Language dysfunction and related amyloid-β in Alzheimer's disease mediated by brain functional connectivity changes.

BACKGROUND: Language dysfunction occurs early in Alzheimer's disease (AD) and whether amyloid-β pathology is related to language dysfunction remains unclear. Functional connectivity (FC) in language networks is critical for language function. We hypothesize that altered FCs in fronto-temporal regions (core language areas) mediate the association between amyloid-β burden and language impairment. METHODS: A total of 110 individuals were recruited including 44 cognitively unimpaired individuals, 24 mild cognitive impairment patients and 42 AD patients. The acoustic features and semantic content

An evaluation of reading comprehension of expository text in adults with traumatic brain injury.

PURPOSE: This project was conducted to obtain information about reading problems of adults with traumatic brain injury (TBI) with mild-to-moderate cognitive impairments and to investigate how these readers respond to reading comprehension strategy prompts integrated into digital versions of text. METHOD: Participants from 2 groups, adults with TBI (n = 15) and matched controls (n = 15), read 4 different 500-word expository science passages linked to either a strategy prompt condition or a no-strategy prompt condition. The participants' reading comprehension was evaluated using sentence verifi

How We Fail Children With Developmental Language Disorder.

Purpose For over two decades, we have known that children with developmental language disorder (DLD) are underserved. We have also known that DLD does not attract the research attention that it merits given its prevalence and impact. The purposes of this clinical focus article are to present evidence that these failures continue, explore the reasons behind these failures, and propose solutions. Method I reviewed the literature and applied bibliometric analysis procedures from Bishop (2010) to quantify research efforts aimed at DLD compared to other neurodevelopmental disorders. Results The per

Acoustic analysis of tracheo-oesophageal voice: cepstral and voicing measures.

OBJECTIVES: To develop an acoustic analysis protocol examining the correlations between acoustic and perceptual parameters in tracheo-oesophageal (TE) voices. METHODS: Forty-nine laryngectomised patients with TE voice (42 M, 7 F; mean age 68 ± 9.3 years) were enrolled. Voice samples holding the sustained vowel /a/ and the Consensus Auditory-Perceptual Evaluation of Voice (CAPE-V) sentences were recorded and analysed to extract the fundamental frequency (F0), the fraction of locally unvoiced frames (FLUF) and the cepstral peak prominence smoothed (CPPS). A perceptual evaluation was performed w

Preliminary Psychometric Review of Neurologists' Speech Ratings on the Unified Parkinson's Disease Rating Scale.

PURPOSE: The purpose of this study was to conduct a preliminary psychometric evaluation of the speech item of the Movement Disorder Society-sponsored revision of the Unified Parkinson's Disease Rating Scale (MDS-UPDRS) Part III. METHOD: Twenty neurologists specializing in movement disorders evaluated the speech-unscripted and sentence reading-of people with Parkinson's disease (PD) on the MDS-UPDRS Part III. The concurrent validity of the speech item was evaluated by comparing the ratings to a previously collected estimate of the speakers' intelligibility during sentence reading. The reproduc

Ultrasound as visual feedback in speech habilitation: exploring consultative use in rural British Columbia, Canada.

Ultrasound has shown promise as a visual feedback tool in speech therapy. Rural clients, however, often have minimal access to new technologies. The purpose of the current study was to evaluate consultative treatment using ultrasound in rural communities. Two speech-language pathologists (SLPs) trained in ultrasound use provided consultation with ultrasound in rural British Columbia to 13 school-aged children with residual speech impairments. Local SLPs provided treatment without ultrasound before and after the consultation. Speech samples were transcribed phonetically by independent trained l

Challenges in Service Provision to Children and Families from Linguistically Diverse Backgrounds.

The purpose of this study was to examine and describe experiences and perceptions of speech-language pathologists (SLPs) serving children and families from culturally and linguistically diverse populations. Twenty SLPs were interviewed using an online audio platform regarding common practices, resources and supports, challenges, and communication. Participants' responses to the semi-structured questions were transcribed and analyzed to identify themes in experiences. Results suggested six overall themes including: considerations for assessment; cultural impact; linguistic access and barriers;

Look at Mummy: challenges in training parents to deliver a home treatment program for childhood apraxia of speech in a rural Canadian community.

INTRODUCTION: Children requiring speech pathology services in rural and remote locations face many barriers in accessing adequate services. This has particular consequences for children who need intensive treatment for therapy to be effective, such those with childhood apraxia of speech (CAS). Parent training has been used to overcome speech pathology service delivery barriers for a range of other communication disorders. However, the effectiveness of training parents to deliver a motor-based treatment for CAS within rural and remote contexts has not been evaluated. This study examined the eff

EmotionaL and Language Recovery in Aphasia (ELLA): Codesigning and Proof-of-Concept Testing a Novel Intervention.

PURPOSE: People with aphasia are at risk of becoming depressed, anxious, and isolated. They want therapy to focus on improving communication, confidence, and finding ways to live well; family members want to be involved in the therapy process. We aimed to codesign and test a novel intervention (EmotionaL and Language recovery in Aphasia [ELLA]) that would address these priorities through integrating two evidence-based therapy approaches: elaborated semantic feature analysis (SFA), a naming therapy, and solution-focused brief therapy (SFBT), a psychological therapy. METHOD: A series of worksho

Expectations and experiences of parents taking part in parent-child interaction programmes to promote child language: a qualitative interview study.

BACKGROUND: Parent-child interaction therapies are commonly used by speech and language therapists (SLTs) when providing services to young children with language learning difficulties. However, the way parents react to the demands of such interventions is clearly important, especially for those from socially disadvantaged backgrounds. Parents play a central role in the therapy process so to ensure parent engagement, and to maximize intervention effectiveness, parents' views must be considered. AIMS: To explore the expectations and experiences of parents from socially disadvantaged backgrounds

Accessible decision-making capacity evaluations for patients with aphasia: a quality improvement project to develop, implement, and evaluate an online training module for interdisciplinary healthcare providers.

PURPOSE: Decision-making capacity (DMC) is the ability to make informed, autonomous choices. In healthcare settings, a range of providers typically evaluate DMC using a semi-structured interview approach. The presence of aphasia introduces a significant challenge to DMC assessments because a patient's communication challenges can mask their underlying capacity. This quality improvement project aimed to (1) train providers to integrate evidence-based communication accommodations with support from a speech-language pathologist (SLP) to reduce the risk of underestimating capacity during a DMC ass

A Survey of the Management of Submucous Cleft Palate Among Surgeons and Speech Pathologists in Australia and New Zealand.

BackgroundSubmucous cleft palate (SMCP) is a rare condition with a variable symptomatology. While in overt cleft palate, earlier repair is associated with superior speech outcomes, this is not always possible as many patients with SMCP present at an older age. There is controversy in how patients with SMCP who present at a young age should be treated because it is difficult to differentiate which patients will be symptomatic and which will be asymptomatic later in life. Because of this, we anticipate that even within a small population geographical region such as Australia and New Zealand, tha
