Biomechanical analysis of sandwich vertebrae in osteoporotic patients: finite element analysis.

OBJECTIVE: The aim of this study was to investigate the biomechanical stress of sandwich vertebrae (SVs) and common adjacent vertebrae in different degrees of spinal mobility in daily life. MATERIALS AND METHODS: A finite element model of the spinal segment of T10-L2 was developed and validated. Simultaneously, T11 and L1 fractures were simulated, and a 6-ml bone cement was constructed in their center. Under the condition of applying a 500-N axial load to the upper surface of T10 and immobilizing the lower surface of L2, moments were applied to the upper surface of T10, T11, T12, L1, and L2 a

Bilateral rib-to-pelvis Eiffel Tower VEPTR construct for children with neuromuscular scoliosis: a preliminary report.

BACKGROUND CONTEXT: Neuromuscular scoliosis could develop at a young age and progress beyond skeletal maturity. An early spinal fusion arrests growth of the spine and thorax, risking the development of secondary thoracic insufficiency syndrome. Vertical expandable prosthetic titanium rib (VEPTR) is a fusionless technique aiming at correction of the deformity with preservation of growth potential. PURPOSE: To demonstrate the preliminary results of the use of VEPTR in an Eiffel Tower construct in children with neuromuscular scoliosis in regard to coronal and sagittal profiles, space available f

Clinical and neuroradiological characterisation of spinal lesions in adults with Neurofibromatosis type 1.

Neurofibromatosis type 1 (NF1) manifests itself in many ways in the spine. This study aims to report the types of spinal lesions, clinical and demographic data in a large cohort from a complex NF1 centre. The characteristics of those with spinal neurofibromatosis, where neurofibromas are present on every spinal nerve root, were sought for comparison with the wider group of NF1 patients. This is a retrospective review of MDT minutes of 303 patients from a UK NF1 centre and the largest reported series of NF1 patients based on radiological data. Prevalence of each symptom and lesion was calculate

[Development of the spine after traumatic spinal cord injury in children and adolescents].

In the 1970s, there was growing concern about the risk of secondary deformations of the spine as these seemed to endanger the otherwise improving prognosis for the life of paraplegic subjects, especially of paralyzed children and adolescents. According to the literature, the level and extent of the spinal cord injury and the age at the time of injury are determinants of the development of scoliosis, hyperlordosis or global kyphosis. Correction of the deformity by brace orthosis is not indicated except for children. The indications for surgical intervention in terms of the extent of the scolios

Salivary Progesterone Levels in Female Patients with a History of Idiopathic Scoliosis: A Retrospective Cross-Sectional Study.

Idiopathic scoliosis is a condition characterized by a three-dimensional curvature of the spine. However, in addition to the spinal curvature, it has also been reported that patients with idiopathic scoliosis can display other abnormal physiologic parameters, such as hormone imbalances, genetic variants, and micronutrient deficiencies. The present study evaluates the salivary progesterone levels from a historical cohort of patients seeking treatment at a single integrative medicine clinic. A group of female patients with a history of idiopathic scoliosis is compared to a group of non-scoliosis

The role of muscles and effects of load on growth.

If mechanical modulation of growth explains progression of scoliosis during adolescent growth, two elements of the 'vicious cycle' must be quantified. Firstly we must identify the magnitude of the asymmetrical loading imposed on the spine as a function of the scoliosis curve magnitude. Secondly, we must characterize the growth response of vertebrae and remodeling response of discs to this asymmetrical loading. Animal models are helpful to quantify the former, and extrapolation to the human spine should be possible once the parameters that influence the growth modulation response have been iden

Accelerated Protocols in Adolescent Idiopathic Scoliosis Surgery.

Adolescent idiopathic scoliosis (AIS) is the most common form of scoliosis. Treatment depends on the degree of curvature, skeletal maturity, and age of the patient. Once the curve reaches 50 degrees, posterior spinal fusion (PSF) is necessary to stabilize the spine and prevent further progression of the curve. PSF causes significant trauma to the tissues and often results in significant pain postoperatively. The purpose of this article is to provide the audience with a review of preoperative, intraoperative, and postoperative pain control with an accelerated protocol in patients with AIS under

Scoliosis Management for Primary Care Practitioners.

For many families, the possibility that their child may have scoliosis causes great anxiety because their child may be deformed for life, may need to wear a brace for years, or may need to undergo a large and dangerous operation. For most families, these fears are groundless. Up to 3% of the population has a spinal curvature, most of which are small curves that may not need referral or repeated imaging. Many adolescents with scoliosis do well and do not need to wear a brace or have surgery.

Unsupervised local center of mass based scoliosis spinal segmentation and Cobb angle measurement.

Scoliosis is a medical condition in which a person's spine has an abnormal curvature and Cobb angle is a measurement used to evaluate the severity of a spinal curvature. Presently, automatic Existing Cobb angle measurement techniques require huge dataset, time-consuming, and needs significant effort. So, it is important to develop an unsupervised method for the measurement of Cobb angle with good accuracy. In this work, an unsupervised local center of mass (LCM) technique is proposed to segment the spine region and further novel Cobb angle measurement method is proposed for accurate measuremen

Kinematic Magnetic Resonance Imaging Assessment of the Degenerative Cervical Spine: Changes after Anterior Decompression and Cage Fusion.

Study Design A prospective cohort study. Objective Decompression and fusion of cervical vertebrae is a combined procedure that has a high success rate in relieving radicular symptoms and stabilizing or improving cervical myelopathy. However, fusion may lead to increased motion of the adjacent vertebrae and cervical deformity. Both have been postulated to lead to adjacent segment pathology (ASP). Kinematic magnetic resonance imaging (MRI) has been increasingly used to evaluate range of motion (ROM) of the cervical spine and ASP. Our objective was to measure ASP, cervical curvature, and ROM of i

Anterior and posterior variations in mechanical properties of human vertebrae measured by nanoindentation.

Osteoporotic spinal fractures are a significant global public health issue affecting more than 200 million people. Local degradation of the mechanical properties of bone and changes in global spine curvature increase fracture risk. However, a gap in knowledge exists relating material properties of trabecular bone in different regions of the spine. The purpose of our project was to measure the intrinsic mechanical properties of the anterior and posterior regions of human vertebral bodies in the thoracic and lumbar spine. Nanoindentation was used to evaluate Young's modulus (E) and hardness (H)

[Chronological changes in the operative indications and approaches for the treatment of spondylosis deformans of the spine].

Spondylosis deformans of the spine occurs due to degenerative changes of the intervertebral disc, vertebral body and hypertrophy of the interarticular joint. Such changes, including narrowing of the disc space, degeneration of the endplate, spur formation, hypertrophy of the facet joint, spondylolisthesis, abnormal curvature of the spine, are generally observed in the elderly people. Neurological abnormalities may develop because of spinal cord compression or circulation disturbances. In such cases, surgical intervention may be considered. Significant decompression of the spinal cord and root

Radiographic assessment of immediate postoperative lumbar lordosis redistribution after pedicle subtraction osteotomy with pre-contoured rods: A retrospective study.

BACKGROUND: Sagittal alignment is a major determinant of clinical outcomes in adult spinal deformity surgery. This study assessed the effect of pre-contoured rods (40°, 65°, or 90°) with a short-radius bend on postoperative lumbar lordosis after lumbosacral fusion with Pedicle Subtraction Osteotomy (PSO). This radiographic study was designed (1) to assess immediate postoperative redistribution in Global Lumbar Lordosis (LLGlobal) and L1S1 Lumbar Lordosis (LLL1S1) after L4 or L5 PSO with pre-contoured rods, (2) to quantify the mismatch (ΔL) between postoperative lordosis and rod curvature, (3)

Minimally Invasive Placement of Pedicle Screws Using Robotic-Assisted Navigation and Magnetically Controlled Growing Rods in a Patient with Early-Onset Scoliosis: Technical Note and Case Report.

INTRODUCTION: Early-onset scoliosis (EOS) refers to spinal curvature exceeding ten degrees in the coronal plane in patients under 10 years old. When non-operative management fails to control the curvature, surgical intervention may be indicated. In younger patients, growth-friendly instrumentation may be necessary to allow for continued spinal growth while controlling the curve, which includes magnetically control growing rods (MCGR). This paper is the first description of robotic-assisted navigation in a patient with EOS undergoing MCGR insertion with the minimally invasive placement of pedic

Validation of a novel handheld 3D ultrasound system for imaging scoliosis - phantom study.

Use of 3D ultrasound (US) scanners to detect and monitor scoliosis have been validated. The Cobb angle, axial vertebral rotation, spinal flexibility, curvatures in the sagittal profile and the Cobb angle on the plane of maximum curvature (PMC) can be measured from coronal, transverse and sagittal planes of ultrasound images. However, traditional 3D ultrasound scanners are relatively bulky and expensive. 2D US handheld and low-cost scanners are widely available. To adapt the 2D scanners for scoliosis applications, a position and orientation system is integrated with the scanner. The objective o

Laminoplasty with preservation of posterior cervical elements: surgical technique.

OBJECTIVE: We have developed a modified laminoplasty procedure that preserves the posterior cervical elements, and we have used it to treat multilevel cervical canal stenosis, with or without spondylotic changes, in 30 patients. METHODS: In this procedure, the posterior bony elements, such as the spinous processes, are not completely detached from the ligaments and musculature and are repositioned in the midline, to maintain normal cervical anatomic relationships. Thirty patients (mean age, 55.2 yr) were registered and evaluated in this study, with a mean follow-up period of 18.1 months, betw

Reduction of Adolescent Idiopathic Scoliosis Utilizing the Labyrinthine Righting Reflex: A Case Report.

Adolescent idiopathic scoliosis (AIS) remains a significant clinical challenge due to limited nonsurgical interventions and inconsistent treatment protocols. Current standard practices, including observation, rigid bracing, and spinal fusion surgery, often overlook patients with mild to moderate curvature, contributing to progression and decreased quality of life. This retrospective single case report evaluates a nonsurgical treatment integrating a cantilever traction system, weekly spinal mobilization, and reflexive engagement of the labyrinthine righting reflex (LRR) in an adolescent female

Butylated hydroxytoluene (BHT) induces zebrafish spinal cord defects and scoliosis by inhibiting the hedgehog pathway.

Butylated hydroxytoluene (BHT) is widely employed as an antioxidant in various industries. However, concerns persist regarding its safety and environmental impact, as its toxicological mechanisms remain poorly understood. Zebrafish embryos were exposed to BHT at 3, 5, and 7 mg/L to assess developmental toxicity. BHT exposure dose-dependently induced spinal cord malformations concomitant with reduced larval motility. Mechanistically, BHT suppressed Hedgehog signaling, evidenced by dysregulation of Shha, Gli1, and Smo. Notably, pharmacological activation of the Hh pathway by SAG rescued both str
