[Changes of neural electrophysiology properties of cauda equina in experimental lumbar spinal canal stenosis under dynamic burden].

OBJECTIVE: To study the changes of neural electrophysiology properties of cauda equina under double level compression and dynamic burdens, and to clarify the mechanisms of intermittent neurogenic claudication. METHODS: Thirty SD rats were divided into 5 groups (6 in each group). The laminectomy of L5 was performed in control group. In the experimental groups, the silicon sheets were inserted into the spinal canal of L4 and L6 to cause double level compression of cauda equina by 30%. Two hours after onset of compression, no dynamic burden was introduced in experimental group 1. Only high frequ…

Patterns of chronic adhesive arachnoiditis following Myodil myelography: the significance of spinal canal stenosis and previous surgery.

109 patients who had undergone Myodil myelography on at least one occasion were identified. The patterns of lumbar nerve root distribution in this group were examined using magnetic resonance imaging. The relationship between these patterns and the presence of spinal stenosis or previous surgery was investigated. Chronic adhesive arachnoiditic nerve root patterns were seen in 68 patients and were classified into three groups according to Delemarter et al. Central clumping of nerve roots (type 1) and complete opacification of the thecal sac (type 3), extending over at least one vertebral level,

Readmission rates and hospital charges: a comparative study of surgical interventions in degenerative spondylolisthesis and spinal canal stenosis.

OBJECTIVE: Degenerative spondylolisthesis and spinal canal stenosis are some of the more common clinical conditions associated with low back pain, with various surgical techniques available, ranging from standalone decompression to fusion. Interspinous spacer devices (ISD) have emerged as an intermediary surgical option. The aim was to compare 90-day all-cause readmission rates between techniques, with secondary outcomes including total hospital charges and postoperative complications. METHODS: Utilizing the 2020 Nationwide Readmissions Database (NRD), adult patients (> 18 years) were selecte…

The relationship between the ratio of interpedicular distance increase and the ratio of spinal canal compromise in thoracolumbar burst fractures.

BACKGROUND: The aim of the study was to investigate the relationship between the interpedicular distance increase ratio and the ratio of canal compromise in thoracolumbar burst fractures. METHODS: Thirty-one patients (18 male and 13 female) with an average age of 30.8 (14-57) who had been treated for thoraco-lumbar burst fractures in the Department of Orthopaedics and Traumatology were included in the study. The initial anteroposterior radiographs of the patients were used to calculate the increase ratio of interpedicular distance (both from medial-to-medial and from center-to-center). The ar

The Change of Spinal Canal According to Oblique Lumbar Interbody Fusion in Degenerative Spondylolisthesis: A Prospective Observational Study.

OBJECTIVE: Oblique lumbar interbody fusion (OLIF) involves inserting large cages into the interbody disc space. This expands the spinal canal and neural foramen by stretching the ligament flavum and releasing the facet joint, resulting in indirect neural decompression. Our objective was to investigate the changes in the spinal canal and ligament flavum over time after OLIF. METHODS: This was a prospective observational study involving 30 patients who underwent OLIF L4-5 between 2015 and 2018. In total, 27 of the 30 patients underwent preoperative, early follow-up ( < 5 days), and late follow-…

Factors Associated with Thickening of the Ligamentum Flavum on Magnetic Resonance Imaging in Patients with Lumbar Spinal Canal Stenosis.

STUDY DESIGN: Experimental study of the ligamentum flavum (LF) thickness among patients with lumbar spinal canal stenosis (LSCS). OBJECTIVES: To elucidate the factors associated with thickening of the LF on magnetic resonance imaging (MRI). SUMMARY OF BACKGROUND DATA: Thickening of the LF is a major contributor to LSCS. This thickening is attributed to tissue hypertrophy or buckling of the ligament, and there may be several associated factors on MRI; however, these factors remain unclear. METHODS: We studied the LF in 56 patients (a total of 106 ligaments) with LSCS, who underwent decompres…

Spinal canal stenosis in children with achondroplasia: the role of augmentation laminoplasty-a 15-year single institution experience.

PURPOSE: Achondroplasia typically results in compressive spinal canal stenosis in one-third of children, but rare under the age of 15 years. Laminectomy is the mainstay of treatment but this leads to instability and progressive deformity requiring complex fixation. In order to reduce that risk, we developed a novel modified augmented laminoplasty that increases spinal canal diameter while preserving the posterior column stability. METHODS: All laminoplasty cases for spinal compressive achondroplasia from 2006 to 2020 were included. Ten augmentation laminoplasty procedures were performed in 7 …

Spinal canal decompression for hypertrophic neuropathy of the cauda equina with chronic inflammatory demyelinating polyradiculoneuropathy: A case report.

BACKGROUND: Hypertrophic neuropathy of the cauda equina (HNCE) is a rare disease, especially in children. It can be caused by different etiological agents such as inflammation, tumor or hereditary factors. Currently, there is no uniform standard for clinical treatment of HNCE. Furthermore, it is unclear whether spinal canal decompression is beneficial for patients with HNCE. CASE SUMMARY: We report the case of a 13-year-old boy with enlargement of the cauda equina. The onset of the disease began at the age of 6 years and was initially marked by radiating pain in the buttocks and thighs after …

Congenital Spinal Canal Stenosis with Ossification of the Ligamentum Flavum in an Achondroplastic Patient: A Case Report and Literature Review.

Achondroplasia has an effect on intracartilaginous ossification during the development of the spine resulting in a narrow spinal canal. This abnormal anatomy could make an achondroplastic patient tend to have spinal canal stenosis. We reported a case of congenital spinal canal stenosis with achondroplasia combined with ossified ligamentum flavum (OLF) at the thoracolumbar and lumbar spine, which was treated by decompressive surgery. We reported a 52-year-old Thai male with achondroplasia presented with progressive myelopathy and neurogenic claudication due to spinal canal stenosis. Spinal cana…

A newly developed drill with a polished tip for the anterior cervical approach in spinal canal stenosis: a technical note.

PURPOSE: The goal of our efforts was to develop a modified drill that allows a safe milling of the dorsal osteophytes located close to the dura. METHODS: Usually a normal high-speed drill is used for the resection of uncarthrotic and spondylophytic bone. In one patient, we used our new high-speed drill close to the dura in an area, which is not easy reachable with a normal drill or a rongeur. RESULTS: Employing the new drill with a polished tip, the spine surgeon is able to overcome the anatomical restrictions, which he has to face when the anterior approach to the cervical spine. The resect

Congenital stenosis of lumbar spinal canal: comparison of results of surgical treatment for this and other causes of lumbar syndrome.

The operative results in 37 consecutive patients suffering from developmental stenosis of the lumbar spinal canal, compared with those in spondylosis and disc herniations, are discussed. The diagnostic certainty of stenosis, suspected on the bases of clinical and radiographic data, is reached only at the operating table. To judge the usefulness of the operation we have considered not only the patients' verdicts, but also the improvements in neurological signs and the appearance of new deficits. Satisfactory results are around 80%, slightly less good with stenosis than with the other causes of …

Congenital spondylolysis of cervical vertebrae with spondylolisthesis anf frontal narrowing of the spinal canal. Report of two cases.

Two cases of cervical spondylolisthesis, due to a congenital defect in the pars interarticularis of C6, associated with spina bifida and elongation of the contralateral pars interarticularis, are reported. Tomography showed an abnormal direction of the articular facets of C6 and demonstrated a narrow frontal diameter of the cervical spinal canal at the level of the malformation. Gas myelography showed a slight compression of the cord. Unlike previously reported cases the patients had neurological findings which, in one case at least, were certainly referable to the spinal cord.
