A clinician's view of spinal cord injury.

The primary cause of spinal cord injury (SCI) is automobile collisions, followed by violence, falls, and injuries in sporting events. The patient is most frequently a young male. Regardless of cause and age, SCI is a potentially catastrophic injury. The unique anatomical relationship of the spinal cord, being enclosed in the dural sac within the bony vertebral column, make it venerable to a wide range of traumatic insults. SCI is classified as complete or incomplete with several subclasses arranged under each of these respective headings. The probability of recovery to a functional state is us…

Re-irradiation of the human spinal cord.

PURPOSE: Experimental animal data give evidence of long-term recovery of the spinal cord after irradiation. By extrapolation of these data, re-irradiation regimens were designed for eight patients who required palliative radiotherapy. As a consequence of re-irradiation, their spinal cords were exposed to cumulative doses exceeding the tolerance dose. Radiobiological and clinical data are presented. PATIENTS AND METHOD: Eight patients were re-irradiated on the cervical (n = 1), thoracic (n = 5) and lumbar (n = 2) spinal cord. The time interval between the initial and re-treatment ranged from 4…

Anatomy of the lumbosacral spinal cord in rabbits.

This study provides an accurate, anatomical description of the rabbit's lumbosacral spinal cord. Following humane euthanasia, 64 rabbits were radiographed and dissected. The number of thoracic (T) and lumbar (L) vertebrae was 12T/7L in 43.8%, 13T/6L in 32.8%, and 13T/7L in 23.4% in the 64 rabbits studied. The 13th ribs were rudimentary, asymmetric, or both in 15 animals. The spinal cord terminated within the second sacral vertebra (S2) in 79.3%, within the first sacral vertebra (S1) in 19.0%, and within the third sacral vertebra (S3) in 1.7% of the sample. Spinal nerve contributions to the fem…

[Anatomy and basic knowledge of the spinal cord].

Basic knowledge of macroscopic anatomy is needed to understand the clinical signs and distinct syndromes resulting from the disturbed function of the tracks, segments and roots of the spinal cord. The cord is contained within the vertebral canal, continuing the medulla down to the conus medullaris. Descending tracks are motor (corticospinal and extrapyramidal), and ascending tracks are sensory (spinothalamic and lemniscal). Spinal cord involvement must be considered when a sensorimotor spinal tract and a radicular spinal syndromes are combined. Syringomyelia, Brown-Séquard syndrome and subacut…

The older patient with a spinal cord injury.

The most common cause of spinal cord injury (SCI) in older persons is falls, followed by motor vehicle crashes and pedestrian/motor vehicle crashes. Upper cervical injuries, particularly central cord syndrome, are prominent in the geriatric patient population. In addition, the mortality is higher, the complications are life threatening, the hospital stay is longer, and the cost of care is significantly increased for the older trauma victim.

Extraneural metastasizing ependymoma of the spinal cord.

This paper reports a case of the rare entity of an extraneural metastasizing ependymoma of the spinal cord. The tumor which arose in the conus medullaris and in the cauda equina was first diagnosed in 1956 when a thoracolumbar myeloresection was performed. At autopsy, 40 years after the primary diagnosis, a massive local tumor recurrence with extraneural metastases in the lungs, the pleura, the liver, and the thoracal and abdominal lymph nodes were found. Immunohistochemical stains of the extraneural metastases showed a strong cytoplasmatic expression of glial fibrillary acidic protein (GFAP).…

Breathlessness and exercise in spinal cord injury.

After spinal cord injury (SCI), breathlessness during daily activities is common. In 308 individuals with SCI, the authors measured pulmonary function and administered a survey regarding health status, participation in wheelchair athletics, and breathlessness during different activities. The following questions were included: A. Are you troubled by shortness of breath when hurrying on the level or going up a slight hill?; B. Do you have to go slower than people of your own age on the level because of breathlessness?; C. Do you ever have to stop for breath when going at your own pace on the lev…

Solitary fibrous tumor of the spinal cord.

We report a case of primary solitary fibrous tumor occurring in the intramedullary thoracic spinal cord in a 47-year-old man. The tumor predominately consisted of spindle cells separated by abundant collagen; a few areas of hemangiopericytomatous morphology were also present. The diagnosis was confirmed by immunohistochemistry and electron microscopy. The tumor was reactive to vimentin and CD34 but was negative for glial fibrillary acid protein (GFAP), S-100, smooth muscle actin, epithelial membrane antigen, HMB-45, myelin basic protein, and keratin; ultrastructural examination showed fairly u…

Spinal cord lesions in patients with AIDS.

In the past, little attention has been given in the imaging literature to spinal cord findings in the human immunodeficiency syndrome. This article summarizes the major pathologic conditions found in the spinal cord in patients with AIDS and compares them to MR features. The details of toxoplasma myelitis, spinal cord lymphoma, HIV myelitis, and tract pallor are demonstrated. A review of literature and a description of the pathologic cord changes will help the reader understand the alterations of the spinal cord in patients with AIDS.

Intramedullary tuberculoma of the spinal cord with syringomyelia.

We describe a patient with a presumed intramedullary tuberculoma treated only with antituberculous therapy. MRI showed a ring-enhancing annular lesion in the thoracic spinal cord with a distant syringomyelic cavity, which improved after therapy. MRI in this case allowed us not only to achieve a presumptive diagnosis but also to assess the therapeutic response, making surgical exploration unnecessary.

[Lesions of the spinal cord in HIV infection].

Neurological lesions are frequent complications of human immunodeficiency virus (HIV) infections. Organs involved include the brain, peripheral nerves and muscles. Since the widespread use of immunodepressive agents, spinal cord complications have also appeared although poorly documented in the literature. We observed six cases of spinal cord involvement which help indicate the modalities of practical management. In the first case, a 45-year old HIV1 + male presented dysesthesia evolving progressively over the T10 to L2 zones leading to the diagnosis of spinal cord toxoplasmosis. A gait disord…

Disorders of spinal cord, nerve, and muscle.

Substance abuse may lead to acute or chronic injuries to the spinal cord or PNS in addition to injuries caused by trauma and infection. The most widespread of these conditions are the chronic myopathy and chronic peripheral neuropathy of alcoholism. Less common, but more striking, illnesses attributed to substance abuse include ATM, a subacute myeloradiculoneuropathy resembling combined systems disease, Guillain-Barré syndrome, plexitis, fulminant axonal neuropathy, and acute myoglobinuria with or without severe weakness. If the cause of such syndromes is not otherwise evident, careful probing…

The urological manifestations of the tethered spinal cord.

The tethered cord is the fixation of the cord resulting in stretching as growth occurs. In this paper, three cases of tethered cord with symptoms related to the urinary tract were presented. In the first case, a 12-year-old girl presenting with abdominal pain and urinary incontinence had bilateral hydronephrosis and neurogenic bladder due to a tethered cord without having any other neuropathological manifestation. In the second case, an eight-year-old girl presented with enuresis and a mass in her back was found to have a lipomyomeningocele, hyperactive tendon reflexes in the lower limbs and p

Injuries to the spinal cord in elderly patients.

There are certain differences between the mechanism of injury and the course and results of treatment of post-traumatic spinal cord injuries in younger and older victims. Between 1965 and 1990 there were 564 patients over 60 years of age with spinal cord or cauda equina injuries treated at the Spinal Cord Centre in Konstancin, Poland. This article presents the level and degree of the nervous system injury, the methods of treatment and early mortality in the series. Of the patients, 43 per cent were aged between 60 and 65 years, but 31 per cent were patients over 70 years of age. More patients …

[Cavernous angioma of the cervical spinal cord].

A 34-year old man gradually developed a paraparesis which spontaneously regressed within a few months. One year later, the patient suffered from acute torticollis immediately followed by flaccid tetraparesis with pyramidal signs, dissociated sensory deficit and acute retention of urine. Magnetic resonance imaging (MRI) showed, in the cervical cord, a vascular malformation that was not opacified at angiography. The lesion, a cavernous angioma, was surgically removed, leading to recovery. So far, thirty five cases of spinal cord angioma have been published, most of them since the advent of MRI w…

The clinical diagnosis of disorders of the spinal cord.

Disease of the spinal cord or cauda equina may cause pain, motor abnormalities, sensory disturbance, alteration of reflexes and muscle tone, and bladder dysfunction. These five neurologic abnormalities occur in various combinations, evolve rapidly or slowly, and result in one of eight possible spinal cord syndromes. When a particular syndrome is defined by neurologic history and examination, differential diagnosis is limited to only a few possible disorders. An appropriate neuroradiologic or electrophysiologic procedure done at this point will usually provide the correct diagnosis.
