Respiratory and sleep effects of remifentanil in volunteers with moderate obstructive sleep apnea.

BACKGROUND: There is concern that opioid-based analgesia will worsen sleep-related respiratory insufficiency in patients with obstructive sleep apnea (OSA), resulting in serious morbidity or mortality. However, there are no studies that directly address the merit of this concern. Consequently, the authors designed this study as the first prospective, double-blind, placebo-controlled investigation of opioid pharmacology in patients with documented OSA. METHODS: Patients (n = 19) with moderate OSA documented by polysomnography (sleep study) were randomized to undergo an additional sleep study w

nCPAP treatment of obstructive sleep apnea increases slow wave sleep in prefrontal EEG.

According to standard sleep stage scoring, sleep EEG is studied from the central area of parietal lobes. However, slow wave sleep (SWS) has been found to be more powerful in frontal areas in healthy subjects. Obstructive sleep apnea syndrome (OSAS) patients often suffer from functional disturbances in prefrontal lobes. We studied the effects of nasal Continuous Positive Airway Pressure (nCPAP) treatment on sleep EEG, and especially on SWS, in left prefrontal and central locations in 12 mild to moderate OSAS patients. Sleep EEG was recorded by polysomnography before treatment and after a 3 mont

[A case of obstructive sleep apnea syndrome with increased lung water during sleep].

A 22-year-old man was admitted to the department of plastic surgery in our university with diagnosis of ankylosis and microgeniea. His complaints were snoring and somnolence in the daytime, so we performed a Respigraph (Chest) that showed obstructive apnea during sleep. We diagnosed obstructive sleep apnea syndrome. So we observed pulmonary circulation and measured lung water using the double indicator dilution method (Nihon Koden, MTV-1100) during sleep. During the period of apnea, arterial oxygen pressure and mixed venous saturation decreased, reversely, mean pulmonary arterial pressure rose

Sleep Disturbances and Male Reproductive Dysfunction: Pathophysiological Mechanisms Linking Obstructive Sleep Apnea and Sleep Deprivation.

BACKGROUND: Sleep disturbances, particularly obstructive sleep apnea (OSA) and sleep deprivation, are critical yet underrecognized contributors to male reproductive dysfunction. Both conditions disrupt endocrine homeostasis, reducing testosterone and luteinizing hormone levels, which are central regulators of spermatogenesis. OBJECTIVE: This review aimed to synthesize current evidence on the mechanisms linking OSA and sleep deprivation to impaired male fertility, with emphasis on endocrine, oxidative, and inflammatory pathways. METHODS: We conducted a structured, non-systematic narrative rev

Recurrence of sleep apnea syndrome following tracheostomy. A shift from obstructive to central apnea.

This report describes an unusual case of severe obstructive sleep apnea and alveolar hypoventilation leading to hypersomnolence and cor pulmonale, which were corrected by tracheostomy. Four years later, after a 22.5-kg weight gain, nocturnal apneas of similar frequency, duration, and depth of desaturation reappeared but were totally central in origin. The central apneas were eliminated with home nocturnal positive-pressure ventilation via cuffed tracheostomy tube. Each time the patient's apneas were corrected (obstructive: tracheostomy; central: mechanical ventilation), daytime alveolar hypove

Association between obstructive sleep apnea and metabolic profiles across sleep stages in pediatrics.

OBJECTIVE: To evaluate the association between metabolic profiles and obstructive sleep apnea (OSA) during rapid eye movement (REM) and non-REM (NREM) sleep in children. METHODS: A total of 514 children underwent overnight polysomnography (PSG) and biochemical testing. Obstructive apnea hypopnea indices (OAHI) and oxygen desaturation indices (ODI) were recorded for total sleep time (TST), REM, and NREM. A multivariate analysis of covariance model assessed the associations of OAHITST, OAHINREM, and OAHIREM with overall metabolic parameters, adjusting for body mass index (BMI) z-score and other

Combined impact of obstructive sleep apnea and periodic limb movements on sleep parameters.

STUDY OBJECTIVES: This study examines the combined effects of Obstructive Sleep Apnea (OSA) and Periodic Limb Movements of Sleep (PLMS) on sleep parameters. Specifically, it aims to determine whether their co-occurrence exacerbates sleep fragmentation, reduces sleep efficiency, and alters restorative sleep stages compared to either condition alone. METHODS: We retrospectively analyzed 581 adults undergoing polysomnography at UCSF Fresno. Subjects were categorized into three groups: OSA, PLMS, and OSA and PLMS. Sleep metrics, including Total Sleep Time (TST), Wake After Sleep Onset (WASO), and

Is sleep bruxism in obstructive sleep apnea only an oral health related problem?

BACKGROUND: The etiology of sleep bruxism in obstructive sleep apnea (OSA) patients is not yet fully clarified. This prospective clinical study aimed to investigate the connection between probable sleep bruxism, electromyographic muscle tone, and respiratory sleep patterns recorded during polysomnography. METHODS: 106 patients with OSA (74 males, 31 females, mean age: 56.1 ± 11.4 years) were divided into two groups (sleep bruxism: SB; no sleep bruxism: NSB). Probable SB were based on the AASM criteria: self-report of clenching/grinding, orofacial symptoms upon awakening, abnormal tooth wear a

Sleep Endoscopy Patterns in Six Adults With Trisomy 21 and Obstructive Sleep Apnea.

Obstructive sleep apnea (OSA) is prevalent amongst individuals with Trisomy 21 (T21). This case series describes the results of drug-induced sleep endoscopy of six adult patients with OSA and T21 and compares the patterns of collapse to those observed in adults without T21. Predominantly hypopharyngeal airway collapse was found in four of the six (66.7%) subjects with T21, a much higher proportion than in the general population. This finding may implicate anatomic differences underlying the increased prevalence of OSA in individuals with T21 and may have treatment implications. Laryngoscope, 1

The role of sleep endoscopy in oral appliance therapy for obstructive sleep apnea.

OBJECTIVE: Although oral appliance therapy is considered a validated treatment for obstructive sleep apnea, its therapeutic success varies significantly among patients. Drug-induced sleep endoscopy is often employed in order to identify candidates for upper airway surgery; however, it remains unknown whether its findings can be associated with success of oral appliance therapy. This study tested the hypothesis that drug-induced sleep endoscopy variables can predict the outcome of oral appliance therapy in obstructive sleep apnea patients. METHODS: Forty-nine obstructive sleep apnea patients [

Sleep hygiene-related conditions in patients with mild to moderate obstructive sleep apnea.

OBJECTIVE: Sleep hygiene-related conditions are factors that affect the symptoms experienced by patients with obstructive sleep apnea (OSA). However, very few studies have investigated the association between sleep hygiene and sleep symptoms, especially in patients with mild or moderate OSA. This study evaluated the relationship between factors related to sleep hygiene and clinical symptoms in patients with mild to moderate OSA. METHODS: One hundred and seventy-four patients who visited the Sleep Breathing Disorder Center at a tertiary academic center to evaluate suspected OSA were included.

Sleep-Dependent Directional Coupling of Cardiorespiratory System in Patients With Obstructive Sleep Apnea.

OBJECTIVE: Cardiorespiratory interactions have been widely investigated in different physiological states and conditions. Various types of coupling characteristics have been observed in the cardiorespiratory system; however, it is difficult to identify and quantify details of their interaction. In this study, we investigate directional coupling of the cardiorespiratory system in different physiological states (sleep stages) and conditions, i.e., severity of obstructive sleep apnea (OSA). METHODS: Directionality analysis is performed using the evolution map approach with heartbeats acquired fr

Sleep-Related Rhythmic Movement Disorder and Obstructive Sleep Apnea in Five Adult Patients.

Sleep-related rhythmic movements (SRRMs) are typical in infancy and childhood, where they usually occur at the wake-to-sleep transition. However, they have rarely been observed in adults, where they can be idiopathic or associated with other sleep disorders including sleep apnea. We report a case series of 5 adults with sleep-related rhythmic movement disorder, 4 of whom had a previous history of SRRMs in childhood. SRRMs mostly occurred in consolidated sleep, in association with pathological respiratory events, predominantly longer ones, especially during stage R sleep, and recovered in 1 pat
