Upper extremity fractures due to intimate partner violence versus accidental causes.

PURPOSE: The purpose of this study is to evaluate the prevalence of intimate partner violence (IPV)-related upper extremity fractures (UEF) … l striking (53.3%) but accounted for only 10% of fall-related UEF. There was a higher proportion of shoulder fractures in IPV (9.2%) compared to accidental falls (7.4%) or strikes (2.9%). The odds of a finger fracture were 4.32 times greater in IPV than falling and of a shoulder fracture were 3.65 greater in IPV than accidental striking (p < 0.0001). CONCLUSIONS: W …

Intramedullary nailing of humeral shaft fractures. A retrospective study of 126 cases.

Antegrade intramedullary nailing with four different implants was used in 126 humeral shaft fractures. There were 74 acute fractures, 17 pathologic fractures, 16 fractures malaligned in a hanging cast or brace, 15 fractures with delayed union and 4 fractures that were nailed after failed open reduction and internal fixation. The nonunion rate was 21/95 after primary operation, and after reoperations 14/95. Distraction of the fracture was a significant cause of nonunion, but not type of fracture, localization, implant, and delay between injury and surgery. Shoulder joint function was significan

Injury patterns to other body regions and load vectors in nearside impact occupants with and without shoulder injuries.

CIREN and NASS-CDS databases were used to analyze nearside impact injuries. Front seat occupants with and without shoulder injuries were examined on an individual basis in both databases. All vehicles were from model year 2000 or newer. Variables such as the type of collision, change in velocity, principal direction force, demographics, injuries scored by the MAIS and ISS metrics, and injuries to the head, thorax, abdomen and pelvis were included. Shoulder injuries included fractures to the humerus, scapula and clavicle, and associated joint traumas. The median changes in velocities for occupa

Perioperative Factors Affecting the Length of Hospitalization After Shoulder Arthroplasty.

OBJECTIVE: To determine factors related to length of hospitalization after shoulder arthroplasty. METHODS: A retrospective review identified patients who underwent shoulder arthroplasty between 2009 and 2012 at our institution. Factors were evaluated for their association with hospitalization length in a univariate model. Diagnoses were categorized into four groups: (1) osteoarthritis, cuff tear arthropathy, massive rotator cuff tear, or osteonecrosis; (2) acute fracture or fracture sequelae; (3) inflammatory arthropathy; and (4) failed shoulder arthroplasty. Significant factors were then eva

Acute Fractures of the Glenoid.

Glenoid fractures are unique in which they span the fields of orthopaedic traumatology and sports medicine. Treatment of glenoid fractures, whether surgical or nonsurgical, may be challenging and have long-term implications on pain and shoulder function. Plain radiographs are always indicated, and most glenoid fractures will require advanced imaging in the form of CT scan. Two general categories of glenoid fractures exist and differ in mechanism of injury, fracture morphology, and treatment. The first category is glenoid fractures with extension into the scapular neck and body. These fractures

Displaced mid-shaft clavicular fractures: surgical treatment with intramedullary screw fixation.

INTRODUCTION: Clavicle fractures account for around 4 % of all fractures and up to 44 % of fractures of the shoulder girdle. Fractures of the middle third account for approximately 80 % of all clavicle fractures. Management of mid-shaft clavicular fractures is often challenging and the outcome can be unsatisfactory. MATERIALS AND METHODS: We prospectively evaluated 20 patients (16 males and 4 females) with an average age of 31 years (range 18-50 years) presented with fresh mid-shaft clavicular fractures who underwent open intramedullary fixation using a 6.5 partially threaded cancellous screw

Double plating - surgical technique and good clinical results in complex and highly unstable proximal humeral fractures.

INTRODUCTION: A stable fixation of highly unstable proximal humerus fractures remains challenging and complication rates, especially secondary varus dislocation, remains high. Different techniques of double plate osteosynthesis have been suggested for the treatment of complex proximal humeral fractures as they are well established for other fractures. The aim of this study was to evaluate an operative technique using an angular stable lateral plate supported by a one-third tubular plate positioned anteriorly at the lesser tuberosity for unstable proximal humeral fractures. PATIENTS AND METHOD

Displaced fractures of the greater tuberosity: a comparison of operative and nonoperative treatment.

BACKGROUND: Displaced two-part fractures of the greater tuberosity requiring surgical intervention are rare and the literature gives only few data of functional results after operative treatment. The purpose of this study was to analyze functional and radiographic long-term results in patients who had undergone surgical treatment of displaced greater tuberosity fractures and to compare those results with the results of patients who had been treated nonoperatively. MATERIAL: From a prospectively gathered database, we retrospectively analyzed functional and radiographic results of 52 patients w

[Anatomical bases of ultrasonic examination of the shoulder joint].

Authors present the parts of the shoulder that can be represented by ultrasound. Beside the rotator cuff of accentuated significance (within this, first of all the tendon of the supraspinatus muscle) the subacromial bursa, the tendon of the long head of the biceps, the muscle-tendon junction of the infraspinatus muscle are analysed. It is stated that the ultrasonography is suitable for a reliable representation of the soft tissues, surrounding the shoulder joint, further in possession of literary data they see possibility for a very exact judgement of the pathological alterations, too.

Isolated avulsion fracture of the lesser tuberosity of the humerus. A case report.

An extremely rare isolated avulsion fracture of the lesser tuberosity, not complicated with other proximal humeral fractures or posterior shoulder dislocations, occurred in a 12-year-old girl. The avulsion fracture of the upper part of the lesser tuberosity appears to have been caused by hyperextension and hyperexternal rotation of the shoulder. In this position, the coracohumeral ligament, superior glenohumeral ligament, and subscapularis tendon could have pulled out the upper part of the lesser tuberosity. She gained full recovery of left shoulder function after three months of conservative

A concept analysis and overview of outcome measures used for evaluating patients with proximal humerus fractures.

PURPOSE: It is unclear to which degree existing studies evaluate the primary goal of treatment for patients with proximal humerus fractures (restoration of daily activities). Our purpose was to systematically review and analyze the concepts reflected by outcome measures used in studies of patients with proximal humerus fractures. METHODS: We reviewed three databases from 2000 to 2018. Two reviewers categorized outcomes in each study into concepts of the International Classification of Functioning, Disability and Health framework. RESULTS: The most commonly represented concept across 35 studi

Nonoperative treatment of ipsilateral fractures of the scapula and clavicle.

BACKGROUND: Internal fixation of one or both bones is the recommended treatment for floating shoulder injuries (ipsilateral fractures of the scapula and clavicle). Perceived risks of nonoperative treatment include abduction weakness, decreased range of motion, chronic pain, malunion, and nonunion. None of these problems, however, have been confirmed by clinical studies. The purpose of this retrospective study was to analyze the clinical and radiographic results of nonoperative treatment of floating shoulder injuries. METHODS: Twenty patients with a floating shoulder injury were treated with e

A variant of a type V lateral clavicle fracture involving a posteriorly displaced medial segment. A case report.

The clavicle connects the shoulder girdle to the axial skeleton, providing support and mobility for optimal upper extremity function. Fractures of the clavicle account for up to 4% of all fractures and comprise up to 44% of all injuries to the shoulder girdle. We present a 63-year-old female patient who suffered what appeared to be a minimally displaced Type V lateral clavicle fracture after a fall as evidenced by an anteroposterior shoulder radiograph. However, an axillary projection demonstrated the proximal segment to be posteriorly displaced and buttonholed through the trapezius musculatur

Clinical Outcome of Carbon Fiber Reinforced Polyetheretherketone Plates in Patients with Proximal Humeral Fracture: One-Year Follow-Up.

BACKGROUND: Proximal humerus fractures are seen frequently, particularly in older patients. The development of new osteosynthesis materials is being driven by the high complication rates following surgical treatment of proximal humerus fractures. Plate osteosyntheses made of steel, titanium and, for several years now, carbon fiber-reinforced polyetheretherketone (CFR-PEEK) are used most frequently. METHODS: A prospective, randomized study was conducted in order to evaluate whether there are differences in the functional postoperative outcome when comparing CFR-PEEK and titanium implants for s

Role of us in the preoperative evaluation of patients with anterior shoulder instability.

PURPOSE: To assess the value of ultrasonography (US) in the preoperative evaluation of patients with anterior shoulder instability. MATERIALS AND METHODS: Twenty-two patients with one-sided anterior shoulder instability were examined with US by using three dynamic scanning approaches: two frontal and one axillary. The anterior labrum, the anterior ligamental-capsular complex, and the presence of humeral head and glenoid rim fractures were evaluated. Arthroscopy or arthrotomy was subsequently performed in all patients and was considered the standard. RESULTS: US correctly depicted the presenc

Influence of rotator cuff preload on fracture configuration in proximal humerus fractures: a proof of concept for fracture simulation.

INTRODUCTION: In regard of surgical training, the reproducible simulation of life-like proximal humerus fractures in human cadaveric specimens is desirable. The aim of the present study was to develop a technique that allows simulation of realistic proximal humerus fractures and to analyse the influence of rotator cuff preload on the generated lesions in regards of fracture configuration. MATERIALS AND METHODS: Ten cadaveric specimens (6 left, 4 right) were fractured using a custom-made drop-test bench, in two groups. Five specimens were fractured without rotator cuff preload, while the other
