Alternative approach to management of early loss of second primary molar: a clinical case report.

Preservation of space after premature loss of the second primary molar is essential to prevent mesial drifting of the permanent first molar. Various modifications of distal shoe space maintainers, which have been documented, are all nonfunctional in nature. This paper describes an innovative design of a functional distal shoe space maintainer that is intended to overcome the disadvantages of conventional non-functional space maintainers and that does not hamper the periodontal status of the abutment tooth.

Influence of Shod and Barefoot Running on the In Vivo Kinematics of the First Metatarsophalangeal Joint.

The biomechanics of the first metatarsophalangeal joint (MTPJ) is affected by different shoe conditions. In the biomechanical research field, traditional skin marker motion capture cannot easily acquire the in vivo joint kinematics of the first MTPJ in shoes. Thus, the present study aims to investigate the differences of the first MTPJ's six-degree-of-freedom (6DOF) kinematics between shod and barefoot running by using a high-speed dual fluoroscopic imaging system (DFIS). In total, 15 healthy male runners were recruited. Computed tomography scans were taken from each participant's right foot f…

Toe walking in children with cerebral palsy: a possible functional role for the plantar flexors.

Equinus and toe walking are common locomotor disorders in children with cerebral palsy (CP) walking barefoot or with normal shoes. We hypothesized that, regardless of the type of footwear, the plantar flexors do not cause early equinus upon initial foot contact but decelerate ankle dorsiflexion during weight acceptance (WA). This latter action promoted by early flat-foot contact is hypothesized to be functional. Hence, we performed an instrumented gait analysis of 12 children with CP (Gross Motor Function Classification System class: I or II; mean age: 7.2 yr) and 11 age-matched typically deve…

Lemongrass and Perilla Essential Oils Synergistically Increased Antimicrobial Activity.

We postulated that disinfection of viable Trichophyton species in shoes would help reduce the number of patients with tinea pedis in Japan and that this might be accomplished safely using volatile components of essential oils. As vapor of lemongrass (Cymbopogon citratus) oil and citral have strong antimicrobial activities against Trichophyton, we examined the conditions under which lemongrass oil or citral show optimal antimicrobial activity in shoes. First, we investigated whether or not a strong antimicrobial effect could be obtained by combining with terpene aldehydes or aromatic aldehydes.…

Bösch technique for hallux valgus surgery in a tropical setting.

OBJECTIVES: To evaluate the early clinical and radiological results using the Bösch technique to treat hallux valgus. MATERIAL AND METHODS: We reviewed retrospectively four patients with 6 feet undergoing the Bösch technique for mild and moderate hallux valgus from 2009 to 2012 with an average follow-up of 10.8 months. All patients complained of pain around the first metatarsophalangeal joint. They had cosmetic concerns, and difficulty in wearing shoes. At final follow-up patients were asked about the improvement of pain, cosmetic appearance of the foot, problems with wearing shoes, the abili…

Transport of fecal bacteria by boots and vehicle tires in a rural Alaskan community.

People living without piped water and sewer can be at increased risk for diseases transmitted via the fecal-oral route. One rural Alaskan community that relies on hauling water into homes and sewage from homes was studied to determine the pathways of fecal contamination of drinking water and the human environment so that barriers can be established to protect health. Samples were tested for the fecal indicator, Escherichia coli, and the less specific indicator group, total coliforms. Shoes transported fecal contamination from outside to floor material inside buildings. Contamination in puddles…

[Clinical risk factors of vitreoretinal proliferations in rhegmatogenous retinal detachment].

PVR is a complication of rhegmatogenous retinal detachment which can occur only in predisposed eyes. Preoperative PVR and postoperative PVR can develop solely in retinal detachments associated with retinal breaks related to vitreous traction (horse-shoe tears, operculated tears, crescent tears and paravascular tears of the postequatorial region). PVR never develops in retinal detachments due to retinogenic retinal breaks (atrophic holes in lattice, and oral dialysis). There are 3 independent risk factors for postoperative PVR whose role has been demonstrated: preoperative PVR, horse-shoe tears

Juvenile plantar dermatosis in Singapore.

Juvenile plantar dermatosis (JPD) is the most common form of foot dermatitis in children in Singapore. Sixty-four patients (29 boys) with an age range of 2-25 years were studied. The Chinese have a greater predisposition for JPD compared with the Malays and Indians. The mean age of onset was 8 years, but onset in adulthood is not uncommon. Friction and closed shoes are the main aggravating factors. Association with an atopic history (42%) and ichthyosis vulgaris (19%) is reported. Patch testing was not rewarding. The histology was essentially a subacute dermatitis. Women with no atopic backgro…

Effect of different casting design characteristics on offloading the diabetic foot.

BACKGROUND: Non-removable knee-high devices, such as a total contact cast (TCC), are recommended for offloading diabetic plantar forefoot ulcers. However, it is insufficiently known how each of the different design characteristics of these devices contribute to offloading the diabetic foot. RESEARCH QUESTION: What is the offloading effect of the different design characteristics that make up a non-removable knee-high cast for people with diabetes and active or previous plantar forefoot ulcers? METHODS: Sixteen persons with diabetes, peripheral neuropathy and a healed or active plantar forefoo

Orthotics and rehabilitation for chronic hemophilic synovitis of the ankle. An overview.

The ankle is the second most affected joint in hemophilia. Recurrent bleeding leads to chronic synovitis. Prevention of chronic synovitis should start with prophylactic replacement therapy or on demand treatment of recurrent hemarthrosis. Attention must be given to the function of the ankle joint. When instability is present, an extensive range of exercises is important. An orthotic or shoe adaption may be useful during the rehabilitation process. Viscoheels worn in shoes seem to reduce the bleeding frequency and pain.

Neurofibromatosis-I Presentating with Multiple Spinal and Intracranial Neurofibromas.

Von Recklinghausen's neurofibromatosis (NF-1) is a phacomatosis characterised by widespread nervous system tumours with cutaneous manifestations and variably associated anomalies. We report here a case, who, in addition to classical features of NF-1 (café-au-lait spots, cutaneous and subcutaneous neurofibromas) demonstrated radiological evidence of both spinal and intracranial neurofibromas and an incidentally discovered horse-shoe kidney. The unique constellation of spinal and intracranial neurofibromas, with associated horse-shoe kidney.

The great toe as a clinical problem in rheumatoid arthritis.

Two hundred consecutive in-patients with rheumatoid arthritis were examined for pain or deformity of the feet, and of the great toe in particular. Some abnormality occured in 196 feet and the deformities observed are presented. The symptoms that arise from these deformities are mainly derived from ill-fitting shoes, and the need for suitable foot-wear is emphasized. Two hitherto un-named entities are described namely Hallux tortus and chisel toe, since they give rise to their own shoe-fitting problems.

Iliotibial band syndrome.

The iliotibial band syndrome is most often diagnosed in runners but can be found in athletes who participate in other sports. From our experience most patients are high mileage runners with shoes either worn out or with insufficient cushioning. A total of 19 athletes with this condition have been seen in the past 3 years. Treatment consisted of rest or decreased distance, shoe changes, modification in exercise technique, anti-inflammatory medication, steroid injections and stretching. Surgery was not required.

[Reconstruction of large foot sole areas with microsurgical tissue transfers].

In 14 patients with microsurgically reconstructed areas of the sole videografic gait analysis was performed. The best results are reached after reconstruction of the forefoot. Loss of more than two thirds of the sole leads to functional impairment. For all reconstructed areas and all types of flaps gait is improved significantly by shoe adaptation: elevation of the shoe under the reconstructed part of the sole directs the main load to the opposite side of the foot and by this relieves the reconstructed area. Even bone corrections can reduce load on a circumscribed area.

T2-weighted MR imaging of thigh muscle response in male distance runners wearing advanced footwear technology.

BACKGROUND: Advanced footwear technology shoes (AFTs) combine highly resilient midsoles with embedded carbon-fiber plates and are known to alter knee joint kinetics and perceived stability. Whether these biomechanical and perceptual differences translate into altered muscle metabolic responses at the thigh in running remains unclear. OBJECTIVE: This study aimed to clarify whether AFTs elicited greater exercise-induced transverse relaxation times (T2) shifts (an MR imaging index of metabolic responses in activated muscles) in quadriceps and hamstrings compared with traditional running shoes (T…
