Two cases of exploding head syndrome documented by polysomnography that improved after treatment.

Exploding head syndrome is a rare sleep disorder, characterized by an explosive feeling in the head, that occurs during the sleep-wake transition. Usually the attacks are painless, but the fear caused by the attack can result in awakening and insomnia when it is generated frequently. It has been suspected that exploding head syndrome is related to emotional stress, because most patients report stressful life situations in periods when attacks are intense and frequent. The benign character and good prognosis of exploding head syndrome are the most likely reasons why it has not become a subject

Dream enactment behavior: review for the clinician.

Dream enactment behavior commonly occurs on occasion in normal children and adults. Disruptive and frequent dream enactment behavior may come to the attention of the clinician either as the primary reason for consultation or as a prominent characteristic of a patient with other sleep disorders. Questioning patients with chronic neurologic and psychiatric disorders may also reveal previously unrecognized behavior. In the absence of sleep pathology, process of dream enactment likely begins with active, often emotionally charged dream content that may occasionally break through the normal REM sle

Hypocretin/Orexin Receptor Pharmacology and Sleep Phases.

The hypocretins/orexins are two excitatory neuropeptides, alternately called HCRT1 or orexin-A and HCRT2 or orexin-B, that are the endogenous ligands for two G-protein-coupled receptors, HCRTR1/OX1R and HCRTR2/OX2R. Shortly after the discovery of this system, degeneration of hypocretin/orexin-producing neurons was implicated in the etiology of the sleep disorder narcolepsy. The involvement of this system in a disorder characterized by the loss of control over arousal state boundaries also suggested its role as a critical component of endogenous sleep-wake regulatory circuitry. The broad projec

Impact of Astroglial Connexins on Modafinil Pharmacological Properties.

STUDY OBJECTIVES: Modafinil is a non-amphetaminic wake-promoting compound used as therapy against sleepiness and narcolepsy. Its mode of action is complex, but modafinil has been recently proposed to act as a cellular-coupling enhancer in glial cells, through modulation of gap junctions constituted by connexins. The present study investigated in mice the impact of connexins on the effects of modafinil using connexin inhibitors. METHODS: Modafinil was administered alone or combined with inhibitors of astrocyte connexin, meclofenamic acid, or flecainide, respectively, acting on Cx30 and Cx43. S

Interventions to improve the sleep quality of adults with personality disorder: A systematic review.

Poor quality sleep is common for people who have a diagnosis of personality disorder (PD). Core cognitive and behavioral features of PD may cause and perpetuate poor sleep, but to date, no review has collated the evidence on the efficacy of interventions to improve sleep quality for people with PD. Structured searches for interventional studies among adults with PD and reporting validated measures of sleep quality were conducted up to November 2022 in multiple databases. Single-case reports were excluded. Study quality was assessed with standardized risk of bias tools. Unreported data was soug

A series of 8 cases of sleep-related psychogenic dissociative disorders and proposed updated diagnostic criteria.

STUDY OBJECTIVES: To identify the most relevant clinical and video-polysomnographic characteristics of patients with sleep-related dissociative disorders (SRDDs) and to propose a framework for new diagnostic criteria. METHODS: We searched potential SRDD cases from the scientific literature and from a database of patients referred for clinical and video-polysomnographic assessment in a single sleep disorders center for disruptive nocturnal behaviors (n = 731). The most relevant clinical and neurophysiological characteristics of the cases were extracted and a descriptive analysis was performed.

The sleep phenotype of Borderline Personality Disorder: A systematic review and meta-analysis.

AIM: To delineate the sleep profile of Borderline Personality Disorder (BPD). METHOD: A meta-analysis to synthesise findings on the objective and subjective sleep characteristics of BPD. RESULTS: We identified 32 studies published between 1980 and December 2015. Meta-analysis indicated significant differences between BPD and healthy control groups across objective sleep continuity (sleep onset latency, total sleep time, sleep efficiency) and architecture (rapid eye movement latency/density, slow wave sleep) measures, and self-reported sleep problems (nightmares, sleep quality). Findings were

The role of sleep and wakefulness in the genesis of depression and mania.

Disturbances of the sleep-wake cycle are frequently seen in affective illness and are exhibited in other psychiatric illness as well. In addition to being a useful research probe, manipulations of the sleep-wake cycle such as sleep deprivation (SD) and phase advance can cause depression to remit and thus can be used as alternative or as adjunctive to pharmacologic treatment. The antidepressant response to SD occurs whether antidepressant drugs are administered or not. However, there is some evidence that the concomitant use of antidepressants may prevent the relapses that occur after recovery

Acupuncture-related peripheral stimulation as a candidate neuromodulatory model of insomnia: clinical evidence, sleep-wake circuitry, and translational limits.

Acupuncture-related peripheral stimulation is often discussed as a non-pharmacological option for insomnia disorder, but its relevance to sleep neuroscience depends on more than improvement in sleep questionnaires. This critical narrative Review evaluates whether acupuncture and related acupoint-based stimulation methods can serve as testable peripheral models of sleep-wake and hyperarousal circuitry. Across clinical studies, the most consistent signal is improvement in subjective sleep quality and insomnia severity in selected populations. Evidence for objective sleep architecture, multiday s

Rats exposed to chronic alcohol display protracted insomnia and daytime sleepiness-like behavior during alcohol withdrawal✰.

INTRODUCTION: Alcohol use disorder (AUD), a chronic brain disorder, is characterized by a multitude of symptoms, including insomnia, during withdrawal. Previously, we have shown that rats exposed to chronic alcohol displayed insomnia-like symptoms during acute withdrawal. Since insomnia lasts for several years and is a major risk factor of relapse to alcoholism, the present study is designed to investigate the long-term effects of alcohol withdrawal on sleep-wakefulness. METHODS: Adult male Sprague-Dawley rats, instrumented with sleep recording electrodes, were divided into two groups: Alcoho

Understanding Sleep Regulation in Normal and Pathological Conditions, and Why It Matters.

Sleep occupies a peculiar place in our lives and in science, being both eminently familiar and profoundly enigmatic. Historically, philosophers, scientists and artists questioned the meaning and purpose of sleep. If Shakespeare's verses from MacBeth depicting "Sleep that soothes away all our worries" and "relieves the weary laborer and heals hurt minds" perfectly epitomize the alleviating benefits of sleep, it is only during the last two decades that the growing understanding of the sophisticated sleep regulatory mechanisms allows us to glimpse putative biological functions of sleep. Sleep con

Sleep features and long-term incident neurodegeneration: a polysomnographic study.

STUDY OBJECTIVES: Sleep is altered early in neurodegenerative diseases (NDDs) and may contribute to neurodegeneration. Long-term, large sample-size studies assessing NDDs association with objective sleep measures are scant. We aimed to investigate whether video-polysomnography (v-PSG)-based sleep features are associated with long-term NDDs incidence. METHODS: Retrospective cohort study of patients referred 2004-2007 to the Sleep Disorders Unit, Neurology, Medical University Innsbruck, Austria. All patients ≥ 18 years undergoing v-PSG and without NDDs at baseline or within 5 years were include

Deep Learning-Based Assessment of Sleepiness from Fingertip Pulse Wave Analysis During Objective Daytime Testing.

PURPOSE: Objective evaluation of daytime sleepiness is a key factor in the management of sleep disorders. However, objective testing is limited due to the labor-intensive nature of standard methods; the Multiple Sleep Latency Test (MSLT) and the Maintenance of Wakefulness Test (MWT). Deep learning analysis of a simple fingertip pulse wave (photoplethysmography, PPG) recording could be a viable option for the routine assessment of sleepiness in patients with suspected sleep disorders. METHODS: A deep learning-based sleep staging model was tested in MSLT (n = 143) and MWT (n = 127) datasets. Th

Trauma exposure and sleep: using a rodent model to understand sleep function in PTSD.

Post-traumatic stress disorder (PTSD) is characterized by intrusive memories of a traumatic event, avoidance behavior related to cues of the trauma, emotional numbing, and hyper-arousal. Sleep abnormalities and nightmares are core symptoms of this disorder. In this review, we propose a model which implicates abnormal activity in the locus coeruleus (LC), an important modifier of sleep-wake regulation, as the source of sleep abnormalities and memory abnormalities seen in PTSD. Abnormal LC activity may be playing a key role in symptom formation in PTSD via sleep dysregulation and suppression of

Interventions for Migraine and Sleep: A Systematic Review Exploring Their Bidirectional Association.

BACKGROUND: Migraine and sleep disturbances share a bidirectional relationship, influencing each other's frequency and severity. The aim of this systematic review is to examine the effects of migraine-targeted interventions on both migraine outcomes and sleep parameters (including sleep quality and insomnia symptoms), as well as the effects of sleep-focused interventions on both sleep and migraine outcomes. METHODS: Following PRISMA 2020 guidelines, a systematic search was conducted across six databases (PubMed, Medline, Scopus, Embase, PsycINFO, CINAHL) for studies published until December 5

Sleep disturbances in early clinical stages of psychotic and bipolar disorders: A meta-analysis.

OBJECTIVE: To provide a qualitative view and quantitative measure of sleep disturbances across and between early stages - clinical ultra high-risk and first episode - of psychotic and bipolar disorders. METHODS: Electronic databases (PubMed, Cochrane, Embase, PsychINFO) were searched up to March 2021 for studies comparing sleep measures between individuals with an early stage and controls. Standard mean deviations (Cohen's d effect sizes) were calculated for all comparisons and pooled with random-effects models. Chi-square tests were used for direct between-subgroups (ultra high-risk vs first

Disrupted sleep-wake cycles and circadian rhythms in a Drosophila model of C9orf72-FTD.

Frontotemporal dementia (FTD) is a neurodegenerative disorder that affects behavior, personality, motor activity, speech, cognition, and sleeping patterns. Previous findings support the idea that disruption of sleep and circadian systems may not only be affected by this disease but also work to actively shape the clinical phenotype of FTD. Thus, understanding how sleep-wake cycles are altered may provide insight into mechanisms that influence both disease progression and quality of life. We studied an established Drosophila model of FTD to investigate changes in the sleep-wake cycle of both yo
