP) with and without other physiologic compromise for identifying trauma patients requiring early critical resources. METHODS: This was a secondary analysis of a prospective cohort of injured patients 13 years or older with out-of-hospital hypotension (SBP ≤ 90 mm Hg) who were transported by 114 emergency medical service agencies to 56 Level I and II trauma centers in 11 regions of the United States and Canada from January 1, 2010, through June 30, 2011. The prima

Local hemostasis, immunothrombosis, and systemic disseminated intravascular coagulation in trauma and traumatic shock.

Knowing the pathophysiology of trauma-induced coagulopathy is important for the management of severely injured trauma patients. The aims of this review are to provide a summary of the recent advances in our understanding of thrombosis and hemostasis following trauma and to discuss the pathogenesis of disseminated intravascular coagulation (DIC) at an early stage of trauma. Local hemostasis and thrombosis respectively act to induce physiological wound healing of injuries and innate immune responses to damaged-self following trauma. However, if overwhelmed by systemic inflammation caused by exte

Resuscitation strategies with different arterial pressure targets after surgical management of traumatic shock.

INTRODUCTION: Hypotensive fluid resuscitation has a better effect before and during surgical intervention for multiple trauma patients with haemorrhagic shock. However, it is questionable whether hypotensive fluid resuscitation is suitable after surgical intervention for these patients, and whether resuscitation with different mean arterial pressure (MAP) targets after surgical intervention can obtain different results. The aim of this study was to investigate these questions and to explore the underlying mechanisms. METHODS: A total of 30 anesthetized piglets were randomly divided into 3 gro

Sirtuin 1 Agonist Minimizes Injury and Improves the Immune Response Following Traumatic Shock.

Survival from traumatic injury requires a coordinated and controlled inflammatory and immune response. Mitochondrial and metabolic responses to stress have been shown to play a role in these inflammatory and immune responses. We hypothesized that increases in mitochondrial biogenesis via a sirtuin 1 agonist would decrease tissue injury and partially ameliorate the immunosuppression seen following trauma. C57Bl/6 mice were subjected to a multiple trauma model. Mice were pretreated with either 100 mg/kg per day of the sirtuin 1 agonist, Srt1720, via oral gavage for 2 days prior to trauma and ext
