Approach to the treatment of primary anterior shoulder dislocation: A review.

Glenohumeral joint dislocation is common among younger, active patients. Anterior dislocation is the most common direction of instability following a traumatic event. Due to a high rate of recurrence following primary traumatic anterior shoulder dislocation, an evidence-based approach is necessary to determine the best treatment regime for a patient presenting with this problem. A history, physical examination, and radiographic imaging can help guide treatment recommendations by determining the extent of soft tissue damage following dislocation. Controversies in the treatment of the first-time

Shoulder arthrodesis as a treatment for severe shoulder deformity secondary to septic arthritis in two cats.

CASE SERIES SUMMARY: Two rescued juvenile cats with unilateral severe shoulder deformities (SDs) suspected to be secondary to septic arthritis were referred for further management. Comprehensive diagnostic evaluations confirmed SD with suspected septic arthritis in both cases. Because of the poor prognosis for preserving shoulder joint function, shoulder arthrodesis was performed to relieve pain and restore forelimb weightbearing ability. At 6 and 8 years postoperatively, both cats exhibited normal motor function in the affected limbs, no signs of lameness and maintained a good quality of life…

Functional outcome of acromioclavicular joint injury in polytrauma patients.

OBJECTIVE: To evaluate the functional outcome of displaced acromioclavicular (AC) injuries in polytrauma patients. DESIGN: A matched, case-control study using a prospectively gathered trauma database. METHODS: Twelve polytrauma patients with a concomitant displaced acromioclavicular joint injury were matched to polytrauma patients without an acromioclavicular joint injury and to patients with an isolated displaced acromioclavicular joint injury. They were evaluated with a series of five shoulder questionnaires and the SF-36 general health status questionnaire. RESULTS: Shoulder function in

The Global Track Concept for Assessment of Engaging Hill-Sachs Defects in Anterior Shoulder Instability.

BACKGROUND: The glenoid track concept is used to determine preoperatively whether a Hill-Sachs defect is engaging or not. Currently, the glenoid track concept relies on measurements of bony structures as well as on the confines and elasticity of the rotator cuff as a reference point, which varies extensively among individuals and therefore limits the reliability and accuracy of this concept. PURPOSE: To evaluate the reliability of the global track concept, which determines the angular distance of the Hill-Sachs defect from the center of the articular surface of the humeral head as a new refer

Editorial Commentary: Shoulder Bankart and Latarjet Procedures Show Different Complications at Different Time Periods.

In patients with shoulder instability, arthroscopic Bankart procedure results in a high recurrence rate if there is associated bone loss, even small-to-moderate bone loss. The Latarjet procedure mitigates against recurrence in such cases but has greater short-term (30-day) complications than an arthroscopic Bankart and a greater rate of revision or reoperation. In truth, short-term complications are low with both procedures. That said, complications after arthroscopic Bankart are less likely to manifest in the short term, and possible future failure is generally not discovered until after a pa

Arthroscopic Management of the Contact Athlete with Anterior Instability.

Anterior glenohumeral instability is one of the most common injuries suffered from sport. Despite padding and conditioning, the shoulder joint remains particularly vulnerable to injury, especially in the setting of contact. The overall rate of anterior instability is reported to be 0.12 injuries per 1000 athlete exposures, although this is increased up to 0.40 to 0.51 in the contact athlete. Successful treatment requires consideration of restoring stability while minimizing loss of glenohumeral motion. Common treatment strategies involve addressing the pathology that results from anterior shou…

Clinical Outcomes, Union Rates, and Complications of Screw Versus Button Fixation in the Bristow-Latarjet Procedure for Anterior Shoulder Instability: A Systematic Review and Meta-Analysis.

BACKGROUND: The Latarjet procedure is a common procedure for treating critical glenoid bone loss in anterior shoulder instability. Implants such as the screw and cortical button are widely used. The aim of this study was to compare studies on screw versus button fixation techniques in the Bristow-Latarjet procedure for anterior shoulder instability in terms of clinical outcomes, union rates, and complications. METHODS: The PubMed, Scopus, and Embase databases were searched to find comparative studies that reported outcomes of using screw versus button fixation in the Bristow-Latarjet procedur

Editorial Commentary: Urgency Toward Arthroscopic Bankart Repair Is Essential for Anterior Shoulder Dislocation: You Don't Have to Fix After the First Dislocation, but Definitely Before the Second!

While still hotly debated, primary arthroscopic management of the first-time anterior shoulder dislocation has an extensive list of known benefits: lower overall health care costs, improved patient-reported outcomes, a vast reduction in secondary instability, and higher quality-of-life measures. Yet, despite these meaningful contributions to health care quality, we continue to bypass the predictable success of an acute arthroscopic Bankart repair in order to tempt fate with "a trial" of nonoperative care for our young, high-risk collision athletes. Whether for the in-season athlete, the "early

Defining Critical Glenoid Bone Loss in Posterior Shoulder Capsulolabral Repair.

BACKGROUND: Although critical bone loss for anterior instability is well defined, a clinically significant threshold of posterior bone loss has not been elucidated. HYPOTHESIS: Patients with failed arthroscopic posterior shoulder capsulolabral repair will have increased posterior glenoid bone loss with a defined critical threshold. STUDY DESIGN: Case control study; Level of evidence, 3. METHODS: Athletes older than 18 years with unidirectional posterior instability treated with arthroscopic repair were evaluated at 2-year minimum follow-up. Failure was defined as revision surgery, American

Editorial Commentary: A Functional Capsule, Not the Type of Bone Graft, Determines Outcome After Shoulder Stabilization in Cases of Anterior Glenoid Bone Deficiency.

Many clinical results of surgical treatment of recurrent anterior shoulder instability confirm the importance of anterior bone grafting of the glenoid defect. Some studies even propose to perform a graft, even when there is no bony defect. Short- and middle-term studies report comparable results between bone grafting and Latarjet procedure. But one of the main questions that still remains is the quality and efficiency of the capsule. If Latarjet is the gold standard in anterior shoulder stabilization with very good results at very long follow-up, it is probably because the capsule is replaced

Editorial Commentary: We Need to Customize Surgical Treatment When Treating Patients With Recurrent Anterior Shoulder Instability.

Surgical repair of shoulder instability is challenging, and multiple procedures have been proposed. In an attempt to reduce risk of recurrence following surgical reconstruction, some surgeons have added steps to prior arthroscopic procedures, and other surgeons have selected a bone reinforcement procedure. These additional augmented repair techniques have reduced the risk of postoperative recurrence, but introduced additional risk of complications related to hardware, fixation, and possible need for additional surgery. Surgeons should become familiar with multiple surgical procedures to treat

Posterior Shoulder Instability.

Posterior shoulder instability is a relatively uncommon condition, occurring in ∼10% of those with shoulder instability. Because of the rarity of the condition and the lack of knowledge in treatment, it is often misdiagnosed or patients experience a delay in diagnosis. Posterior instability typically affects athletes participating in contact or overhead sports and is usually the result of repetitive microtrauma or blunt force with the shoulder in the provocative position of flexion, adduction, and internal rotation, leading to recurrent subluxation events. Acute traumatic posterior dislocation

The Epidemiology of Glenohumeral Joint Instability: Incidence, Burden, and Long-term Consequences.

Acute traumatic glenohumeral joint instability is common within the general population and endemic in young athletes and physically active populations. Individuals who experience acute traumatic glenohumeral joint instability events are at high risk of recurrent injury. In addition, glenohumeral joint instability results in substantial short-term and long-term consequences, including significant time loss from activity, degenerative joint changes, and reduced quality of life. This review provides an overview of the epidemiology of glenohumeral joint instability, reviews the modifiable and nonm

[Anterior glenoid rim defects of the shoulder].

Bony instability of the shoulder due to glenoid defects has recently received increasing attention. Glenoid defects can be divided into acute fragment-type lesions (type I), chronic fragment-type lesions (type II) and glenoid bone loss without a bony fragment (type III). The diagnosis and classification are mainly based on imaging methods including a radiographic instability series and/or computed tomography. The management of anterior glenoid rim lesions depends on many factors including the clinical presentation, type of lesion, concomitant pathology as well as age and functional demands of
